B-243 FD Epoxy Floor Repair

1. Introduction
The facility currently has an epoxy floor system but, upon inspection it was found that the floor is in substandard condition. The floor contains major cracks, spalls, chips and adhesion issues. The new epoxy floor will be an epoxy flooring system that must be fuel, stain, solvent, and chemical resistant.
The floor will be applied at the nominal thickness of 3/16 in. Vehicle stop bars to be painted. (See attached files for specific measurements, colors and locations.)
1.1 Scope
The contractor shall provide all personnel, equipment, tools, materials, supervision, trash removal and quality control necessary to perform the epoxy floor repair for building 243 owned by the New
Hampshire Air National Guard.
2. General Requirements
This section describes the general requirements for this effort. The following sub-sections provide details of various considerations on this effort.
2.1 Non-Personal Services
The Government shall neither supervise contractor employees nor control the method by which the contractor performs the required tasks. Under no circumstances shall the Government assign tasks to, or prepare work schedules for, individual contractor employees. It shall be the responsibility of the contractor to manage its employees and to guard against any actions that are of the nature of personal services, or give the perception of personal services. If the contractor believes that any actions constitute, or are perceived to constitute personal services, it shall be the contractor's responsibility to notify the Contracting Officer
(KO) immediately.
2.2 Business Relations
The contractor shall successfully integrate and coordinate all activity needed to execute the requirement.
The contractor shall manage the timeliness, completeness, and quality of problem identification.
Contractor employees shall not loiter in any working or patron area. Upon completion of their assigned shifts employees shall promptly depart from the installation. The contractor shall provide corrective action plans, proposal submittals, timely identification of issues, and effective management of subcontractors.
The contractor shall seek to ensure customer satisfaction and professional and ethical behavior of all contractor personnel.
2.3 Contract Administration and Management
The following subsections specify requirements for contract, management, and personnel administration.
2.3.1 Contract Management
The contractor shall establish clear organizational lines of authority and responsibility to ensure effective management of the resources assigned to the requirement. The contractor must maintain continuity between the support operations at Pease Air National Guard Base (ANGB) and the contractor's corporate offices.
2.3.2 Contract Administration
The contractor shall establish processes and assign appropriate resources to effectively administer the requirement. The contractor shall respond to Government requests for contractual actions in a timely fashion. The contractor shall have a single point of contact between the Government and Contractor personnel assigned to support contracts or task orders. The contractor shall assign an onsite manager whom can make decisions in the absence of the contract holder.
2.3.3 Personnel Administration
The contractor shall provide the following management and support as required. The contractor shall provide for employees during designated Government non-work days or other periods where Government offices are closed due to weather or security conditions. The contractor shall maintain the currency of their employees by providing initial and refresher training as required to meet the SOW requirements. The contractor shall make necessary travel arrangements for employees. The contractor shall provide necessary infrastructure to support contract tasks.
2.4 Subcontract Management
The contractor shall be responsible for any subcontract management necessary to integrate work performed on this requirement and shall be responsible and accountable for subcontractor performance on this requirement. The prime contractor will manage work distribution to ensure there are no Organizational
Conflict of Interest (OCI) considerations. Contractors may add subcontractors to their team after notification to the Contracting Officer (KO) or Contracting Officer Representative (COR). Cross teaming may or may not be permitted.
2.5 Contractor Personnel, Disciplines, and Specialties
The contractor shall accomplish the assigned work by employing and utilizing qualified personnel with appropriate combinations of education, training, and experience. The contractor shall match personnel skills to the work or task with a minimum of under/over employment of resources. The contractor shall ensure the labor categories as defined in the Labor Categories document (see attachment), labor rates, and man-hours utilized in the performance of each Task Order (SOW line item) issued hereunder will be the minimum necessary to accomplish the task.
2.6 Location and Hours of Work
Accomplishment of the results contained in this SOW requires work at BLD-243 302 Newmarket Street, Pease ANGB, NH 03801.
Hours of operation are: Monday thru Friday
The following are recognized U.S. holidays. The contractor shall not perform services on the following dates, unless prior approval is obtained from the Contracting Officer (KO):

Period of performance. Requires the work to be accomplished by 30 September 2020.
3. Requirements
This section describes the requirements for this effort. The following sub-sections provide details of various considerations on this effort. The services will include the following;
3.1 Conduct moisture test
Conduct industry standard moisture testing prior to removal of coating. Test results above 95 percent relative humidity will require a special moisture mitigation primer application. The contractor will provide the program manager and contracting officer the results prior to moving onto sealing the floor, section 3.2
3.2 Removal of current coating:
The contractor is solely responsible for removal of the current coating and paint on the concrete floor.
The joint sealant will also be removed around the drainage system. The contractor will utilize a “shot blast system” to remove the coating and paint. Dust mitigation is a priority inside the fire station.
Vacuuming the floor after paint removal and before further installation efforts is required.
3.3 Repair of concrete and joint sealant
The contractor shall repair cracks and spalls on the floor and ensure a smooth finish. The contractor will remove existing joint sealant surrounding trench drains and replace with an elastomeric polymer approved for concrete floor applications. The contractor will ensure approval from the project manager to move onto section 3.4.
3.4 Paint floor lines
In accordance with the drawing attached, the contractor shall paint the floor. Provide paint samples in accordance with delivery schedule in section 5 of the SOW.
3.5 Seal Floor with Epoxy
Prior to the application of both the new epoxy and joint sealant. The entire work area must be clean, sound and dry. Contaminants such as previously applied sealants, form release agents, grease, oil, etc.
must be removed by scarifying, wire brushing, or sanding. Dust should be blown from joint locations with oil free moisture free compressed air.
The Floor finish shall be temperature, fuel, stain, solvent, and chemical resistant.
4. Special Requirements
This section covers specific on site safety and security needs of Pease AFB. The Contractor shall initial that they have read and understand each section.
4.1 Security and Safety
This section describes the security and safety for this effort. The following sub-sections provide details of various considerations on this effort.
4.1.1 SECURITY REQUIREMENTS
4.1.1.1 The contractor shall comply with all security regulations imposed by the base/post commander and/or agency occupying the space where work is to be performed. Any necessary security clearances shall be obtained prior to commencement of work.
4.1.1.2 It is expected that new security requirements to satisfy contractor personal identification requirements in Homeland Security Presidential Directive (HSPD-12), ''Policy for a Common
Identification Standard for Federal Employees and Contractors,'' and Federal Information Processing
Standards Publication (FIPS PUB) Number 201, ''Personal Identity Verification (PIV) of Federal
Employees and Contractors” are incorporated in the life of this contract. Contractor shall comply with all requirements, at no additional cost to the Government.
4.1.1.3 The contractor shall ensure that all parts of the facility where work is being performed are adequately protected against vandalism and theft.
4.1.1.4 Contractors shall follow instructions provided by the Security Forces for access to the North
Contractor Gate. If a gate pass is not granted to a contractor allowing the delivery of goods, execution of warranty support, or performance of services please contact the contracting officer. Be sure that all of your employees have the Real ID (Real ID driver’s license, state ID, immigration card, U.S. Passport, etc.). If they do not have the Real ID, they will be turned away. The REAL ID Act establishes minimum security standards for license issuance and production and prohibits Federal agencies from accepting for certain purposes driver’s licenses and identification cards from states not meeting the Act’s minimum standards. Several notices have been put out in the past and the base has deferred this requirement since
Jan 2014. However, beginning October 1, 2020, SFS will start enforcing the Act as part of the vetting process. As of last Thursday contractor are highly encouraged to submit REAL ID compliant documents or other acceptable identification as described below during the vetting process. Between now and 30
September contractors may continue to submit badging request with non-compliant IDs but those badges will expire on 1 October. More approved document for Real ID is at the following website:

4.1.1.5 The Prime Contractor and all of its subcontractors will be required to wear orange or lime colored safety vests while performing work on the installation.
4.1.1.6 U.S. Government will not be held liable for any delays or breach of contract caused by refusal of the Security Forces to allow entry.
4.1.1.7 All prospective contractors MUST be U.S. citizens or MUST have established and maintain legal residence in the U.S., and are authorized by the US government to work in the United States (i.e. Green card, worker authorization, etc.). All prospective contractors MUST have at least one form of U.S.
government issued photo identification (drivers license, U.S. passport, U.S. work visa, etc.) for entry to https://www.dhs.gov/real-id the installation. Security Forces will review any questionable identification documents for determination as to whether or not said identification is authorized for access to the installation.
4.1.1.8 All prospective contractors/contractor employees, once it has been established that they maintain the appropriate credential as described in one above, are subject to a Criminal History Check.
4.1.1.9 Note: For the purposes of Contracting services, it is implied that, that any and all parties agree to a
Criminal History Background Check once entered into any contract. Any documents that are provided and found fraudulent to include, but not limited to driver’s licenses, contracting licenses, etc., will result in an arrest entry onto the installation and while working on the installation. Any refusal to submit to a search of their person or vehicle will be deemed as a violent act of aggression against the installation and will be cause for dismissal from the installation and revocation of future privileges and installation access.
4.1.2 AT Level I Training. This standard language is for contractor employees with an area of performance within a Government controlled installation, facility or area. All contractor employees, to include subcontractor employees, requiring access to Government Installations, Facilities and Controlled
Access areas shall complete AT Level I awareness training within 10 calendar days after contract start date or effective date of incorporation of this requirement into the contract, whichever is applicable.
Certificates of completion for each affected contractor employee and subcontractor employee will be maintained by the COR or Antiterrorism Representative. AT level I Awareness Training is available through ADLS by completing the Force Protection CBT

4.2 Safety
Contractor Compliance: The contractor and its subcontractors shall comply with Public Law 91- 596
(Occupational Safety and Health Act (OSHA)) and the Environmental, Safety, and Occupational Health
(ESOH) (DODD 4715.1E). These requirements shall be incorporated into the contractor’s safety and health program. The Department of Defense (DoD) participates in the OSHA Voluntary Protection
Program (VPP). Contractor personnel performing services on a DoD installation shall participate in the local VPP. Information on the VPP is available at http://www.osha.gov/dcsp/vpp/index.html.
Mishap Notification and Investigation: The contractor and its subcontractors (if applicable) shall promptly report pertinent facts regarding mishaps involving Government property damage or injury to
Government personnel and to cooperate in any resulting safety investigation. The contractor shall notify
(via telephone) the cognizant contracting officer, the contracting officer’s representative, and/or other applicable members within four (4) hours of all mishaps or incidents. The Government person notified by the contractor will in-turn notify the Safety office. Contractor notifications made after duty hours shall be reported to the appropriate installation Command Post. If requested by the cognizant contracting officer, the contracting officer’s representative, and/or the cognizant program manager, the contractor shall immediately secure the mishap scene/damaged property and impound pertinent maintenance and training records until released by the investigating safety office. If the Government investigates the mishap, the contractor and the subcontractors shall cooperate fully and assist the
Government personnel until the investigation is completed.

4.3 Government Furnished Materials
Facilities. No alterations to the facilities will be made without specific written permission from the http://www.osha.gov/dcsp/vpp/index.html contracting officer.

4.4 Quality
This section describes the Quality Control components for this effort. The following sub-sections provide details of various considerations on this effort.
4.5 Contract Manpower Reporting Application (CRMA)
Contractors will begin reporting manpower data relating to the performance of services contracts into the
System for Award Management (www.sam.gov), consistent with existing service contract reporting requirements under the Federal Acquisition Regulation (FAR) that is currently applicable only to Federal civilian agencies. The new initiative will allow industry to report all manpower data into a single Federal-wide portal, removing duplicative requirements; one for DoD and another for the rest of the Federal
Government, which requires the same data elements to be reported from industry for services contracts performed for both DoD and Federal civilian agencies that essentially perform the same function. The outcome will be a process that is less burdensome on both industry and Government, while improving data integrity and accuracy.
