About this file

This document appears to be a Front Desk Screening Questionnaire for the Department of Mental Health (DMH) facilities. It outlines the procedures for screening employees for COVID-19 symptoms and potential exposure prior to beginning their work shifts. The questionnaire covers temperature checks, symptoms, recent travel, and potential close contacts with confirmed or suspected COVID-19 cases. Based on the responses, the document indicates whether the employee should be sent home or allowed to continue working, with additional testing and masking requirements for fully vaccinated employees. The document requires the initial of the staff member conducting the screening and notification to the Center Director if an employee is sent home.

The document does not contain information about a state or local contract opportunity. It appears to be an internal administrative document related to COVID-19 screening procedures for DMH employees.
