Original Date: May 7, 2024 Request for Proposal: May 15, 2024 Title: FLATBED CUTTER Requiring Activity Name: Defense Health Agency (DHA), Resources and Personnel Integration (R&PI J-1 Administration and Management, Integration Cell Division (ICD Requisition No.:

1.0 Scope

This award provides for a single one-time purchase to include delivery of one (1) Flatbed cutter, capable of cutting various kinds of media with high accuracy. Installation will be performed by the vendor at Defense Health Agency (DHA) in Falls Church, VA. The Contractor shall deliver the equipment within the delivery timeframe specified in its proposal and delivery schedule.

2.0 Objectives

The purpose of this procurement is to provide the DHA Duplication Center with the capability to cut media vice manually cutting media to deliver value-added media to DHA stakeholders.

3.0 Products and Services

The Contractor shall provide products as listed in Section 4.3 below. This requirement has been reviewed and contains no services that are inherently governmental functions. Any/all services are strictly non-personal in nature.

4.0 Delivery of Flatbed Digital Cutter

4.1 Delivery:

The COR and Technical POC will monitor the delivery of materials identified in Section

4.3 upon delivery.

The Contractor shall coordinate closely with the Government to reconsider any changes to an approved delivery schedule. The Contractor shall deliver all materials to the Defense Health Agency, Falls Church, VA. Point of Contact: Tyrone Davis.

The materials shall be delivered TBD. Products shall be delivered onsite via the loading dock and set up within the DHA Duplication Center. The Contractor shall deliver during normal business hours, which are 7:00 AM – 6:00 PM Monday through Friday. The Contractor shall be responsible for removing all trash from the site, as it accrues (not at the end of the period of performance).

4.3 Itemized List of Materials

The Contractor shall provide materials/products that are brand name or equivalent.

The Government requires the following materials (the Contractor is encouraged to use this list as a basis for pricing):

4.3.1 Product

Ongoing Support/ Warranty Program/ Plotter Instant/Full-time 800# phone support from certified specialists 2-year factory warranty on plotter from day of installation Certified Technicians available on site within 3-5 days of diagnosis Software revision upgrades & firmware flashes

4.3.4 The quantities annotated above are not to exceed quantities.

5.0 Requirements

5.1 The Contractor shall have access to the Flatbed Cutter within the workspace as follows.

The hours for onsite delivery of materials shall be 7:00 AM - 6:00 PM. The Contractor shall coordinate with the Government COR and/or Technical POC with regards to scheduling the delivery of the Flatbed Cutter. The Government anticipates delivery between Monday-Friday standard business hours.

The Government will not pay for any temporary storage of items and will not require any items to be stored prior to the delivery.

6.0 Warranty

The Contractor shall provide a minimum of 2-year factory warranty on plotter from day of installation.

7.0 General Duties/Requirements

General duties and requirements to be fulfilled by the Contractor are as follows:

7.1 For all phases of this procurement, the Contractor shall be responsible for managing the delivery of the materials on schedule and in accordance with contract requirements. This includes coordination with the COR and DHA Facility Management Office.

7.2 The Contractor is responsible to follow and comply with all job site access rules and general job site rules and regulations. The contractor is responsible for the protection of the office facility during delivery and shall reimburse the Government for costs associated with repairs due to the delivery/replacement effort for any damage caused by the Contractor. It is incumbent upon the Contractor to identify any existing office damage prior to the commencement of delivery/assembly and notify the DHA Facility Management Office.

7.3 Contractor Equipment

7.3.1 The Contractor shall furnish any materials handling equipment, i.e., pallet jacks, dollies, etc. necessary for completion of the delivery. The Government will not be held responsible for loss or damage to Contractor property left unattended/unsecured.

7.4 No relief will be given to the Contractor for services/materials outside this scope of work or performed without the Contracting Officer’s written approval.

7.5 The Contractor shall provide its own communication devices, if necessary, during the project.

7.6 Acceptance And Invoicing

7.6.1 Upon delivery of the materials, the COR and Technical POC will inspect all items to ensure that they meet all acceptable standards of quality and workmanship as specified in the Contractor’s quote and this SON. The COR and Technical POC will also monitor whether the delivery is IAW the approved delivery schedule.

7.6.2 Upon full acceptance, the COR and Technical POC will provide written acceptance of the material delivery. This signed statement is the acceptance of the work and services performed under this task order and shall be based upon COR and Technical POC determination that work performed is satisfactory.

7.6.3 The Contractor shall submit to the COR for approval the total fees in accordance with the contract. The Contractor shall submit an invoice and expect payment only after the COR provides written authorization approving products and services rendered.
