MANUFACTURING SOURCE QUALIFICATION STATMENT
(PUBLIC LAW\ V 98-525 10 Usc SECTION 2319)

SECTION C: QUALIFICATION REQUIREMENTS THAT MUST BE SATISFIED TO BECOME A QUALIFIED
SOURCE AND QUALIFICATION WAIVER REQUIREMENTS AND OTHER INFORMATION.
1. Pre-Qualification Notice The offeror shall notify the Small Business Office (00-ALC/SB) or, if responding to a solicitation, the contracting officer in 748 SCMG/PKB of intent to qualify as a source for this item.
2. Manufacturing, Test and Inspection Capability The offe ror shall certify to the design control activity (416
SCVS/GUMAA), the availability of the required manufacturing, test, inspection and diagnostic facilities and equipment or provisions for their acquisitions or utilization from another source.
3. Data Verification The offcror shall verity possession of a complete data package by providing a list of applicable
Drawings, procedures, and specifications currently in their possession related to the manufacture of this item. This data package must be of the most recent re vi sion or it must include any Engineering Change Orders (ECO's). This data must be sufficient to show in thorough technical detail ho'" ' the part shall be manufactured and tested. In addition the package must contain an acceptance test procedure designed to be a final functional checkout of the manufactured devices sufficient in detail to ensure all operational functions meet specification. When the offeror proposes to use industry standards during the manufacture of this item , the offeror shall document the differences between the industry standard and the military standard , and state why those differences do not adversely affect this item (performance, longev ity or otherwise). The design control ac t i v i ty shall determine if the industry standard is acceptable for use in the manufacture of the part. Manufacturing this item may require the use of some classified data, if so vendor must be approved to receive and store classified material.
Vendor may propose using a FFFI (Form Fit Function Interface) compatible solution in lieu of government proposed de sign.
4. Process Verification The offeror shall identify sources for mater ia ls and standards for internally used processes. The offeror shall further de fin e/prov ide written procedures for the utilization of any special test or inspection facilities and /or equipment.
Development of special l test procedures not available from the government ma y be required. The, manufacturing process shall show how the offeror shall produce, inspect, and perform a final functional checkout. Where development of test procedures not available from the government is required, the offeror shall develop those procedures for review and approval by the design control activity.
5. Test and Evaluation or Verification The offeror shall submit to the design control activity an explanation of how they will ce11ify that the manufacture i ng procedures and test methods meet the specifications for this item. The government retains the option to i inspect the offeror’s manufacturing and test i ng facilities. To allow this, the offero·rs facilities shall be made available to government inspection during the qualification effort upon ten days written notice of intent to inspect.
6. Item Unique Identification (IUID) The offeror shall l ce11ify to the government the ability to meet V1 I L-ST D- 1 30'\' requirement s for unique identification if the item being qualified has a value of S5000 or more, contains sensitive material or data, is crit ica l or at the d iscret ion of the engineering authority the item is required to have unique identification.
The offeror has the opt ion to either provide the capability or show they can provide the capability by contracting out the requirement to another vendor who possesses this capability which also mu t be proven to the government that the contracted vendor does meet the requirement for providing IL I D.
7. Testing and Manufacturing Evaluation. The offeror shall be required to submit a complete test report verify i ng compliance with all specifications including performance, environmental, mechanical and quality assurance requirements identified by the part or related drawings. Test report shall document in detail, equipment and test procedures used, the expectations and the results of each test. In addition the test report shall contain detailed documentation describing the testing procedures during manufacture. This data shall de scribe in detail the point(s) in the manufacturing process that testing occurred, and if failures were noted the corrective action taken and the results of the retest.
The government retains the option to inspect the manufacturing and test i ng process, including on-site witnessing

Application: F-16 SST Tester of any or all manufacturing and testing activities. The offeror will provide notification to the government of testing and evaluation 30 days in advance so the government can witness, if needed, coordinate needed activities, support, etc.
Vendor shall certify that all sub-part to be used in do not suffer from DMS (Diminishing Manufacturing Source) issues, meaning these parts are available for purchase.
8. Qualification Ar t icle Upon satisfactory compliance with the provisions of paragraphs I to 6 above, offeror shall provide a qualification article to the government so Government can verify that item is suitable. The Government will put this part into its next higher assembly to verify that (Form, Fit, Function and Interface) are acceptable. Estimated likely costs for testing and evaluation, which will be incurred by the potential offeror to become qualified: $1,250.00.
9. Sensitive Military Technology Not Applicable
I 0. Government Qualification, Time Estimate ln reference to item 8 above, it is estimated that the government qualification test on this article will take l 80 days . Events which occur during qualification process may cause this qualification time to increase or decrease depending on circumstance.
11. Offeror Right s An offeror may not be denied the opportunity to submit and have considered an offer for a contract if the offeror can demonstrate to the satisfaction of the contracting officer that the offeror's item meets the standards for qualification or can meet them before the date specified for award of the contract. The award need not be delayed to provide the contract with an opportunity to demonstrate its ability to meet the standards specified in this source qualification statement.
12. Government Rights The offeror must be fully qualified in order to be eligible for a contract award. Therefore, the offering party must fulfill all of the requirements stated, in writing. in this qualification requirement before the date specified for award of the contract. The government reserves the right to award a contract at the t i me specified for award , to a qualified source even though other sources may not have finished qualifying.
13. Waiver An offeror may apply 10 t he, 4 16 SCMS/GUMAA engi neer, for a waiver of any of the stated qualification requirements. Paragraphs I through 5 \,will be waived at the discretion of government engineer only if there is an obvious or oven·id i ng reason to do so. (Waivers would typically be granted to one who has previous experience in supplying this item, or similar items).
14. Approval Once the qualification requirements are met, the offeror will be listed as an approved source for this i tem.
Approval l, however, does not guarantee subsequent contract award.
