STATEMENT OF WORK

FOR

Vacuum Thermoforming Machine

FOR

76 CMXG

OC-ALC/OBCA

20 November 2018

1 SCOPE. This Statement of Work (SOW) defines the effort required for the Vacuum Thermoforming Machine to include delivery, installation and training in Building 3907 at Tinker Air Force Base OK. This specification describes the minimum requirements for the purchase and installation of the latest generation of a deep draw vacuum thermoforming machine. This equipment will be used in the manufacture of aerospace interior panels.

1.2. Operation: This vacuum thermoforming machine when purchased shall be turn-key, ready for use, and require no additional equipment purchases to commence production.

2 REQUIREMENTS.

2.3. Order of Precedence: In the event of a conflict between the text of this document and the references cited herein, the text of this document takes precedence. Nothing in this document, however, supersedes applicable laws and regulations unless a specific exemption has been obtained.

3. Minimum Machine Specifications 3.1. Physical: 3.1.1. Max Size (footprint): W x L x H, 12 ft. x 14 ft. x 15 ft.

3.1.2. Minimum Forming Area: 45 in. x 65 in.

3.1.3. Minimum Depth of Draw: 23 in.

3.1.4. Plastic Clamping: Adjustable pneumatic and/or manual

3.1.5. Tooling Table: Pneumatic or servo-electric driven

3.1.6. Configuration: Single Station, Single Sheet

3.1.7. Heaters: Energy efficient quartz

3.1.8. Heater Controls: Multiple zones with variable adjustability

3.1.9. Heater Movement: Servo-electric driven

3.1.10. Pyrometer Control of heating cycle

3.1.11. Recipe Profiles Minimum capability to store 40 profiles

3.1.24. Must not require a pit or other type of foundation work.

3.1.25. Working height shall be less than 48 inches by design or through the use of contractor supplied work stand.

3.2. Utilities: 3.2.1. Electrical: Shall function on 480V electric utilities and if a transformer is necessary the contractor shall supply as part of this Purchase Document. Electrical requirements shall be provided with the contractor’s bid to include transformer details if necessary.

3.2.2. Air: Shall function on 100 psi shop air and if a regulator is necessary the contractor shall supply as part of this Purchase Document.

3.3. Performance: 3.3.1. Types of material: Plastic sheets i.e.: Lexan, Kydex, Royalite

3.3.2. Material Thickness: up to 0.20 in.

3.3.3. Pre-Stretch: Billow-vac or Snap-back

3.3.4. Auto Level: Control sheet sag by air application

3.3.5. Noise: Below 80dB(A)

3.4. Acceptance Testing: 3.4.1. The contractor shall supply a report of the equipment performance meeting or exceeding all contract requirements and any additional contractor inspected performance parameters before the delivery.

3.4.2. The government may perform any testing necessary after installation to verify performance of equipment to this Purchase Document.

4. Deliveries:

4.1. Delivery and Installation: 18-24 weeks after contract award

4.2. Shipment: F.O.B. Destination

4.3. Building Entrance: Multiple vehicle entrances are available at 13.5 ft. Max Width x 11.5 ft. Max Height and a single entrance is available at 15.0 ft. Max Width x 11.5 ft. Max Height. The aisles are 15 ft. wide and the ceiling is over 15ft in height at the installation location. Entrances are 400 ft. or greater from installation location and requires turning down several aisles.

5. Documents and Manuals:

5.1. Schedule: The contractor shall submit a detail schedule of delivery, installation, and training to Contracting who in turn will coordinate with the using activity.

5.2. Installation Instructions: The contractor within 45 days after award shall coordinate with the Engineer on providing equipment size, orientation, air, and electrical requirements so that the site and utilities may be prepared for installation.

5.3. Manuals: In addition to the installed and operational systems, the contractor shall supply three (3) sets in the ENGLISH language of manuals, maintenance and operator manuals in electronic and paper format. The three copies of electronic and paper manuals shall contain all information required to properly install, operate, disassemble and reassemble, reorder replacement parts, and otherwise guarantee the operability and maintainability of the delivered equipment. All appropriate electrical, mechanical circuit and functional diagrams shall be provided in native electronic format as AutoCAD or equivalent file types to properly describe the equipment operation. Operational specifications, and listings of test points and checks to be performed to verify satisfactory operation, shall be provided. Complete step-by-step operating procedures shall be included. All drawings in the paper manual shall be no larger than "B" size. Photographic reduction to the required size is permitted if the result is legible. These manuals shall contain a total illustrated parts list with a recommended list for stocking as spare parts.

6. Warranty:

6.1. Warranty Period and Plan: Items proposed shall be warranted for one (1) year for parts, and one (1) year labor. The standard commercial warranty, if applicable, will apply to items offered in response to this solicitation. Offerors are requested to include a copy of each applicable warranty with their response to this solicitation.

6.1.1. Warranty Commencement: Warranty period shall begin at final acceptance of equipment listed herein.

7. Training 7.1. Operation: The contractor shall provide on-site training to operators, engineers, and maintenance personnel covering at minimum the operation of the equipment to include routine inspections, start up, shut-down, emergency controls, LOTO, recipe creation, and both manual and semi-automatic operation.

7.2. Maintenance: The contractor will provide on-site training to maintenance personnel to include servicing, troubleshooting, maintaining, and calibration.

8. Monitoring Network 8.1. Purpose: Monitor equipment health and use through the Honeywell EBI system currently in use at the facility.

8.2. Communication Protocol: BACnet 8.2.1. Adjustable IP address by Tinker and Honeywell to ensure unique address.

8.3. Preferred Controllers: To be used whenever possible and if alternate is required it must be pre-approved prior to contract award. 8.3.1. Honeywell CPO-PC-6A, Honeywell HC900, Lynx Spring JACE JENE-PC6128, Johnson Controls FX80

8.4. Ports: disable Port 21 (FTP) and Port 23 (Telnet)

8.5. Create export table with data points to include at minimum modes and status.

8.6. Gateway: If necessary provide a gateway between the PLC and the communication controller.

3 MISCELLANEOUS REQUIREMENTS.

3.1 Environmental Protection & Safety. The Contractor shall be knowledgeable of and comply with all applicable federal, state and local laws, regulations, instructions and requirements regarding environmental protection and occupational safety and health. The Contractor shall comply with any laws, instructions or regulation changes during the term of this contract.

Contractors shall be compliant with Tinker Air Force Base (TAFB) Supplement to Air Force Instruction (AFI) 32-7086, “Hazardous Materials Management”, Tinker AFB procedures per Tinker Air Force Base Instruction (TAFBI) 32-7004, “Hazardous Waste Management”, and Complex procedures per Oklahoma City Air Logistics Complex (OC-ALC) Supplement to AFI 32-7086, “Hazardous Material Management” and OC-ALCI 32-101 “Air Quality Record Keeping Procedure”. Publications are available digitally on e-Publishing website at www.e-publishing.af.mil for downloading or ordering.

3.2 Hazardous Material (HAZMAT). If no HAZMAT will be brought on base or used in performance on this contract, the contractor shall certify “No HAZMAT will be brought on base” or “No HAZMAT Usage” by official correspondence. This certification shall be provided to the GPOC within ten (10) days after award. Upon GPOC receipt of the certification, the requirements of this section no longer apply. Appropriate penalties for the contractor, or administrative and disciplinary action for the latter shall be assessed if found in violation of above certification.

The Contractor shall comply with applicable federal and military procurement policies IAW OC-ALC Supplement to AFI 32-7086, “Hazardous Material Management”. The Contractor shall obtain access to the standardized Air Force HAZMAT tracking system and manage HAZMAT within the system or have a government designated HAZMART comply. The Contractor shall establish and reconcile the inventory, print the standardized Air Force HAZMAT tracking system bar code labels, and apply the labels to containers of HAZMAT so that the Occupational Safety and Health Administration (OSHA) compliant labels are not covered. Any failure to comply with proper HAZMAT management procedures shall be reported to the CO and GPOC who will notify 76 MXSG/MXDEU, OC-ALC Environmental and Occupational Health Compliance.

3.3 Safety Data Sheet (SDS) and Labeling. Upon GPOC receipt of the “No HAZMAT Usage” certification, the requirements of this section no longer apply.

The Contractor shall not transport, ship or use any chemical, material or substance used in the performance of this SOW on Tinker AFB before written approval is provided by 72 ABW/CEIE, Natural Infrastructure Management (Environmental Compliance). The Contractor shall submit to the GPOC, ten (10) calendar days after contract award date all Safety Data Sheets (SDSs) and a completed OC-ALC Form 152, SDS Information Sheet. The Contractor shall receive approval from the GPOC prior to shipping any chemical, material or substance to Tinker AFB. A copy of the manufacturer SDS shall accompany the shipment. Contractor shall comply with the effective dates for SDS requirements IAW the Occupational Safety and Health Administrations (OSHA) Hazardous Communication Standard, Title 29, Code of Federal Regulation (CFR) Part 1910.1200. The Contractor shall ensure containers of hazardous chemicals are labeled with the following information, prior to shipping to Tinker AFB IAW OSHA Hazardous Communication Standard, Title 29, CFR Part 1910.1200 (2012): product identifier; signal word; hazard statement(s); precautionary statement(s); pictogram(s); and name, address and telephone number of the chemical manufacturer, importer, or other responsible party. The GPOC will forward all SDS submittals to 72 ABW/CEIE for approval. A current manufacturer SDS shall be submitted to the GPOC if the Contractor changes manufacturer or vendor for any chemicals that were prior approved. The Contractor shall maintain copies of manufacturer SDSs for all chemicals and SDSs shall be readily accessible at all times by Contractor personnel as well as government employees.

3.4 Hazardous Waste. If no HAZWASTE will be generated during performance of this contract, the contractor may certify “NO Hazardous Waste Generation” by official correspondence. This certification shall be provided to the GPOC within ten (10) days after award. Upon GPOC receipt of the “No Hazardous Waste Generation” certification, the requirements of this section no longer apply. Appropriate penalties for the contractor, or administrative and disciplinary action for the later shall be assessed if found in violation of above certification.

The Contractor shall dispose of contaminated solid waste, spent or processed chemical solutions, used rags and hazardous waste in government furnished waste containers IAW TAFB HWMP.

The Contractor shall be responsible for the Initial Accumulation Point (IAP) within the Contractor assigned space. The IAP shall be under the direct control of the Contractor who generates the waste. Before accumulation begins, the Contractor shall ensure appropriate hazard labels are affixed to government furnished waste containers as specified in 49 Code of Federal Regulation (CFR) 172.400. Management procedures for IAPs include written weekly inspections and reporting. When the container has been filled, the Contractor shall seal the container, mark the Certified Filled Date on the container label using an indelible pen, place the container at a designated pick up location, and call the Hazardous Waste Management Facility (HWMF) at 734-3285 for pick-up service. The Contractor shall ensure the HWMF picks up the container within three (3) calendar days after the Certified Filled Date. If container is not picked up within three (3) calendar days, Contractor shall notify the GPOC who will notify 76 MXSG/MXDEU, OC-ALC Environmental Engineering Section.

The Contractor shall perform written weekly inspections using Tinker AFB IMT Form 487, Initial Accumulation Point Management Aid. Weekly denotes once per working week. The Contractor shall submit Tinker AFB IMT Form 487 on a monthly basis to the GPOC no later than the third (3rd) day of the month following the reporting period. If the third (3rd) day of the month is a weekend or holiday, the logs are required by the last business day 'PRIOR' to the third (3rd). The GPOC will forward the form(s) to the UEC no later than the fourth (4th) day of the following month or the first (1st) subsequent business day. Contractor shall maintain a signed copy of the form(s) on site for one (1) year.

3.5 Spills. Upon occurrence of any spill that could enter the storm system or cause any harmful environment effects, the Contractor shall immediately call 911 to report the incident. When possible, the Contractor shall use a base telephone to receive help from Tinker AFB 911. If using a cell phone, the Contractor shall request Tinker AFB 911. The Contractor shall immediately report all environmental violations to the GPOC and UEC. The Contractor shall provide five (5) copies of a typed or written report about the incident within four (4) clock hours as follows:

One (1) copy to the CO, two (2) copies to the GPOC, a copy for UEC and one (1) copy for record. The format of the report shall be left up to the Contractor, but the report shall contain the following information:

1.) Name and contact number of person reporting the spill 2.) Date and time of incident 3.) Location and source of spill 4.) Substance or pollutant spilled 5.) Amount spilled and rate of discharge 6.) Any damages or injuries involved 7.) Extent of area impacted 8.) Potential hazards 9.) Actions taken 10.) Organizations contacted 11.) Remarks Section - that addresses the cause of the spill (e.g. human error, equipment, etc.) and whether 911 were called and which agencies responded.

3.6 Privately Owned Motor Vehicles (POV). Contractor personnel shall comply with the directives pertaining to operation of POVs on Tinker AFB per AFMAN 31-116, in its entirety, “Motor Vehicle Traffic Supervision”. The Contractor shall comply with Tinker AFBI 24-302, in its entirety for the operation of POVs within Tinker AFB.

3.7 Visitors Pass. The Contractor shall contact the Government POC seven (7) business days before arriving on Tinker AFB in order to make arrangements for Visitors Passes. The Government POC will assign a sponsor for the Contractor from within the organization. The Contractor and the sponsor shall arrange to meet at Pass and Registration, Building 6611 at the Tinker Gate which is South of I-40 off of Air Depot. The Contractor shall provide two (2) forms of identification, one of which shall be a picture ID, to the 72 SFS. The 72 SFS will provide an AF Form 75, Visitors Pass, which will be valid for up to thirty (30) calendar days.

3.8 Foreign Nationals. The Contractor shall notify the Government POC before sending a Foreign National representative to perform services. Special rules apply for foreign nationals visiting a US Government facility.

3.9 Safety and Health on Government Installations. While performing work under this contract on a Government installation, the Contractor shall comply with federal, state and local regulations, and AFI 91-203, “Air Force Consolidated Occupational Safety Instruction”, as established by this contract. The Contractor shall notify the CO, COR, and Government Contract Administrator (CA) within one (1) business day of any injuries that occur to contractor personnel while they are on the installation. The Contractor shall notify the CO, COR, and Government CA within one (1) business day of damage to Government property or equipment during the execution of the contract.

3.10 Emergency Procedures. The Contractor personnel shall follow the direction of the Government personnel with regard to emergency procedures, such as fire, tornado, active shooter, bomb threats, or other emergency. It is the responsibility of the Contractor to provide guidance and to establish procedures for its employees to respond to an emergency. It is the Contractor’s responsibility to account for their employees following an emergency evacuation. The Contractor personnel shall participate with the Government personnel in all fire and tornado drills at no additional cost to the Government.
