STATEMENT OF WORK

RVKQ 17-0717 – Repair/Replace Utility Poles and Wiring

4 OCT 16

1. SUMMARY OF WORK: The Contractor shall furnish and install all material, equipment, components, parts, tools, labor, transportation, hauling, disposal/recycling, cleaning, inspection, testing, commissioning, training, reporting, etc., necessary to perform this project as described in the contract-delivery order, including this Statement of Work and any / all applicable federal, state and local law, regulation, standards. Unless otherwise stated, any

/ all requirement for contractor to remove and properly dispose of / recycle off base, and to furnish and install / incorporate any / all new materials / equipment

/ components needed to properly complete the work item; any Government furnished property and / or reinstallation of existing materials / equipment / components shall be as stated / specified herein. Contractor shall submit materials / methods for approval prior to ordering and / or incorporation into the work; as a minimum, submittals shall include product literature, MSDS, standard color, texture and style samples / swatches / boards, and place of origin

/ manufacture. Demolished materials, waste / debris is property of the contractor, unless otherwise stated herein; contractor is responsible for proper disposal / recycling of such materials off base; such materials shall be promptly removed from the installation and shall not be stored / stockpiled unless submitted and approved for such. Contractor is responsible for verification of all details, measurement, etc., prior to ordering materials and execution of work.

Buildings will be occupied during repairs, all repairs must be done hot/live where possible. The contractor must coordinate/ reduce any disruption to occupants during demolition/ build of this project. Requests to shut off power to facilities must be coordinated 3 weeks in advance with NFAS

CEC office to allow for base coordination.

2. Repair/replace utility poles, wiring and hardware as noted below and on the attached inspection report.

a. All poles noted as having loose hardware shall all hardware checked and tightened.

b. All poles marked as being removed will be removed. Back fill the hole from the pole. The contractor will relocate 4 poles to a designated area.

The poles and the relocation area to be noted during the walkthrough.

c. All poles noted as being replaced will have all attachments removed.

Install new hardware and attachments on new pole. Any exceptions to new hardware will be noted during the walkthrough.

d. All cross-arms noted as being replaced will be replaced in kind to include any attached hardware unless failing to meet code. The contractor will turn over cross arms identified during walkthrough, laydown area for these cross arms is to be determined.

e. All wiring marked as needing to be replaced will be replaced in kind as existing unless no longer available, not sized for load or failing to meet code.

f. All hardware noted as being broken or missing will be replaced in kind as existing hardware unless no longer available, not sized for load or failing to meet code.

g. All other items noted on the inspection are to be replaced, work performed or removed in accordance with the line item. Any questions on the line item are to be addressed during the walkthrough.

h. All removed poles, hardware, wiring, etc. removed will be disposed of by the contractor, except poles, cross arms, hardware marked as being relocated. All disposal will be in accordance with the environmental specification attached and noted below.

i. Contractor is to coordinate any power interruption or data interruptions with base communications and/or building occupants. Contractor is to minimize power/data interruptions to the fullest extent.

3. A submittal of intended materials is requested for the following items for review before installation: all wiring, hardware and poles. However, see Environmental section below for required submittal documentation.

4. All installation of materials will be per manufacturer’s recommendations and local and state building code requirements. All materials must meet all NEC, NFPA codes and IAW current standards.

5. The Contractor shall be responsible for the proper disposal of all materials removed and shall ensure that the facility is secure and available for use at the end of each duty day.

6. ENVIRONMENTAL MANAGEMENT AND PROTECTION

a. A Demolition Plan meeting the requirements of environmental specification section 01 57 20, must be submitted for review prior to any work.

b. A Storm Water Pollution Prevention Plan (for disturbance less than 1 acre) meeting the requirements of environmental specification section

01 57 20, must be submitted for review prior to any work.

c. Demolition materials to be listed on the Waste / Recycle Materials Log for Contractors

d. The contractor is to remove all lamps, ballasts, thermometers, batteries

(typically found in exit signs) and any other mercury containing devices and turn over to the 914 MSG/CEV for proper disposal.

7. Green Purchasing

Contractor shall comply with the Green Purchasing requirements of the contract. Known / possible EPA Comprehensive Procurement Guideline, USDA Biobased, Green Seal Energy Star, etc., items. Recycled materials must be used to fullest extent in accordance with Executive Order 13101. The
