About this file
This Request for Bid (RFB) is issued by the Commonwealth of Kentucky Transportation Cabinet (KYTC), Division of Purchases, on behalf of the Division of Right of Way and Utilities for grave relocation services in Bath County. The project involves the disinterment, relocation, and reinterment of an estimated five graves from an Unnamed Cemetery/Jacob Steele Grave Cemetery to Owingsville Cemetery, located approximately 1.5 miles south on KY 36. The solicitation requests a licensed funeral director to provide services including opening and closing graves, providing reinterment containers constructed of minimum 12-gauge steel, transporting remains and monuments, and providing state-specified monuments with matching inscriptions. Key dates include a written questions deadline of July 15, 2020 at 4:00 PM EST, bid closing on July 23, 2020 at 10:00 AM EST, and a tentative award date of August 7, 2020. All work must be completed within 30 days of receiving the work order notification, with graves disinterred and relocated within 10 days of the work order receipt. The contract is awarded as a one-time service with no renewal options.
Award will be determined using a best value approach based solely on price, with the lowest responsive bidder receiving the contract. The successful bidder must provide a performance and payment bond equal to 100 percent of the contract price (or a minimum of $20,000 if the contract amount is $20,000 or less), maintain general liability insurance with $1,000,000 per occurrence limits, and provide Kentucky workers' compensation insurance. Payment terms stipulate that 90 percent of the contract amount will be paid upon work completion, with the final 10 percent paid after the Grave Relocation Agent determines all work is satisfactory and no property damage has occurred. A reciprocal preference for Kentucky resident bidders applies, and all subcontracting requires prior written approval from the Director of the Division of Right of Way and Utilities. Bidders must submit sealed bids via U.S. Postal Service, UPS, Fed Ex, or email to KYTC.Bids@ky.gov, including a signed solicitation cover page, completed bid information, certificate of authority (for foreign entities), required affidavits, performance and payment bond, and certificate of insurance.
