About this file

This document is Addendum No. 1 for Invitation to Bid (ITB-003-26) issued by the Florida Department of Highway Safety and Motor Vehicles (FLHSMV) for Kiosk Registration Receipt Forms. The solicitation seeks thermal transfer paper rolls with 800 receipts per roll, including removable mounted decals for customer convenience kiosks in Tax Collector's Offices. The bid was originally issued on July 8, 2025, with questions due by July 15, 2025, and bids due on August 20, 2025, at 2:00 p.m. ET. The anticipated Notice of Intent to Award is scheduled for September 2, 2025. The contract appears to have provisions for potential multi-year terms with renewal options.

The current pricing for the registration receipt forms is $127.20 per roll, with the most recent bid tabulation showing Surys, Inc. as the potential awardee at $0.229 per unit. The addendum includes detailed questions and answers from potential vendors addressing specifics like color requirements, decal characteristics, and shipping considerations. The procurement allows for potential negotiation of terms with the selected vendor under Florida Statutes, and requires all colors to be printed as specified on both front and back of the receipt forms. The solicitation supports kiosk operations across multiple Florida counties and includes technical specifications for decals, including a requirement for square-cornered yellow retro-reflective decals.
