About this file

This document is a First Amendment to the Guardianship Services Master Agreement Request For Response (RFR) issued by the Massachusetts Department of Mental Health (DMH). The amendment revises sections 4f, 7.3b, and 8.1.2.1 of the original RFR. Key changes include bidders being responsible for complying with Criminal Offender Record Information (CORI) and/or similar background checks required by the Probate Court, and updated requirements for bidder profiles and the review process to be considered "Otherwise Qualified." The overall RFR seeks to establish a pool of individuals and non-profit corporations from which DMH Legal can purchase guardianship and/or conservatorship services for individuals receiving services from or paid for by DMH.

The RFR requires bidders to obtain a Massachusetts Vendor Code and necessary insurance, as well as complete and submit various Commonwealth of Massachusetts standard forms when requested by DMH. No other amendments are being made to the original RFR.
