The following changes have been incorporated within this solicitation:

FA8052-16-R-0013:

1) The Questions submission date has been changed to 25 April 2016 at 10:00 A.M. Local San Antonio, Texas time.

2) The Proposal submission date has been changed to 12 May 2016 at 10:00 A.M. Local San Antonio, Texas time.

3) The estimated award date is 26 May 2016.

4) All other conditions within the solicitation remain the same as Amendment 01.

Description: This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice and attached Performance Work Statement (PWS). This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solicitation document and clauses are those in effect through Federal Acquisition Regulation (FAR) circular 2005-86 effective 01 Feb 2016, Defense Federal Acquisition Regulation Supplement (DFARS) Publication Notice (DPN) 20151230, effective 30 Dec 2015, Air Force Federal Acquisition Regulation Supplement (AFFARS) AFAC 2015- 1001, Baseline effective, 1 Oct 2015. Solicitation number FA8052-16-R-0013 is issued as a request for proposal (RFP), entitled Health Coaching Demonstration for Air Force Surgeon General’s Aerospace Medicine Directorate (AFMSA/SG3P). The Department of the Air Force, Air Force Installation Contracting Agency (AFICA) 773d Enterprise Sourcing Squadron, and (773 ESS/PKA) intends to award a Firm Fixed Price non-personal services contract under Full and Open Competition. Potential offeror's are to review and address this Combined Synopsis Solicitation and all attachments which include the Performance Work Statement (PWS) dated 30 Mar 2016, and all applicable provisions and clauses. This requirement is made available to interested parties through https://www.fbo.gov.

NOTE: Contractors please see all posted attachments regarding this acquisition for instruction and compliance to this Synopsis.

FAILURE TO PROVIDE ANY REQUIRED INFORMATION MAY RESULT IN AN OFFER

BEING DETERMINED UNACCEPTABLE.

Requirement: The Government intends to award a firm fixed price contract for three (3) FTEs to support AFMSA/SG3P, Aerospace Medicine Directorate for advisory and assistance services. The Contractor shall provide project management support services to assist the AFMS leverage the timing of the Department of Defense Healthcare Management System Modernization (DHMSM) to integrate technology, patient partnership, and health coaching to assist and empower people to lead healthier lives. The Contractor shall assist the AFMS to develop and implement a Health Coaching Demonstration (HCD) project to integrate into the patient workflow and leverage the standup of the Department of Defense (DoD) electronic health record (EHR). The Contractor shall provide subject matter expertise to obtain approved/validated capability needs resulting from gaps/shortfalls identified by the capabilities planning process. The Contractor shall provide one (1) Project Manager and two (2) Health Coaches; a comprehensive health coaching training program transitionally or fully accredited by the National Consortium for Credentialing Health & Wellness Coaches (NCCHWC); an electronic library of behavior change materials tied to the DoD EHR patient portal function; and a quality assurance, evaluation and improvement program to increase efficiency and effectiveness in accordance with the attached Performance Work Statement (PWS). The associated NAICS code is 541611 with a size standard of $15,000,000.00. The contract will consist of CLIN 0001/1001/2001, Labor-Project Manager, 1 FTE; CLIN 0002/1002/2002, Labor-Health Coach, 1 FTE; CLIN 0003/1003/2003, Labor-Health Coach, 1 FTE; CLIN 0004/1004/2004, Non-Labor Travel. The base year and each option period will allow for continued performance up to an additional 6 months, as needed, after that respective period of performance IAW FAR Clause 52.217-8 Option to Extend Services, which is included in this solicitation. The pricing for this period (if utilized) will be the rates identified, at the time of proposal, on CLIN 3001, 3002, and 3003. All proposals shall conform to Contract Line Item Number (CLIN) structure of the Pricing Schedule (Attachment 1).

Place of Contract Performance: For the Program Manager and one (1) health coach, the work to be performed under this contract shall be performed at the DHHQ, Falls Church, VA, 7700 Arlington Blvd, Falls Church, VA 22042. The Contractor may on occasion be required to attend briefings or training at Bolling AFB, Fort Detrick, the Pentagon, or other locations within the Washington DC area. For one (1) health coach, the primary place of work will be Fairchild AFB, Washington.

Period of Performance. The Period of Performance for this requirement is a 12 month base year plus two (2), twelve month option periods. Estimated award date is 26 May 2016. The base year and each option period will allow for continued performance up to an additional 6 months, as needed, after that respective period of performance IAW FAR Clause 52.217-8 Option to Extend Services, which is included in this solicitation. The pricing for this period (if utilized) will be the rates identified, at the time of proposal, on CLIN 3001, 3002, and 3003.

Basis of Award. All proposals will be evaluated in accordance with FAR 15.101-1 -- Tradeoff Process.

The Government will select the technically acceptable proposal found to be most advantageous to the Government price and other factors considered. The evaluation criteria are divided into three factors:

Technical Capability, Past Performance and Cost/Price. Technical Capability will be assessed as acceptable or unacceptable based on demonstrated technical expertise and management approach to meeting the requirements of the PWS; Past Performance will be evaluated as significantly more important than price.

Award may be made with or without discussions with the offeror(s). This RFP is not authorization to begin performance and it in no way obligates the Government for award of a contract or payment of any costs incurred by the contractor in preparation of proposals for this requirement. Prior to commencement of any activities associated with performance of this requirement, the Government will issue a contractual document signed by the Contracting Officer with an effective date of performance.

Proposal Requirement. Please provide your proposal by addressing the PWS requirements to include:

Providing a technical proposal (identified as Part II – Technical – no more than 25 pages) addressing the technical capabilities. See Attachment 4, Instructions to Offerors -- Commercial Items.

Providing a Price/Cost proposal (identified as Part I - Price/Cost) which will be evaluated for price fair and reasonableness IAW FAR Part 15.404-1. See Attachment 4, Instructions to Offerors -- Commercial Items.

Providing no more than three (3) Past Performance Questionnaires completed by an organization the proposing firm has provided health coaching services in the past. See Attachment 6, Past Performance Questionnaire.

Proposal Due Date: Proposals must be submitted electronically to the Contract Specialist, Percilla McDuffey, via e-mail percilla.mcduffey@us.af.mil and the Contracting Officer, MSgt Christopher T.

Sutton, via email christopher.sutton.3@us.af.mil. Proposal must conform to the requirements of this RFP and be received no later than 12 May 2016 at 10:00 A.M. Local San Antonio, Texas time. It is the responsibility of the offeror to ensure that the proposal and its attachments are received.

Performance Work Statement (PWS), Appendix B - Business Associate Agreement shall be filled out by the contractor and submitted with proposals.

Any proposals received after designated closing date and time will not be considered for award (see Attachment 4, Instructions to Offerors -- Commercial Items).

Questions regarding this RFP: Questions are due no later than 25 April 2016 at 10:00 A.M. Local San Antonio, Texas time and shall be submitted in writing to the Contract Specialist, Percilla McDuffey, via e-mail at percilla.mcduffey@us.af.mil. Questions received after 25 April 2016 need not be considered.

Organizational Conflict of Interest (OCI). In accordance with FAR Subpart 9.5 and relevant supplements, the contractor shall make an immediate and full disclosure of any potential OCIs in writing to the Contracting Officer. Such disclosure may include a description of any action in which the contractor has taken or proposes to take to avoid, neutralize, or mitigate any resulting conflict of interest.

Contact Information. The point of contact responsible for answering all inquiries is the Contracting Officer via the Contract Specialist. Questions to the solicitation must be in writing and e-mailed to the Contracting Specialist No Later than 25 April 2016, 10:00AM Local San Antonio, Texas Time.

Section 508. Per FAR 11.002(f) In accordance with Section 508 of the Rehabilitation Act of 1973 (29 U.S.C. 794d), requiring activities must prepare requirement documents for electronic and information technology that comply with the applicable accessibility standards issued by the Architectural and Transportation Barriers Compliance Board at 36 CFR part 1194 (see Subpart 39.2).
