About this file

The document is an Official Letter of Intent (Form MBD-40) issued by the City of Tampa as part of its Equal Business Opportunity Program, specifically designed to document the intent of a prime contractor to utilize a Minority/Small Local Business Enterprise (WMBE/SLBE) subcontractor. This form is a critical component of the bidding process for a veterinary services contract for the Tampa Police Department's K-9 unit, requiring detailed documentation of the subcontractor's scope of work, cost, and percentage of the total contract value. The form must be submitted within ten working days of the bid opening, and failure to do so may result in bid disqualification or award delay.

The form serves as a formal commitment between the prime contractor and the WMBE/SLBE subcontractor, requiring signatures from both parties and mandating the submission of a detailed subcontract agreement or purchase order prior to the commencement of work. The document is part of the City of Tampa's efforts to promote diversity and inclusion in municipal contracting by ensuring transparency and opportunity for smaller and minority-owned businesses. While the specific details of the veterinary services contract are not filled out in this template, the form is designed to capture critical information such as the scope of work, cost of services, and contact information for both the prime contractor and the subcontractor.
