About this file

This document is a Request for Quote (RFQ) issued by the Illinois Veterans Home (IVHQ) for the procurement of Vanish Point Insulin Syringes (or equal) for the fiscal year 2024 (July 1, 2023 - June 30, 2024). The RFQ specifies the required syringe features, including 1CC, 29-gauge x 1/2 inch size with automatic needle retraction, and estimates the order quantity at 40 cases of 100 syringes per box. Vendors are required to submit quotes by the due date listed in the BidBuy system, and the selected vendor must be able to provide all required supplies and offer the lowest price.

The pricing terms outlined in the RFQ include the requirement for all prices to include shipping and handling charges, with no price increases accepted. The RFQ also reserves the right for the agency to reject all offers and individual offers for failure to meet any requirement, and to cancel the procurement up to 72 hours in advance. Additionally, the RFQ specifies that the selected vendor must adhere to the COVID-19 protection requirements set forth in the Governor's Executive Orders, which may include the use of face coverings, COVID-19 vaccinations, or testing.
