This is a Solicitation Modification document issued by the Commonwealth of Kentucky, specifically the Department of Juvenile Justice, for tornado damage repairs at the Mayfield Youth Development Center. The project, identified as RFB-15-23, encompasses reconstruction of three structures: an Equipment Garage, a Maintenance Garage, and a Lawn & Garden Storage facility. The Equipment Garage work includes demolition and removal of existing siding, roofing, and trim, modification of framing for new overhead and personnel doors, and construction of a connector structure to the adjoining Maintenance Shop. The Maintenance Garage addition involves demolition of existing foundation elements, construction of a 12'-0" x 30'-0" addition with concrete slab, CMU block walls, wood stud framing, and roofing to match existing structures. The Lawn & Garden Storage facility requires complete demolition of the existing 24'-0" x 32'-0" equipment shed and construction of a new pole barn structure with separate equipment and tool storage areas. The solicitation closed on July 26, 2022, at 2:00 P.M. Contractors were encouraged to visit the site on July 14 or 15, 2022, between 9:00 A.M. and 3:00 P.M. Central Time. Questions regarding the project were to be submitted to Curt Swinford at CMS Architects by noon on July 18, 2022.

This modification represents Version 2 of the solicitation and includes Addendum One. The project is classified under Commodity Code 912 for Construction Services, General (including Maintenance and Repair). No specific unit pricing information is provided in the modification document; pricing was to be submitted by bidders in response to the complete solicitation package. All project documentation, including bid documents, specifications, and drawings, was available through the State Plan Room at no charge in digital format, with optional paid hard copies available. Bid submission was to be completed electronically through the Owner's Document Collaboration System (eCommunications), with no online bidding prohibited designation noted. All bidders were required to identify their firm's position as prime bidder, miscellaneous subcontractor, material supplier, or other entity. The Division of Engineering and Contract Administration, located at the Bush Building in Frankfort, Kentucky, served as the shipping and billing address for the project.
