ATTACHMENT 2 – Instructions to Offerors and Evaluation Criteria FAR 52.212-1 & FAR 52.212-2

Addendum 1 to FAR 52.212-1, Instructions to Offerors – Commercial Items (Jan 2017)

INSTRUCTIONS TO OFFEROR

Part 1 Technical Proposal

The minimum font size for the proposal is 12-pitch and the proposal must utilize Times New Roman themed font. Required documents should be submitted via email to the Contract Specialist, Ms. Jacqueline Davis jacqueline.davis.17@us.af.mil AND the Contracting Officer, SSgt Tessa Smith tessa.smith.3@us.af.mil with a read receipt and should be clearly marked with the requirement title. You may request delivery confirmation. Your proposal may not be more than 15 pages in length excluding the cover page, cover letter and table of contents.

Part 2 Pricing

In addition to the submission of the technical proposal, you must submit the Pricing Schedule B (Attachment 3). If only one offer is received, the Government may request a price breakdown of each labor category. The pricing proposal shall be submitted concurrently with the technical proposal to the Contract Specialist and Contract Officer above and it does not contain a page limit.

FAR 52.212-2, Evaluation – Commercial Items (Oct 2014)

(a) The solicitation requirements include all stated terms, conditions, representations, certifications, and all other information required by this solicitation. The following factors will be used to evaluate offers.

TECHNICAL PROPOSAL:

1. This technical proposal will be rated as “Acceptable” or “Unacceptable” based on the technical proposal evaluation criteria stated in the below table on page 7.

a. Acceptable - Meets all specified technical capability requirements necessary for acceptable contract performance.

b. Unacceptable - Fails to meet one or more specified technical capability requirements. A proposal with an unacceptable rating is considered not awardable.

2. Proposals will be evaluated against the technical factors and sub-factors in accordance with the table below titled “Technical Capability and Evaluation Factors”. The offeror shall address as specifically as possible the proposed methodology for accomplishing/satisfying these factors/sub-factors. The proposal shall not simply rephrase or restate the Government’s requirements, but rather shall provide convincing rationale to address how the offeror intends to meet these requirement. All requirements specified in the RFP’s Performance Work

Statement (PWS) are mandatory. By proposal submission, the offeror is representing that the offeror can and will perform ALL requirements specified in the PWS.

Factor 1 – Technical Approach: The offeror shall address the contractor’s skill, education, expertise and experience in providing Advisory and Assistance Services (A&AS) for similar or same services. The offeror shall provide the scope and desired capabilities defined in the PWS dated 09 January 2018 (Attachment 1) as well as address the skills and expertise in Quality of Care Delivery Services.

Factor 2 – Management Approach: A list of qualifications for the labor category must be submitted to demonstrate the offeror’s understanding of the related experience it can find for this requirement. The offeror shall fill position according to the PWS dated 09 January 2018, maintain personnel with the requisite education and skills and maintain personnel to perform tasks in accordance with the PWS.

Factor 1. Technical Approach

Subfactor 1.1 Subject Knowledge

Describe how the Offeror’s capability benefits the Government from the Offeror’s knowledge, experience, and expertise of the subject matter.

Offeror’s capability clearly demonstrates the Offeror’s knowledge, expertise, and in depth experience of the tasks identified in PWS in 3.0, 3.1 and 3.2, and how the Offeror intends to leverage this knowledge, expertise and experience to provide the best possible service to the Government.

3.0, 3.1, and 3.2

Subfactor 1.2 Staffing of Personnel

Describe the Offeror’s capability to staffing this requirement with qualified candidates. The contractor shall submit a list of qualifications for the labor category that each of their candidates possess to meet the Government’s requirements for education, certifications, experience, Knowledge Skills and Abilities, and clearance.

Offeror’s process clearly details how their personnel supporting this contract will be identified, screened, placed, and monitored, as needed, to perform the requirements on the first day of the period of performance. The process should also identify how the Offeror will ensure personnel possess the appropriate levels of education, experience, and expertise.

Subfactor 2.1 Management Approach

Describe the offeror’s management approach that reflects a plan which will lead to the successful accomplishment of the requirement which includes mitigation of service lapses, retention and recruitment of personnel and obtaining the proper security requirements.

Offeror’s approach clearly details how their plan will lead to the successful accomplishment of the requirement. This plan include mitigation of service lapses, retention and recruitment of personnel and obtaining the proper security requirements.

2.2, 2.3, and 3.2

PRICING PROPOSAL:

The Offeror must propose to each CLIN within the Pricing Schedule B (Attachment 3) except those addressing FFP Travel. Pricing for the travel CLIN will be determined by the Government with a Not Exceed value and will be based on the travel estimates in the PWS. All pricing data excluding travel will be used to support the FFP CLINS to allow the government to conduct a thorough pricing evaluation.

In accordance with FAR 15.403-1(b) and FAR 15.403-3(a), information other than pricing data may be required to support price reasonableness or realism in the absence of competition.

Information shall be provided in accordance with FAR 15.403-5. When two or more technically acceptable offers are received (competition) and the lowest price is selected, the price of the lowest Offeror can be concluded to be balanced, fair, and reasonable. A price that is very low will be verified to assure that the Offeror understands what is being asked for and no mistakes have been made in preparing the price.

If after receipt of the proposals, the Contracting Officer determines that there is insufficient information available to determine price reasonableness and none of the exceptions in FAR 15.403-1 apply, the Offeror shall be required to submit pricing data. All CLINs within Attachment 3, Pricing Schedule B except Travel CLIN will be evaluated for reasonableness. The Government will award to the lowest price technically acceptable offeror. The Government will rank all offers by price. If the lowest price is fair and reasonable the proposal will be rated for technical acceptability. Failure to provide prices as stated above may result in the rejection of the proposal as technically unacceptable. All pricing information shall be addressed ONLY in Pricing Volume file format.
