ADDITIONAL INSTRUCTIONS TO OFFERORS
This clause prescribes the format for proposals and describes the approach for the development and presentation of proposal data. These instructions are designed to ensure the submission of information essential to the understanding and comprehensive evaluation of proposals. The instructions permit the inclusion of any additional data or information an offeror deems pertinent within the stated page limitations. Offerors are cautioned to follow the detailed instructions fully and carefully, as the Government reserves the right to make an award based on the initial proposals received, without discussion of such proposals.
Submission of proposals. Submit signed and dated proposals to the office specified below. Proposals may be submitted on the SF 1449, letterhead stationery, or as otherwise specified in the solicitation. As a minimum, offers must show-
a) The solicitation number;
b) The time specified in the solicitation for receipt of proposals;
c) The name, address, CAGE Code, and telephone number of the offeror;
d) Any discount terms;
e) "Remit to" address if different than the mailing address;
f) A completed copy of the representations and certifications at FAR 52.212-3 Alt I (see FAR
52.212-3(1) for those representations and certifications that the offeror shall complete electronically;
g) Acknowledgement of Solicitation Amendments (if applicable);
h) I f t he proposal is not submitted on the SF 1449, include a statement specifying the extent of agreement with all terms, conditions, and provisions included in the solicitation. Proposals that fail to furnish required representations or information, or reject the terms and conditions of the solicitation may be excluded from consideration.
i) The Offeror's proposal must include all data and information requested b y this solicitation and must be submitted in accordance with these instructions and solicitation requirements. Any offeror who submits an incomplete package may be considered non-responsive and be eliminated from the competition. Offerors shall assume that the Government has no prior knowledge of their facilities and experience, and will base its evaluation solely on the information presented in the Offeror’s proposal.
j) Any proposal that modifies, or fails to conform to the essential requirements or specifications of the solicitation shall be considered non-responsive and categorized as unacceptable. If the proposal is determined unacceptable upon completion of proposal evaluations, the offeror will be notified promptly of the basis of the determination.
k) All questions in reference to this solicitation must be submitted to Diane Chen, Contract Specialist, e-mail address: diane.chen@us.af.mil no later than 5 August, 2013.3:00 P.M.
l) If an offeror believes that the requirements in these instructions contain an error, omission, or are otherwise unclear or unsound, the offeror shall immediately notify, the contracting officer in writing with supporting rationale.
m) The original and one copy of the proposal shall be e-mailed to diane.chen@us.af.mil or mailed or hand-carried to no later than 9 August 2013, 3:00 PM Local Time:

(n) Volume 1 - Technical Capability: Limit of 25 pages. If printing appears on both sides of a sheet of paper, it is considered to be two (2) pages.
The Technical Capability Volume (Volume I) shall not make reference to cost or price information. It will be evaluated strictly on technical merit.
Technical Capability Volume Requirements: Your Technical Capability volume will primarily describe the capability of your organization to perform the requirements as stated in the Performance Work Statement (PWS) attached to this solicitation. It should be specific and complete in every detail. A positive statement to the fact that you will accomplish all requirements per the PWS is not acceptable.

The proposal shall outline the actual method proposed as specifically as possible. Your Technical Capability volume shall, as a minimum, contain the information specified below in accordance with the following general outline:
• Technical Approach
• Management Approach
(o) Volume II - Past Performance
(i) Offerors shall include as part of their Past Performance Volume information on contracts they consider relevant in demonstrating their ability to perform all requirements of the PWS. This information may include data on efforts performed by other divisions, corporate management, and critical subcontractors or teaming contractors, if such resources will significantly influence the performance of the PWS requirements. If a teaming arrangement is contemplated, provide complete information as to the arrangement, including any relevant and recent past performance information on previous teaming arrangements with same partner. If this is a first time joint effort, each party to the arrangement must provide a list of past and present relevant contracts and provide questionnaires to their references. Past performance information pertaining to a subcontractor cannot be disclosed to the prime offeror without the subcontractor's consent. Provide with the proposal a letter from all subcontractors that will perform major or critical aspects of the requirement, consenting to the release of their past performance information to the prime contractor.
(ii) Offerors with vast numbers of relevant contracts to select from shall submit no more than six (6) relevant contracts/jobs performed presently or within the past five (5) years. Offerors are required to explain what aspects of each contract are deemed relevant to the proposed effort. Categorize the relevance determination into the specific evaluation areas used to evaluate this proposal. Offerors are permitted, but not required, to submit information on significant achievements or explain past problems considered relevant to the PWS requirements.
(iii) The offeror's proposal shall be evaluated to determine the level of customer satisfaction with the offeror on present and past projects similar in size, scope and complexity.
1. Describe the extent of your experience in carrying out similar work described in the PWS Assess whether your firm performed satisfactorily on similar projects regarding schedules, turnover rates, meeting costs and success in performing the work
2. Describe projects similar in size, scope and complexity to the PWS that your firm has satisfactorily completed.
(iv) In addition to the data provided by each offeror in this part of the proposal, the Government may use data obtained from other sources (Past Performance Information Retrieval System, other Government sources, commercial sources, etc.) in the development of the past performance evaluation.
(v) For all current or past contracts deemed relevant, provide all the following information: Administrative Data
1. Company/Division Name
2. Program Title
3. Contracting Agency
4. Contract Number
5. Type of Contract (Fixed Price, etc.)
6. Dates Performed
7. Contract Value (including any option years)
8. Name, Address, Telephone/Fax Number, and email address of Government Program
Director/Manager and Administrative Contracting Officer (ACO) or CO.
9. For Non-Governmental Contracts, Include name, title, address, email address and telephone number for both a technical and contracting Point-of-Contact for the contract.
10. Relevancy Data: Indicate the percentage of service actually performed by the offeror
(prime), subcontractor (if applicable), and teaming contractors (if applicable). This information indicates the volume of required service actually performed by the offeror and company(ies) other than the offeror on past or current contracts in relation to Relevancy. When applicable, provide the name of the company for each subcontractor or teaming contractor. The offeror may provide clarifying comments in relation to Relevancy.
(p) Volume III - Price Data Care should be exercised to ensure that the Technical Capability and past performance volumes of the proposal do not include any cost or price information. Volume III of the proposal should include only the following:
(i) A signed Standard Form (SF) 1449, Solicitation/Contract, and SF 30-Amendment of
Solicitation/Modification of Contract for all amendments (if applicable).
(ii) All required fill-ins in this Solicitation, including FAR 52.212-3 Alt I Representations and
Certification, and other Statements of Offerors or Respondents must be completed and included.
(iii) The offeror shall make a clear statement in the proposal documentation that the proposal is valid for
60 days.
(iv) The offeror shall complete the Schedule attached to the solicitation. The offeror shall complete all blanks for hourly rates and estimated cost, blanks for total estimated labor for the base period and each option period, blanks for estimated total for base period and each option period, and the blank for the ceiling amount for the base period through option IV. (The ceiling amount consists of the estimated total for the Base Period plus the estimated totals for Options I through IV.) The total estimated labor from the schedule for the Base Period and each Option Period shall be used to complete the Unit prices and total amounts for CLINS 0001, 0002, 0003, 0005, 0007, 0008, 0009, 0010, 0011, 0012, 0013, 0015, 1001, 1002, 1003, 1004, 1005, 1006, 1007, 1008, 1009, 1010, 1011 and 1013. Total amounts for CLINS 0004, 0006, 00014, and 1012 (Material) have been provided in the solicitation. All dollar amounts provided in response to these instructions shall be stated in U. S.
Currency.
