When completed, this document is Source Selection Information IAWFAR 2.101 and 3.104.

The SAF/MG Business Transformation Division of the Air Force District of Washington (AFDW) is in the process of competitively selecting a source for an Executive Leadership Training program.

The Office of the Deputy Under Secretary of the Air Force, Management and Deputy Chief

Management Office (SAF/MG) is continuing to look for more effective and efficient ways to support

Air Force business operations and refine the development of Defense Business System (DBS) requirements supporting the Business Mission Area (BMA) Information Technology (IT) by leveraging existing technologies owned by the Air Force, streamlining and standardizing business processes across the Air Force, and developing integrated solutions that are achievable in a constrained fiscal environment. The Air Force has a requirement for business system information system management Advisory and Assistance Services (A&AS) that includes, but are not limited to, the following categories: technical and engineering; information and data management; process engineering; performance assessment; change management, facilitation, project management, and scheduling. In addition, this effort shall support SAF/MG assessments of progress toward Air Force strategic and program mission objectives. The Contractor is required to provide tools, equipment, personnel, and supervision necessary to support tasks to be described in orders issued under the contract vehicle.

One of the considerations in proposal evaluation is the verification of the offerors' past performance on contracts or other work efforts which reflect the offeror's ability to perform on the proposed effort. We depend on information received from organizations such as yours, which have had first-hand experience with the offeror, for the evaluation of the offeror's performance on those contracts or work efforts.

Our areas of interest regarding the offeror are summarized in the enclosed questionnaire. In order to meet the acquisition milestones, we request your written response no later than

25 April 2018. This schedule will allow us sufficient time to analyze the data prior to the start of evaluations.

PAST PERFORMANCE REFERENCE QUESTIONNAIRE

Vendors/Contractors: Provide the information requested in this form for each contract/program (citation) being described. All five (5) references should meet the recency and relevant for Factor 3: Past Performance detailed in the solicitation.

Vendors/Contractors are to complete sections A – E and then send it to the Point of Contact (POC) identified below for the past performance references. Each Government POC should receive Past Performance Cover Letter and this document for the Government to complete. Vendors are to provide frank, concise comments regarding your performance on the contracts you identify.

The Government POC is requested to complete Sections F to I on the program, training course or the content of the contract noted in Section A - E. The Government POC is requested to provide this information on the performance of the Vendor/Contractor and email this form to Traenesha L.
