About this file
This is a Debarment and Suspension Certification form related to the Seattle Office of Planning and Community Development's Legacy Homeowner Development Support contract opportunity in Washington State. The form serves as a certification document that contractors and vendors must complete to confirm their eligibility to participate in federally funded projects. The certification requirement applies to all entities seeking to submit proposals for either Workstream 1 (Legacy Homeowner Education & Networking) or Workstream 2 (Legacy Homeowner Pre-Development Technical Assistance), which are funded through a U.S. Department of Housing and Urban Development PRO Housing grant. Proposers must attest that they and their principals are not debarred, suspended, or otherwise ineligible to receive federal funds or conduct business with the government.
The form requires certifying officers to declare under penalty of perjury that the contracting entity has not been debarred or suspended by any federal agency and has not been excluded from federal contracts or covered transactions. Any entity that cannot make this certification is ineligible to bid on the contract. The debarment and suspension certification is a mandatory compliance requirement for all federal grant-funded procurements and must be submitted as part of the proposal package. Failure to provide this certification or providing false information may result in contract termination and potential legal consequences.
