Attachment 1
STATEMENT OF WORK
WEEKLY/ MONTHLY/QUARTERLY ARSENIC; MONTHLY FLUORIDE; QUARTERLY TTHM/HAA5; MONTHLY SPECIFIC CONDUCTANCE, Chloride, TDS; ANNUAL NITRATE, ANNUAL CHEMICAL WATER COMPLIANCE SAMPLING ANALYSIS
SCOPE OF WORK: The contractor shall provide all personnel, equipment, sample bottles, tools, materials, supervision and other items and services necessary to perform analysis of water samples at Edwards AFB. Analysis is in compliance with the Safe Drinking Water Act (SDWA), as implemented by the California Department of Public Health pursuant to the California Code of Regulations, Title 22; as well as those required by Environmental Protection Agency (EPA). A list of tests is included at the end of this report.
SAMPLING:
All sampling, unless specified, will be analyzed within a timeframe that will permit all the corresponding reports to be delivered within thirty working days of receipt.
All priority samples will be confirmed in writing (telefax is acceptable) prior to analysis and will require a 24 (twenty-four)-hour analysis completion turnaround. Results will be telefaxed to (661-277-4835) within 24 hours after receipt of samples or another fax number designated by 95 AMDS/SGPB.
Contractor must contact 95 AMDS/SGPB (Environmental Element in Bioenvironmental Engineering, 661-277-3272) to obtain approval of any additional testing that may be required – prior to analysis. Additional test(s) done without approval, will not be paid for.
Contractor will comply with Level 2 Quality Assurance/ Quality Control requirements set by the EPA. The laboratory (ies) must be certified by the State of California.
Contractor is required to notify 95 AMDS/SGPB (661-277-3272), within 1 working day of discovery, in the event the Maximum Contamination Level is exceeded or if samples cannot be analyzed.
PROCEDURES FOR OVER-AND-ABOVE WORK (REFERENCE THE CONTRACT CLAUSE 252.217-7028):
The following procedures shall apply for all Over-and-Above Work Requests pertaining to this contract:
(1) The Government designated representative/Quality Assurance Personnel (QAP) shall request testing, not elsewhere priced in the contract, from the contractor for work at any dollar amount per occurrence.
(2) The QAP for this contract shall-
(a) Promptly review the testing request for technical acceptability and the reasonableness of the price.
(b) Verify that the proposed testing is required, appropriate, and is not covered under the basic contract line item(s).
(c) Verify that funding is available for the Over-and-Above Work request.
(d) Inform and coordinate with other appropriate government agencies on base as necessary, in a timely and professional manner.
(3) The Contractor must obtain approval from the QAP prior to performing the testing. Approval will be done orally. All oral approvals will be followed up in writing. The approval will establish a firm-fixed price for the testing. Failure to agree on the price of the Over-and-Above Work shall be a dispute within the meaning of the disputes clause of this contract. A contract modification will be executed to establish a price for the Over-and-Above Work less than 1 month from the approval date of the Work Request. The Government is NOT required to award the testing to the Contractor.

1. Weekly Arsenic
2. Monthly Arsenic
3. Quarterly Arsenic
4. Quarterly TTHM/HAA5
5. Monthly Fluoride
6. Monthly Specific Conductance, Chloride, TDS
7. Annual Nitrate
