About this file
This document is a First-Tier Subcontractors Disclosure Form associated with a contract opportunity for Washington County Supportive Housing Services (WCSHS) in Oregon. The form is designed for prime contractors to list first-tier subcontractors who will be furnishing labor or labor and materials for a project to supply and construct 30 palletized shelters to be delivered to 2270 SW 198th Ave, Aloha, OR 97003. The form requires detailed disclosure of subcontractor names, dollar values, and categories of work.
The document emphasizes strict compliance requirements, noting that failure to submit this form within two work hours after bid closing will result in a non-responsive bid that will not be considered for award. The contracting agency is required to accept the subcontractor disclosure but is not obligated to verify its accuracy or completeness. Contractors have the option to substitute first-tier subcontractors under ORS 279C.585, and subcontractors may file complaints related to disclosure requirements under ORS 279C.590.
