Will the maintenance of the dermal cover with vegetation top cover require the Contractor to obtain a dig permit?

Yes - An approved 19 CES/CEOER Form 103 will be required prior to digging. Coordination with Base POC at least 2 weeks prior to activity is required.

PWS Section 6.1, LUC

Is any specialized training required to access the work area for work addressed under this section?

Airfield training (30 minute training) to access Site ST025 will be required. The Base POC will have to escort the contractor during work at this site. LF030 is an active EOD range and is a secure area. The Contractor will be required to work with the

Base POC for access to this site that can only be accessed when the site is not being used.

PWS Section 6.1

Maintenance/Repair

For work addressed under this section, what are the chemicals of concern in this area? Is any specialized training required (ie, OSHA, HAZWOPER, etc.)?

Seven of the eight sites have received NFA's from the state agency with restrictions for digging if potential contamination remains. COCs at ST025: Naphthalene and TPH GRO/DRO

LF030 - received NFA and site is a closed residental landfill but an active EOD Range is on top of the landfill. ST-06 received

NFA but is an active POL area. COCs for each site are also discussed in reports available on the Administrative Record.

OSHA HAZWOPER certifications are required for any invasive work.

PWS Section 6.1

Groundwater Monitoring

For work addressed under this section, are any of the wells locked and if so, who has the key(s)?

No wells are locked at this time.

PWS Section 6.1

Groundwater Monitoring

Well

Maintenance/Repair

Paragraph 12 states, “Closure of the taxiways will be required to perform ……… Note that wells MW-21 and MW-22 are located on the taxiway foxtrot and may require closure of the runway to ……..”.

If closure is required, can this be accomplished during normal business hours? If a runway and/or taxiway is closed, how long can it remain closed?

Please expect that work at ST025 in the vicinity of wells MW-21 and MW-22 can only be done on weekends, the taxiway can only be closed for the weekend, and the taxiway/apron needs to be fully available for mission use during the week (Monday through Friday).

PWS Section 6.1

Groundwater Monitoring

Well

Maintenance/Repair

Paragraph 12 states "the concrete is approximately 17-inches thick.”

Yet later in the discussion of core depths an assumption of 15-inches is presented. Is there a reason that the entire depth will not be cored?

Concrete ranges from 18 to 24 inches thick. PWS will be revised.

PWS Section 6.1

Groundwater Monitoring

Well

Maintenance/Repair

Paragraph 12 states “The Contractor shall replace the existing Well box for the following 20 wells.” By well box are you requesting that the well cover and collar be replaced (Manhole)?

Yes

PWS Section 6.1

Groundwater Monitoring

Well

Maintenance/Repair

In paragraph 12, MW-12 is identified as in a paved area, but appears to be in an unpaved area based on the figure and the photograph. Please verify.

MW-12 is in an unpaved area.

PWS Section 6.1

Groundwater Monitoring

Well

Maintenance/Repair

Per the figure, MW-18 is positioned very close to a fuel distribution lateral. Will the base locate the exact position of this lateral?

All of the fuel distribution laterals shown on the provided figure have been plugged and abandoned in place. The Base POC has figures showing the location of these laterals.

PWS Section 6.1

Groundwater Monitoring

Well

Maintenance/Repair

Is a base dig permit required and who will be responsible for routing the permit for signatures?

Yes - An approved 19 CES/CEOER Form 103 will be required prior to digging. Coordination with Base POC at least 2 weeks prior to activity is required.

RFP QUESTIONS / RESPONSES

Provide Annual land Use Control Inspection and Reporting for Multiple Sites and

Groundwater Monitoring Well Maintenance ST025, Little Rock Air Force Base, Arkansas.

[19 July 18]

PWS Section 6.1

Groundwater Monitoring

Well

Maintenance/Repair

In paragraph 12, what are the surface completion of the wells located in grassed areas? Current completions appear to be “concrete donuts”? Is a concrete pad required? If so, what are the dimensions required?

A concrete pad will be required. Minimum size shall be 4-inch diameter surrounding the well box.

PWS Section 6.1

Groundwater Monitoring

Well

Maintenance/Repair

Paragraphs 12 and 13 refer to flush mount monitoring wells. Typically flush mount monitoring wells have a slight rise (approx. 1-inch). When flush mount is specified is it intended to be completely flat without any rise to prevent surface water infiltrating the manhole?

Flush mount wells on-apron must be completely flush. Wells off-apron can have a slight rise.

PWS Section 6.1

Groundwater Monitoring

Well

Maintenance/Repair

Should the final elevations of the manholes be surveyed? Is surveying required to ensure the manhole and the surrounding concrete match exactly (if flush mount as specified is intended to be completely flat without any rise)?

Surveying will only be required if a change is made to the riser pipe, from which depth groundwater measurements are taken to evaluate groundwater elevation. Only the top of casing will need to be surveyed under this circumstance.

PWS Section 6.1

Groundwater Monitoring

Well

Maintenance/Repair

In paragraphs 12 and 13, the diameter of core specified for the 4-inch monitoring wells is an 18-inch core and for the 2-inch monitoring wells is a 16-inch core and both size monitoring wells will have 12-inch replacement flush mounts. Is there a specific reason for the differing size cores? Can the core size be larger?

There is no reason for the differing size cores. The core size can be larger.

PWS Section 6.1

Groundwater Monitoring

Well

Maintenance/Repair

Per paragraphs 12 and 13, the wells are to receive “12-inch diameter replacement flush mounts.” Does this diameter size refer to the cover or the cover and the collar? (A 12-inch diameter cover has an overall diameter of 14.25-inches)

The diameter size refers to the cover and collar.

PWS Section 6.1

Groundwater Monitoring

Well

Maintenance/Repair

In paragraphs 12 and 13, are there specific specifications for the “flush mount” cover; such as the material of the cover, number of bolts, length and material of the skirt, load rating? (It does state designed to accommodate the weight of an aircraft, does that mean it must have H-

20 Certification AASHTO M-306 or CID A-A 60005 or some other spec?)

Well covers are steel construction with 2 to 3 bolts. No load rating is required.

PWS Section 6.1

Groundwater Monitoring

Well

Maintenance/Repair

Paragraph 13 specifies 5,000 psi concrete. How long can the area be closed to allow concrete to set-up? Generally concrete would take 7 days for initial set, or will some quick set concrete be required? If a quick set type is desired what are the specifications?

The taxiway/apron can only be accessed on weekends and needs to be returned for mission use during the week. Therefore, a quick set type of concrete is required that meets the requirement of 5000 psi.

PWS Section 6.1

Groundwater Monitoring

Well

Maintenance/Repair

Will a batch design and/or a set of concrete cylinders or beams be required for confirmation of concrete psi? If beams are specified what is the required flex strength?

Testing of the concrete will not be required.

PWS Section 6.1

Groundwater Monitoring

Well

Maintenance/Repair

Will identification medals or stamped well identifications be required on or near the new well flush-mounts?

Stamped well identification - no medal id tags.

PWS? What areas is the Air Force anticipating having reseeded, fertilized and new topsoil added if work is restricted to the immediate area of the monitoring wells?

The PWS will be revised to state: "The groundwater monitoring well maintenance activities shall be summarized in an Informal

Technical Information Report (ITIR) that will include a discussion, figures, and photographs depicting the well repair activities."
