About this file
This is a Form D Rate Schedule submitted by the State of Florida Department of Environmental Protection for the Petroleum Products Corporation Superfund Site Remedial Action Services contract. The schedule establishes fixed unit prices for a comprehensive environmental remediation project encompassing building demolition, utility management, soil excavation, in-situ soil stabilization/solidification (ISS) treatment, backfill and restoration, asphalt paving, well abandonment, and waste disposal services. The project involves 15 primary service categories with multiple line items, including demolition of seven buildings (D, E, G, H, I, J, L), excavation of approximately 31,890 cubic yards from designated areas, ISS treatment of approximately 93,022 cubic yards across shallow and deep zones, removal and replacement of utilities (stormwater, sanitary, water, gas, electric, and telecommunications), transportation and disposal of approximately 102,000 tons of materials, and site restoration including asphalt paving and landscaping. The contract includes both initial and renewal term sections with identical pricing structures, along with 10 contingency item categories addressing potential scope variations such as revised ISS mixtures, vapor and odor control systems, asbestos and RCRA abatement, lead abatement, and universal waste disposal.
The rate schedule requires respondents to enter fixed unit prices in purple-shaded cells for each line item; any blank cells or zeros result in those services being offered at no cost to the State. Rates serve as ceiling rates that may be reduced at the State's discretion through task assignments. The contractor must complete both the electronic (Excel format) and hard copy versions of this form without modifications, assumptions, or additional terms. The grand total of all base bid items is used for award scoring in accordance with Section 5 of the RFP. All contingency items require fixed unit price quotations for potential scope variations, including specialized pricing for material quantities (per cubic yard, linear foot, square foot, or ton) or shifts of work (eight-hour shifts for equipment operation). The document emphasizes strict compliance with submission requirements and prohibits any alterations to the pre-formatted schedule structure.
