About this file
This is a Required Forms List (Appendix A) for the Forest Hills Public Schools Digital Copiers RFP 2021, prepared by Plante Moran on behalf of Forest Hills Public Schools in Michigan. The RFP seeks digital copier/multifunction devices (MFDs) with copying, scanning, printing, and fax capabilities compatible with Windows and Chrome environments. The school district requires quotes for two equipment categories: 90-100 pages per minute (PPM) copiers and 40-50 PPM copiers, with quantities distributed across multiple elementary schools, middle schools, high schools, administrative buildings, and support facilities. The district is replacing existing copiers installed in 2014-2018 across approximately 50 locations, with annual usage ranging from approximately 13,839 to 1,142,411 copies depending on location. The contract term is five years with warranty and trade-in included, and vendors must submit all required forms in Microsoft Excel format, with hard copies scanned as part of the vendor questionnaire.
Pricing must include capital costs for equipment with trade-in allowances, 60-month consumables and maintenance costs based on 80 million total copies, PaperCut management software implementation and five-year licensing, and optional consumables pricing for 90 million copies. Vendors must provide separate pricing for optional fax capability and two/three-hole punch add-ons for each copier model, as well as incremental costs for copier model substitutions and adjustments for copy volume variations. All required forms—including proposal summary, non-familial relationship affidavit, school safety affidavit, Iran economic sanctions compliance affidavit, vendor questionnaire, comply exception form, and pricing forms for both 90-100 PPM and 40-50 PPM copiers—must be completed and submitted. Failure to submit any required form may result in bid rejection. The district reserves the right to accept or reject proposals in whole or in part, award to other than the lowest bidder, and award to one or more contractors at its sole discretion.
