About this file

The form requires an officer or representative of an entity submitting a bid, proposal, or entering into a contract with a governmental entity to attest that their organization is not owned by, organized under the laws of, or has a principal place of business in a Foreign Country of Concern, and that no such government has a controlling interest in the entity.

The form is a standard compliance document designed to ensure that contractors working with governmental entities do not have significant ties to specified foreign countries, particularly in contexts involving access to personal identifying information. It requires the bidder to declare under penalties of perjury that the statements made are true, with space provided for the authorized representative's printed name, title, signature, and date of signing. The form is part of a broader procurement process for American Sign Language Interpretation Services, though specific details about the contract's scope, value, or duration are not provided in this document.
