Project Requirements
3
Please clarify the dental services requirement, i.e., does it include evaluation and/or treatment?
Response:
Yes, includes evaluation and treatment.
Important Considerations
3
The Government is only interested in potential vendors who are active on the attached Approved Contract Vehicles list/ HHS Mandatory Vehicle Use List. However, none of the listed vehicles cover the full scope of performance requirements, nor do they cover work performed at a contractor site. Those that include relevant portions of the scope are structured in terms of labor rates (e.g., time and materials, fixed price level of effort, etc.) and would not apply to the prospective requirement for a firm fixed price managed service. Will the Government consider removing the requirement for vendors who are active on the Approved Contract Vehicles list and omit Q.1-3 in the Excel accordingly?
Response:
This is the same as Approved Contract Vehicle list/ HHS Mandatory Vehicle Use List. The list is no longer applicable.
Important Considerations
3
This paragraph references an attached Portable Document Format (PDF) file, HHS Mandatory Vehicle Use List. Please clarify if the Approved Contract Vehicles List is the same as the HHS Mandatory Vehicle Use List. If not, will the Government provide the HHS Mandatory Vehicle Use List as an amendment?
Response:
OAMS is referring to Approved Contract Vehicles list/ HHS Mandatory Vehicle Use List. RFI has been updated.
Submission Instructions 1. and 2.
3-4
Our understanding of the submission instructions is that the response will consist of two parts: 1) a capabilities statement providing the information requested in “2. Response Instructions,” limited to 10 double-spaced pages and 2) the completed Excel file, RFI Questions-FOHNet-updated 12-19-27. Please confirm that this is correct or provide clarification of instructions.
Response:
Yes, that correct. 1) Capabilities and 2) Response Instructions,” limited to 10 double-spaced pages and 2) the completed Excel file, RFI Questions-FOHNet-updated 12-19-27.
Past Performance Questionnaires
2.a), p. 4
A Past Performance Questionnaire (PPQ) form has not been provided and would be very time-consuming for both contractors and their customers at this phase in the procurement. Would you consider removing this requirement from the Sources Sought submission?
Response:
No, per sources sought, past performance questionnaires were listed as “if available”. Please, see under Submission Instructions, #1. Response Instructions, 4th paragraph beginning “with the word Note:”.
