MEMORANDUM OF RECORD FOR 10 CONS

SUBJECT: Forest Health Q & A Solicitation FA700017T0043

The following are questions and answers received for the subject solicitation.

1. Regarding Attachment 7 at the bottom: This document is to be completed and returned with the offeror's proposal, not later than the date and time listed in Block 8 of the SF 1449. I don’t see a SF 1449.

a. Correct. There is not an SF 1449. This was an oversite it should have referred to the Request for Quote titled Combined Synopsis/Solicitation. The proposal is due by 19 May at 1PM.

2. On section I. Contract Information the past performance survey there is the following note: *Note: If quoter holds or has held more than one contract with the agency/organization in the last 3 years, please complete separate evaluation forms for those contracts as well. Do you want all contracts with one company even if it is more than the maximum of 3 references?

a. No. The maximum references is 3. The Government does not want more than one contract listed per evaluation form. If you have multiple contracts with one company then each contract counts as a separate reference.
