About this file

This document is a Questions and Answers (Q&A) document for Solicitation Number FHF-25-Q-0080, addressing a vision plan contract for the Federal Housing Finance Agency (FHFA). The solicitation involves a fully insured vision plan for FHFA and FHFA Office of Inspector General (OIG) employees, with a single plan design funded separately for each group. Key details include a $150 retail frame allowance with 20% off any overage, in-network provider coverage, and the cost of premiums to be fully paid by FHFA. The technical quote page limit was expanded from 10 to 25 pages to accommodate vendor responses, and the contract will be non-appropriated and fully funded by FHFA.

The Q&A document addresses various vendor inquiries, including network provider details, employee enrollment information, and administrative requirements. Specific responses clarify that employee zip codes will be used for network access, current enrollment data is available in Attachments B, D, and E, and vendors should quote based on the benefit table in Section 2.0 of the Performance Work Statement (PWS), which mirrors the current FHFA plan. The solicitation seeks a vision plan provider with comprehensive coverage and network accessibility within 35 driving miles of employees.

No, the plan is the same for both groups.

The separation in Attachment B is because each group is funded separately.

8 Please confirm if Walmart and Sam’s Club are in-network providers.

This is completely the vendor’s decision.

As stated in Section 2.0, all in-network vendors shall have coverage up to the full retail frame allowance of $150 plus 20% off any overage after the copay.

Please provide the Avesis benefit summary and/or the full certificate All vendors shall quote based on the benefit table provided in Section 2.0; which is unchanged from FHFA’s current plan.

To guarantee network access to a provider within 35 driving miles, as requested, we would require the employees home address. Is the FHFA willing to provide a census which includes home addresses?

Access is based on the employee’s zip code, which is provided in Attachment E.

The RFP specifies that the Technical Quote is restricted to 10 pages, inclusive of all nonprice factors and related attachments. Would FHFA consider accepting supplemental exhibits associated with the Technical Quote that extend beyond the stated 10-page limit?

Yes, the FHFA shall expand the page limit to 25 pages.
