About this file
The file contains Questions and Answers related to a Medical Testing Blanket Purchase Agreement (BPA) solicitation issued by the 910th Airway Wing of the Air Force Reserve Command. The Q&A clarifies specific requirements for medical examinations, including the use of specific government forms (OF178 and SF600) for comprehensive exams and confirming that a Treadmill Stress Test is not required for firefighter exams.
The solicitation (FA665625Q0007) is seeking a commercial service provider to conduct occupational health exams for reservist and civilian firefighters, including new hire physicals and associated lab tests. The BPA will be awarded using a lowest price technically acceptable evaluation method, with quotes due by 10:00 a.m. EST on 14 April 2025. The government intends to award a single BPA to a contractor registered in the System for Award Management (SAM), with a small business size standard of $20.5 million under NAICS code 621999. Quotes should be submitted via email to ashley.senich@us.af.mil or by mail to the specified address in Vienna, Ohio.

Question:
Is there a specific exam form contractors are required to use for these exams, or will our in-house exam forms suffice?
Answer:
Comprehensive exams need our specific forms/documents completed (i.e. workplace specific OF178 preselection and the SF600’s for baseline, periodic and termination exams). If the exam is a hybrid, some elements completed at the 910th and some through contracting then the contractor’s documentation should work.
Question:
Section 15 of the SOW references NFPA 1582 compliance. Will a Treadmill Stress Test be required as a part of the firefighter exam?
Answer:
NO, this is not part of the Airforce Technical Implementation Guidance regarding NFPA 1582.
