About this file
This is an Inspection Summary report documenting a joint commission fire alarm system inspection conducted at the Veterans Admin Outpatient Clinic located at 4303 Pittman St, Lawton, OK 73503 on January 20, 2026. The inspection was performed by Johnson Controls North America through Inspector Randy Reece (License #6392) under Work Order #60694676.
The inspection covered fire alarm system components including duct, heat, flame, and optical smoke detectors; audible and visual notification appliances; door releasing devices; supervising station fire alarm systems transmission equipment; and air handler shut down annual testing per NFPA 72-2010 standards. Seven device deficiencies were identified, primarily involving functional failures of audible visible devices (EST model 792-7A-006 and EGCT+HDVMH) on the first floor in multiple locations (hall by room 210, men's locker rooms 220 and 223, and mechanical rooms), and two duct detectors (ESD-C) in the new west area that lack sufficient zones to land zone 11. Additionally, one battery (2D12V7AH) in the FACP closet was found to be flat. A cannot inspect report was issued for the AHU Fan Shutdown device on the roof due to duct detector trouble. General inspection notes indicate the annual fire alarm inspection was completed, with five audio visual signal failures detected on EST 792-7A-600, main facp battery failure, and the fire alarm panel system tagged as yellow.
