About this file
This document is a Request for Quote (RFQ) issued by the Illinois Department of Veterans Affairs for the purchase of Vanish Point Insulin Syringes. The RFQ specifies the required syringe type, size, and features, as well as the estimated annual quantity of 48,000 syringes to be delivered in cases of 100 syringes per box and 8 boxes per case. The contract term is for Fiscal Year 2022, from July 2021 to June 2022, with orders to be placed throughout the year. Vendors must submit their quotes by the stated due date, and the award will be made to the vendor with the lowest total bid.
The RFQ outlines the pricing terms, which include a requirement for all-inclusive pricing that covers shipping and handling, a minimum 1-year warranty on the products, and a guarantee of the lowest price for any price decreases during the contract term. The agency reserves the right to reject any or all offers and to award the contract by item, part, or portion of an item, group of items, or total, as well as to waive minor defects. Vendors are also required to agree to partial shipments in the event of a delay on the total order.
