PERFORMANCE WORK STATEMENT

CABLE TV & INTERNET SERVICE,

INSTALLATION & SUPPORT

15 June 2015

1.0 DESCRIPTION OF SERVICES.

The contractor shall furnish all parts, tools, materials, and labor to provide Cable TV (CATV) and internet installation and customer support for Robins Air Force Base, GA. The contractor shall provide CATV service for the official areas listed in Appendix 5.2, and for additional installation of cable and internet access provided during contract performance. The contractor shall provide CATV and internet installation, basic CATV and internet service, and customer support for outages or repairs.

1.1 CATV Infrastructure. Current infrastructure is coaxial cable type RG-6/U (Radio Guide-double shielded) or coaxial cable type RG-6/UQ (Radio Guide -quad shielded) of the same type as used in CATV home and commercial installations.

1.1.1 CATV Installation Support

1.1.1.1 The contractor shall provide installation, activation, and deactivation for CATV service.

The Government Point of Contact (GPOC) will submit an installation, activation and deactivation request to the contractor for processing. The contractor shall provide a cost estimate (labor, material and time to complete) for the installation and/or activation of CATV within 5 working days of the request. Upon coordination and concurrence of the cost estimate by the government, the contractor shall complete the work request within 14 calendar days of upon issuance of work order and contract modification. If additional time is required, extensions must be coordinated with the Contracting Officer’s Representative (COR). The contractor shall notify the government of any changes in schedule.

1.1.1.2 The contractor shall obtain a Base Civil Engineering Work Clearance Request, AF Form 103, prior to any trenching or digging. Obtain an approved Base Civil Engineering Work Request, AF Form 332, prior to any facility, manhole, or hand-hole modification. Trenching, excavation, confined spaces entry, confined spaces atmospheric testing/forced air ventilation, marking and barricading of open trenches is to be performed IAW OSHA standards, any local procedures and other provisions of the contract.

1.1.2 Basic CATV Service. The contractor shall provide basic service which shall include all services required by the rules of the FCC and any other federal regulation and public access channel. Service shall include: CNN, Headline News, Fox News, the Weather Channel and six local TV stations (WGXA, WMAZ, WMGT, WPGA, WGNM and WDCO). Basic service for approximately 882 drops shall not include any premium channels such as HBO, CINEMAX, SHOWTIME or any Pay-per-View type channels. An estimated 336 drops in lodging facilities will include basic service plus one premium channel. See information below:

1.1.3 Internet Service Provider (ISP). The contractor shall provide standard commercial internet service to connect to the world-wide web. The contractor shall assign static IP addresses with a download speed of 30Mbps, or higher, and an upload speed of 5Mbps, or higher. The service established will have unlimited data usage with no data limits. The contractor shall provide a cost estimate (labor, material and time to complete) for the installation and/or activation of internet service within 5 working days of the request. Upon coordination and concurrence of the cost estimate by the government, the contractor shall complete the work request within 14 calendar days of upon issuance of work order and contract modification. If additional time is required, extensions must be coordinated with the COR. The contractor shall notify the government of any changes in schedule.

1.1.4 Sustainment and Customer Support. The contractor shall provide sustainment of CATV and internet service and provide customer support for any outages and/or repair services. The contractor shall return service to the customer within two working days once notified by the GPOC. Extensions may be granted by the government if delays are caused by factors beyond the contractor’s control, such as a cut cable. The Contractor shall notify the GPOC of the nature of the outage and when the services will be restored.

1.1.5 For any outside plant work needed to fulfill the installation or removal requirements, the contractor shall provide redline drawings to the GPOC within 5 working days upon completion.

3.0 GOVERNMENT FURNISHED PROPERTY

The government will provide floor plans showing the proposed location of any new CATV or internet service and will identify the GPOC for the cable drawings for any type of outside plant work required.

4.0 GENERAL INFORMATION

4.1 Hours of Operation

4.1.1 Schedule. Contractor personnel shall adhere to Government holiday and family day schedules. The Contractor shall ensure Contractor personnel are present and available for support during the hours of 0700 through 1600 hours Monday through Friday.

4.1.2 After Hours. The contractor shall coordinate with the COR if work outside normal duty hours is required.

4.2 Place of Performance: Services shall be performed at Robins AFB, GA.

4.3 Contractor Identification Badges. The contractor shall complete all necessary documents for all contractor personnel requiring access to Robins AFB. Contractor badges (AFMC Form 387) will be provided the government’s Trusted Agent for all on-site contractor personnel. Prior to application approval, each individual will be reviewed for eligibility in the Joint Personnel Adjunction System (JPAS) and checked against the WR-ALC barment roster. The contractor shall ensure contractor personnel who require access to government facilities comply with the security procedures of the facility.

4.3.1 The contractor’s Program Manager or alternate shall complete an AF 2586, Unescorted Entry Authorization Certificate, for each employee requiring entry into controlled and/or restricted areas. The Contractor shall request restricted area badges for employees according to RAFBI 31-101, Installation Security Program, through the GPOC.

4.3.2 During performance of the contract, the contractor shall be responsible for obtaining required identification for newly assigned personnel and for prompt return of credentials and vehicle passes for any employee who no longer requires access to the work site. Upon completion or termination of the contract or expiration of the identification passes, the contractor shall ensure that all base identification passes issued to employees and subcontractor employees are returned to the issuing office.

4.4 Other Responsibilities. Contractor shall maintain their installed underground and in house CATV cable/wire at their expense, (includes damages from lighting, flood, rain, etc.). Any underground TV cable cut or cable damaged inside a government facility CATV by non-government employees or contractors shall be the responsibility of the CATV contractor to seek direct payment for repair from said contractor or non-government personnel. If a government employee operating earth moving equipment cuts underground CATV or a government employee inside a government facility damages CATV connections/wires, the government will be responsible to pay for the cost of repair. The contractor shall follow the guidelines set forth in AFI 64-101, Multichannel Video Programming Distribution, for providing cable services.

4.5 Records (Files, Documents, and Work Papers). All official records, regardless of media, as defined in AFI 33-322, Para 2, are the property of the U.S. Government and shall remain so upon termination or completion of this contract.

4.5 Environmental Management System (EMS). Any contractor employee working on Robins Air Force Base will require EMS training. The prime contractor is responsible for ensuring that all subcontractors comply with this requirement. Computer-based training is provided by the Base Civil Engineering Group Environmental Management Office.

4.6 Green Procurement Program (GPP): The contractor shall comply with the Green Procurement Program in accordance with Under Secretary of Defense (USD) Memorandum, Establishment of the DoD Green Procurement Program, dated August 27 2004, and establishes the requirement for a GPP. The GPP is a mandatory federal acquisition program that focuses on the purchase and use of environmentally-preferable products and services. In accordance with FAR 23.404, Agency Affirmative Procurement Programs, the GPP requires 100% of purchases of Environmental Protection Agency (EPA)-designated products, included in the Comprehensive Procurement Guidelines (CPG) list, contain recovered materials unless the item cannot be acquired competitively within a reasonable timeframe, meet appropriate performance standards, and/or be acquired at a reasonable price. In accordance with FAR 23.101, Sustainable Acquisition Policy / Definition, the GPP requirement applies to all acquisitions, including services, using U.S. Government appropriated funds. The Green Procurement Program (GPP) includes these mandatory components:

4.7 Contractor’s Safety Requirement: The Air Force is part of the OSHA Voluntary Protection Program (VPP). All shall familiarize themselves with the requirements of VPP. Information on the VPP can be accessed at http://www.osha.gov/dcsp/vpp/index.html. Additionally, the contractor is responsible for the safety and health of all subcontractor employees.

4.7.1 Safety and Health Plan: Within 5 calendar days after contract award, the contractor shall provide a copy of their company Safety Plan that is required to ensure compliance with the Occupational Safety and Health Act (OSHA) (Public Law 91-596). The Safety Plan shall be provided to the WR-ALC Safety Office for review. The WR-ALC Safety Office will provide acceptance or non-acceptance within 5 working days of receipt to the government point-of-contact who will notify the CO. CDRL A001, Safety & Health Plan

4.7.2 The contractor Safety Plan shall address in detail the items listed in the Contract Appendix C, Part II through Part IV as applicable. For task specific procedures identified in the Contract Appendix C, the contractor’s internal procedures for those tasks must be identifiable within the submitted Safety Plan or shall be provided in an amendment or addendum to the company’s Safety Plan. If task specific procedures identified in the Contract Appendix C, are not submitted with the contractor’s Safety Plan, the submitted Safety Plan will be returned as NOT ACCEPTED. The contractor will have 5 working days to resubmit the Safety Plan, amendment or addendum to address or correct issues identified for non-acceptance. The WR-ALC Safety Office will provide acceptance or non-acceptance of the resubmission within 5 workings days of receipt to the government POC who will notify the CO.

4.7.3 Safety Mishap/Incident Reporting: The contractor shall report all mishaps/incidents in accordance with the Contract Appendix C. The contractor shall immediately secure the mishap scene and damaged property and impound pertinent maintenance and training records until released by the Government’s Safety Office (WR-ALC/SE). Such release shall be accomplished through the CO. The contractor shall coordinate with government personnel in the investigation of the incident and submit an Accident/Incident Report within 24 hours of the accident/incident.

4.8 Franchise Agreement. This agreement allows the cable operator to transmit television signals and operate a cable television system on the installation for the benefit of private subscribers and the CATV operator’s private commercial opportunity. Private CATV operators must obtain a nonexclusive franchise before offering on-base services. The length of a franchise agreement is dependent on and, therefore, cannot exceed the length of the term of the License to Use Real Property. Additionally, franchise agreements should be structured as one base year with annual options. (Ref: AFI 64-101, 4.3.3)

4.9 Quality Control. Contracting Officer’s Representative will inspect contractor performance in accordance with the Quality Assurance Surveillance Plan (QASP) and other technical guidance and regulations. The contractor shall submit a Quality Control Plan (QCP) within 30 days of contract start date. CDRL A003, Quality Control Plan http://www.osha.gov/dcsp/vpp/index.html

4.9.1 The contractor will maintain an internal Self-Assessment Program (SAP) to evaluate the effectiveness of existing procedures and internal controls, to determine organizational compliance with regulatory requirements and to complement external inspections and assessments. Additionally, the contractor shall provide support, as required, for contingencies, exercises, and surge operations. The FC/FD will notify the contractor as soon as notification of upcoming event becomes known. (WR-ALC OI 90-201)

4.9.2 The contractor shall support Air Force initiatives and participate in other applicable Quality Control programs and Continuous Process Improvement (CPI) initiatives designed to indicate the appropriate end-state of managed technical orders. The contractor shall comply with resulting changes to existing processes/procedures from these programs/initiatives. (AFMCI 90-104).

4.10 Essential Services During a Crisis. The Functional Commander/Director (FC/FD) has determined, by placing in writing their decision, that this requirement is not Mission Essential in accordance with DFARS 237.76, Continuation of Essential Contractor Services.

4.11 Contractor Manpower Reporting Requirements: Per reference SAF/AQ Memo, 13 Nov 12, “The contractor shall report ALL contractor labor hours (including subcontractor labor hours) required for performance of services provided under this contract for the USAF via a secure data collection site. The contractor is required to completely fill in all required data fields at http://www.ecmra.mil (select Dept of the Air Force option if you you’re working an AF requirement). Reporting inputs will be for the labor executed during the period of performance for each Government fiscal year (FY), which runs 1 October through 30 September. While inputs may be reported any time during the FY, all data shall be reported no later than 31 October* of each calendar year. Contractors may direct questions to the CMRA help desk.”

4.11.1 Reporting Period: Contractors are required to input data by 31 October of each year.

4.11.2 Uses and Safeguarding of Information: Information from the secure web site is considered to be proprietary in nature when the contract number and contractor identity are associated with the direct labor hours and direct labor dollars. At no time will any data be released to the public with the contractor name and contract number associated with the data.

4.11.3 User Manuals: Data for Air Force service requirements must be input at the Air Force CMRA link. However, user manuals for government personnel and contractors are available at the CMRA link at http://www.ecmra.mil (select Dept of the Air Force option if you you’re working an AF requirement).
