BIDDER’S LIBRARY REQUEST FORM

FORCE PROTECTION SITE SECURITY SYSTEMS (FPS4)

The Bidder’s Library Request Form is used to request FPS4 Official Use Only (FOUO) documents.

Documents will be sent to Company Representative(s) listed on this form only after the Contracting Officer has received this Bidder’s Library Request Form and the Government completes its verification process. Any information obtained from the Contracting Officer will be used for the sole purpose of ___________________________________________________________, Company Name (hereafter referred to as the Contractor) to respond to the FPS4 Request for Proposal (RFP) No.

FA8730-19-R-0005. The Aviation Missile Research Development and Engineering Center (AMRDEC) Safe Access File Exchange (SAFE) (https://safe.armdec.army.mil/safe/) will be utilized for transmission of all FOUO Bidder’s Library documents.

The Contractor is responsible for assuring that the information received will not be used for any purpose(s) other than stated above and that the information shall be destroyed by the Contractor when the work is completed. Prior to disposal, information shall be destroyed in a manner that renders documents and media unrecognizable, such as shredding. Information will not be disposed of in an intact form under any circumstances. The Contractor shall send a notice of destruction to the Contracting Officer, confirming that the information is destroyed.

The Contractor guarantees that the provided information, and/or any modified version thereof, will not be published in any manner or offered for sale. It will not be sold or given to any other firm without prior written approval from the Contracting Officer. All material received will be considered and treated as “For Official Use Only (FOUO)” and will be marked and handled as such. The Contractor understands that the information is for domestic use only. It will not be made available to or used in any contract with a foreign government, company, firm, academic institution, or any entity where a foreign interest has the power, direct or indirect, whether or not exercised, and whether or not excisable, to direct or decide matters affecting the management or operations of said entity.

The Contractor may disseminate FOUO information to its employees, who have an official need in connection with this announcement and possess US citizenship. Prior to access, verification that the intended recipient meets these criteria must be sought from the Contractor’s Facility Security Officer

(FSO).

During work hours, reasonable steps shall be taken to minimize the risk of access by unauthorized personnel. During non-working hours, FOUO information shall be stored in a manner that precludes unauthorized access. Filing such material with other unclassified records in unlocked files or desks, is adequate when internal building security is provided during non-working hours. When such internal security control is not exercised, locked buildings or rooms will provide adequate after-hours protection or the material can be stored in locked receptacles such as file cabinets, desks, or bookcases. Electronic transmission of FOUO information shall be by approved secure communications systems or systems utilizing other protective measures such as Public Key Infrastructure (PKI). FOUO information shall not be transferred to personally-owned computers, devices, or email accounts. FOUO information may be sent to official company e-mail addresses only, and must be encrypted. Discussion of FOUO material on the telephone is authorized if necessary. The Contracting Officer shall be notified immediately of any unauthorized disclosure.

Unauthorized disclosure of FOUO information may result in criminal sanctions.
