Statement of Work Camp Dawson
Demarcation Relocation
WVNG-JFHQ-DES
1. Purpose And Scope
1.1. The purpose of this statement of work (SOW) is to define the requirements for the relocation of frontier network demarcation equipment located in room 2111, Bldg 1001 (Operations Building) Camp Dawson, WV. This is a non-personnel services contract to Engineer, Furnish, Install and Test (EFI&T) a solution to relocate equipment between room 2111and room 1220 and maintain all service provided by the vendor and equipment. The Contractor should furnish all necessary tools, labors, transportation, equipment’s, investigation, and supervision, etc. Work will be performed within in fixed-price contract. This document will also define requirements, equipment, and confidentiality.
1.2. Place Of Performance

1.4. Background
Commercial services providers’ demarcation point with the West Virginia National Guard (WVNG) is located in Building 1001a, RM 2111. The requirement is to move the demarcation location from room 2111 on the second floor to the first-floor room 1220, the current cable entrance room.
1.5. General Information
1.5.1. Place and Performance of Services: Routine work efforts performed shall be performed during normal site operating hours. The normal site operating hours are 0730-1600 except on recognized US holidays or when the Government facility/installation is closed due to local or national emergencies, administrative closings, or similar Government-directed facility/installation closings. The contractor shall always maintain an adequate work force for the uninterrupted performance of all tasks defined within this SOW when the Government facility/installation is not closed for the above reasons. When hiring personnel, the contractor shall keep in mind that the stability and continuity of the work force are essential.
1.5.2. The contractor’s employees shall become familiar with and obey the regulations of the installation, including fire, traffic, safety and security regulations while on the installation. Contractor employees should only enter restricted areas when required to do so and only upon prior approval. All contractor employees shall always carry proper identification with them and shall be subject to such checks as may be deemed necessary. The contractor shall ensure compliance with all regulations and orders of the installation, which may affect performance. The Government reserves the right to direct the removal of an employee from Government property or revoke access to Government systems for misconduct, security reasons, or any overt evidence of communicable disease. Removal of contractor employees for reasons stated above does not relieve the Contractor from responsibility for total performance of this contract.
1.5.3. Recognized Holidays: The following are recognized United States (US) holidays. The contractor shall not perform services on these days:

1.5.4. Installation Access and Security Requirements. The contractor shall comply with all applicable installation/facility access and local security policies and procedures, which may be obtained from the COR. The contractor and all associated subcontractor employees shall provide all information required for background checks to meet installation access requirements to be accomplished by installation Security Office. The contractor shall ensure compliance with all personal identity verification requirements as directed by Department of Defense (DoD), Headquarters Department of Army (HQDA) and/or local policy. Should the Force Protection Condition (FPCON) change, the Government may require changes in contractor security matters or processes.
All contractors and all associated subcontractor employees entering a restricted or classified area will always be escorted. Names, birth date, birth place and social security number will need to be submitted to the COR before personnel can access area.
1.5.4.1. For contractors requiring Common Access Card (CAC). Before CAC issuance, the contractor employee requires, at a minimum, a favorably adjudicated National Agency Check with Inquiries (NACI) or an equivalent or higher investigation in accordance with Army Directive 2014-05. The contractor employee will be issued a CAC only if duties involve one of the following: (1) Both physical access to a DoD facility and access, via logon, to DoD networks on-site or remotely; (2) Remote access, via logon, to a DoD network using DoD-approved remote access procedures; or (3)
Physical access to multiple DoD facilities or multiple non-DoD federally controlled facilities on behalf of the DoD on a recurring basis for a period of 6 months or more. At the discretion of the sponsoring activity, an initial CAC may be issued based on a favorable review of the FBI fingerprint check and a successfully scheduled NACI at the Office of Personnel Management.
1.5.4.2. For contractors that do not require CAC but require access to a DoD facility or installation. Contractor and all associated sub-contractors employees shall comply with adjudication standards and procedures using the National Crime Information Center Interstate Identification Index (NCIC-III) and Terrorist Screening Database (TSDB) (Army Directive 2014-05/AR 190- 13), applicable installation, facility and area commander installation/facility access and local security policies and procedures (provided by government representative).
1.5.4.3. All contractor employees, to include subcontractor employees, requiring access Army installations, facilities and controlled access areas shall complete AT Level I awareness training within 15 calendar days after contract start date. The contractor shall submit certificates of completion for each affected contractor employee and subcontractor employee, to the COR or to the contracting officer, if a COR is not assigned, within 14 calendar days after completion of training by all employees and subcontractor personnel. 1.5.4.4. Per AR 530-1 Operations Security, the contractor employees must complete Level I OPSEC Awareness training.
1.5.4. Post Award Conference/Periodic Progress Meetings: The contractor agrees to attend any post award conference convened by the KO in accordance with FAR 42.5. The KO, COR and other Government personnel, as appropriate, may meet periodically with the contractor to review the contactor’s performance. At these meetings, the KO will apprise the contractor of how the Government views the contractor’s performance and the contractor shall apprise the Government of problems, if any, being experienced. The contractor shall resolve outstanding issues raised by the Government. Contractor attendance at these meetings shall be at no additional cost to the Government.
1.5.5. Protection of Personally Identifiable Information (PII): The contractor shall protect all PII encountered in the performance of services in accordance with Defense Federal Acquisition Regulation Supplement (DFARS) 224.103 Personally Identifiable Information and Department of Defense Directive (DoDD) 5400.11, Department of Defense Privacy Program, and DoD 5400.11-R. If a PII breach results from the contractor’s violation of the a forementioned policies, the contractor shall bear all notification costs, call-center support costs, and credit monitoring service costs for all individuals whose PII has been compromised.

2. Project Execution
2.5. The Vendor shall coordinate with the government 5 days prior to arrival and the start of any on-site work. Prior to any on-site work, the PPO shall provide the Government
Multi-Function Team (MFT) a project coordination call to review all planned efforts and data collection requirements in support of project implementation.
This shall be a turn-key installation the Contractor shall provide all management, personnel, equipment, supplies, transportation, tools, materials, supervision, and other items and non-personal services necessary to Engineer, Furnish, Install and Test (EFI&T) the relocation of Frontier network equipment from room 2111 to 1220. The Contractor shall take the information and requirements outlined in the paragraphs below along with the pre-bid site visit to develop and present a solution.
2.6. Delays: Delays associated with working on an active military installation are at the Contractor's risk.
2.7. Timeline: The Vendor will include schedule/timeline for project completion in bid proposal.
2.8. Hours of Operation: Routine work efforts performed shall be performed during normal site operating hours. The normal site operating hours are Monday thru Friday, 0730- 1600
2.9. Site Acceptance: Site acceptance is required prior to final invoice. Site acceptance shall be concluded upon receipt of all deliverables and successful completion of the hardware and equipment testing.
2.10. Maintenance Warranty: All contractor provided equipment must have a minimum
12- month warranty included with the Firm Fixed Price. This 12 months will start after the site acceptance is complete.
2.11. Service and Support: Contractor must provide 12 months of service support for trouble shooting of hardware.
2.12. Frontier Equipment:
2.12.4. Vendor will place new Rad nids, power plant, DS-1 system, and battery backup in the newly installed IT Racks located in room 1220.
2.12.5. Vendor will ensure all working services to room 1220.
2.12.6. Vendor will move all POTS lines to the Copper terminal to room 1220.
2.12.7. Vendor will remove all Frontier legacy equipment form 1220.
