Labor categories required include maintenance technicians, HVAC mechanics, electricians, plumbers, carpenters, and painters for both residential properties and the Consulate compound. The Government furnishes facilities, equipment, materials, utilities, and telephone services; the Contractor provides all additional equipment, tools, uniforms, and supplies. Performance standards include 100% emergency response compliance (60-second phone answer, 4-hour on-site arrival), ≥95% routine work order completion within 5 working days, ≥98% preventive maintenance completion per GMMS schedules, zero preventable system failures, 100% apartment make-ready completion before occupancy, and ≤5 customer complaints monthly. Deliverables due within specified timeframes include Quality Control Plan, licenses and certificates, Personal History Statements, Standard Operating Procedures, Safety Plan, Hazardous Material Inventory and Control Plan, Performance and Payment Bonds, Insurance Evidence, weekly inspection reports, monthly work schedules, quarterly work order summaries, and waste disposal reports. The Quality Assurance Surveillance Plan establishes Government monitoring through direct observation, GMMS work order review, periodic facility inspections, complaint documentation, deliverable review, monthly invoice review, weekly performance meetings, and 100% inspection of emergency and high-risk work. The COR maintains monthly performance ratings (Exceptional, Satisfactory, Marginal, Unsatisfactory) and may withhold payment for wage/supplier amounts, material costs, defective work correction, or liquidated damages. Sustained or repeated deficiencies may result in contract termination.

4.4 Complaint Handling Procedures

1. Government personnel who observe unacceptable services contact the COR immediately.

2. The COR documents the complaint in writing.

3. The COR determines validity:

o Invalid complaint: COR advises complainant and retains annotated copy.

o Valid complaint: COR notifies Contractor orally (at minimum) and in writing;

provides reasonable time for correction.

4. The Contractor investigates and may challenge the complaint's validity by notifying the

COR.

5. The COR makes a final determination on validity.

6. Complaints are considered resolved unless the complainant notifies the COR otherwise.

4.5 Performance Deficiency and Corrective Action Process

Step Action Responsible Party

1 COR identifies performance deficiency through surveillance or complaint

COR

2 COR notifies Contractor in writing of deficiency and required corrective action

COR

3 Contractor initiates corrective action promptly; corrects deficiency within timeframe determined by COR

Contractor

4 COR verifies correction and documents resolution COR

• Prospective liquidated damages when the Contractor fails to make adequate progress

4.7 Performance Rating Summary

The COR will maintain a monthly performance record using the following rating scale:

Rating Description Criteria

Exceptional Performance exceeds all standards

Zero complaints; 100% on-time deliverables; all PMIs completed; zero emergency response failures

Rating Description Criteria

Satisfactory Performance meets all standards

≤5 complaints/month; ≥95% routine WO on-time; ≥98% PMI on-time; all emergency responses within 4 hours

Marginal Performance partially meets standards;

corrective action required

6–10 complaints/month; 85–94% routine WO on-time; one emergency response failure

Unsatisfactory Performance fails to meet standards; formal action required

>10 complaints/month; <85% routine WO on-time; repeat emergency response failures; repeat complaints for same deficiency

4.8 Documentation and Records

The COR shall maintain the following records throughout the contract period:

• All task orders issued and associated inspection reports (DS-0127)

• Contractor Service Report forms for unscheduled and emergency services

• Complaint documentation and resolution records
