About this file
The document is a Form 10 - Certification of Drug-Free Workplace Program for the MyFloridaNet-3 Core Services Invitation to Negotiate (ITN No. DMS-24/25-260) issued by the Florida Department of Management Services. The form outlines the state's requirements for businesses to establish and maintain a drug-free workplace program as a condition of bidding on state contracts. Specifically, the document details six key requirements for businesses, including publishing a statement prohibiting controlled substances in the workplace, informing employees about drug abuse dangers, providing employees with the policy statement, requiring employees to notify the employer of drug-related convictions, imposing sanctions on convicted employees, and making a good faith effort to maintain a drug-free workplace.
The form serves as a certification mechanism that gives preference to businesses with drug-free workplace programs when bids are otherwise equal in price, quality, and service. By signing this form, a respondent certifies full compliance with the state's drug-free workplace requirements, acknowledging that false statements are punishable by law. While the form itself does not contain specific pricing or contract details, it is part of the broader MyFloridaNet-3 procurement process, which aims to select a statewide telecommunications network infrastructure provider with an estimated annual enterprise expenditure of approximately $60 million.
