STATEMENT OF WORK (SOW)

For the Purchase and Delivery of Dry Ice in Support of 76 CMXG and 76 PMXG

February 22, 2016

OC-ALC/OBCA

1.0 GENERAL REQUIREMENTS

1.1 WORK INCLUDED: The Service Provider shall furnish all vehicles, trailers, containers, tools, equipment, parts, materials, supplies and labor for the loading, hand ling, transportation and delivery of Dry Ice to Tinker Air Force Base.

1.2 DESCRIPTION OF SUPPLIES/SERVICE REQUIRED

1.2.1 6830002470619 Carbon dioxide, Technical (Dry Ice) 99.5% delivered in 10lb/slabs to be delivered in requested quantities IAW the 76 CMXG and 76 PMXG Quantities and Drop Locations List.

1.2.2 6830002470619 Carbon dioxide, Technical (Dry Ice) 99.5% Minimum by weight (10lb bag of pellets) delivered in requested quantities IAW the 76 PMXG Quantities and Drop Location List.

1.3 Delivery Point - The locations designated for 76 CMXG and 76 PMXG Quantities are listed in the attachments titled: 76 CMXG Quantities and Drop Locations List for Dry Ice and 76 PMXG Quantities and Drop Locations List for Dry Ice.

1.4 JOB DESCRIPTION: The work to be performed includes the following pri ncipal features:

1.4.1 Service provider shall deliver two times per week on Monday s & Thursdays

1.4.2 Delivery vehicle must have sufficient means to t ransfer Dry Ice from vehicle to on-site containers.

1.4.3 Service provider shall provide related documentation (Bill of Laden) and Certificate of Analysis at the time of delivery.

1.4.4 A Material Safety Data Sheet (MSDS) shall also be provided for each drop point.

1.4.5 Service provider shall deliver ice in insulated containers and pick-up empty containers at time of delivery.

1.5 DELIVERY REQUIREMENTS

1.5.1 Containers. The contractor shall deliver the product in bulk containers that meet the following requirements:

1.5.2 All bulk containers shall be insulated containers that are approximately 4'x4'x4' in size. Containers must be leak proof and in good condition.

1.5.3 Two bulk containers will be delivered at Building 300 1 and one container to Building 9001 'for PMXG. These containers will remain on premises until the next delivery.

1.5.4 Upon each delivery to Building 300 1 and Building 9001 empty containers from previous deliveries will be picked up from PMXG.

1.6 DELIV IERIES

1.6.1. Service provider shall deliver Dry Ice within 1 working day of being notified. Start date will be coordinated between each delivery point of contact identified on the task order.

1 .6.2. Delivery Hours. All deliveries will be made between the hours of 8:00 to 10:00 am local time Monday and Thursday.

1.6.3 Service provider shall allow for at least two (2) hours unloading time. Inclement Weather Schedule. The contractor shall deliver during inclement weather except in cases of unduly severe weather and as authorized by the contracting officer. Make-up deliveries shall be performed 24 hours after the severe weather has terminated. If all make-up deliveries cannot be made in 24 hours, the contractor shall submit a revised schedule to the contracting officer for approval.

1.6.4 Holiday Schedule. The contractor shall deliver during Federal working days. (The term “working day" excludes weekends and U.S. Federal holidays.) Make-up deliveries shall be performed on the next working day. If all make-up deliveries cannot be made on the next working day, the contractor shall submit a revised schedule to the contracting officer for approval.
