About this file

This document is a Certificate of Interested Parties (Form 1295) instruction and example form from the Texas Ethics Commission, applicable to state and local government contract opportunities. The document outlines a legal requirement implemented on January 1, 2016, mandating that businesses submitting bids or proposals must complete and file Form 1295 through the Texas Ethics Commission's website prior to contract approval by a city government. Bidders must obtain a certificate number online and include it in the designated box on the form, which will be required after bid evaluations and before City Council can consider contract approval.

The form requires detailed disclosure of interested parties, including the business entity's name and location, the governmental entity involved, contract identification information, and specifics about any interested parties with controlling or intermediary interests. Businesses must complete the form accurately, providing information about their company, the government agency, contract details, and any parties with significant involvement in the contract. The document provides links to additional resources (https://www.ethics.state.tx.us/whatsnew/elf_info_form1295.htm and https://www.ethics.state.tx.us/whatsnew/FAQ_Form1295.html) for further guidance on form completion, and includes an example blank form to illustrate the required information and formatting.

CERTIFICATE OF INTERESTED PARTIES

A new law in the state of Texas went into effect on January 1, 2016, which requires your firm to submit a Form

1295 (Certificate of Interested Parties attached) through the Texas Ethics Commission's website, and an original form as printed from the website to the City prior to approval of the contract. More information can be found at the following links:

https://www.ethics.state.tx.us/whatsnew/elf_info_form1295.htm.

https://www.ethics.state.tx.us/whatsnew/FAQ_Form1295.html

Once bid evaluations take place by city staff, you will be notified that an award to your company is pending and that this form is mandatory. You will need to provide this form, filled out and filed with the state of

Texas Ethics Committee, to the city before City Council approval can be considered.

You can fill out the form online, get a certificate number, and that number goes in the upper right box.
