1.0 SCOPE. This SOW defines the requirements for the Contractor to engineer, furnish, install and test (EFI&T) Multi Mode Fiber Premise Wiring in Bldg 191 Secure Vault, Main Comm Room 226, ACMI Room 207, Conference Room, all User Office, and Briefing Areas at Homestead ARB. The Contractor shall provide all equipment, tools, materials, supplies, transportation, labor, supervision, management, and other incidentals necessary to meet the requirements as stated in this SOW. All equipment, supplies, and materials provided shall be new and not refurbished.

2.0 REQUIREMENTS.

2.1 GENERAL REQUIREMENTS.

2.1.1 Safety Requirements.

2.1.1.1 Site Coordination. The Contractor shall meet with the base safety officer immediately upon arrival on site for review of the specific safety requirements prior to installation.

2.1.1.2 Confined Space. The Contractors and Sub-contractors entering spaces on Homestead ARB are responsible for the safety of their personnel and for their own permit space program as outlined in OSHA 1910.146. All confined space operations must be coordinated with the Base Safety Office prior to start of work. The primary Contractor is responsible for all Sub-contractor confined space operations.

2.1.1.3 Accident/Incident Reporting and Investigation. The Contractor shall record and report all available facts relating to each instance of injury to either Contractor or Government personnel to the Base Safety Office unless otherwise stated in the SOW. The Contractor shall secure the scene of any accident and wreckage until released by the accident investigative authority through the Base POC. If the Government elects to conduct an investigation of the incident, the Contractor shall cooperate fully and assist the Government personnel until the investigation is completed.

2.1.2 Security Requirements.

2.1.2.1 Security Clearances. Stated work and associated products shall be performed in an UNCLASSIFIED area. Work in existing comm rooms will require escorts and coordination.

2.1.3 Environmental Management. The Contractor shall comply with the most stringent environmental federal, state, and local laws and regulations; and Air Force policies, instructions, and plans. The federal Government is not exempt from compliance with environmental regulations. The Contractor shall maintain an awareness of changing environmental regulatory requirements to avoid environmental deficiencies for activities on Homestead ARB. The Contractor shall ensure their Sub-contractors comply with these specifications.

2.1.4 Permits. The Contractor shall be responsible for coordination with Base Civil Engineers (CE) on completion of AF Form 103 Digging Permits before any digging is accomplished.

2.1.5 Integrated Product Team (IPT). The Contractor shall chair a weekly IPT meeting that includes Contractor representatives, the Government Contracting Officer (CO), the CSI-B, the System Engineer(s), the 38 ES Project Manager, the Homestead ARB CS Project Manager, and other base personnel as required. The Contractor shall provide an agenda and a worldwide “Meet Me” teleconference capability for the duration of the project. The purpose of the IPT meeting is to discuss project progress, problems being encountered, and other information necessary/beneficial to ensure success and timely completion of contract requirements. The Contractor shall record meeting minutes and distribute in accordance with (CDRL A004).

2.1.6 Quality Assurance. The Contractor shall provide a Quality Control Program for the life of the project. The Contractor’s quality assurance evaluator shall assist the Government representative in performing random spot checks and system acceptance tests. The Contractor shall be responsible for identifying system and outside plant deficiencies and/or discrepancies throughout the life of the project. A weekly report (soft copy) shall be submitted indicating progress/status and listing any deficiencies/discrepancies found and actions to correct them

(CDRL A003).

2.1.7 Contractor Personnel.

2.1.7.1 Project Management. The Contractor shall provide a Project Manager (PM) and alternate(s) responsible for contract performance and continuity. The Contractor shall identify the Project Manager’s or alternate's range of authority to act for the Contractor relating to daily contract operation.

2.1.7.2 Site Point of Contact (POC). The Contractor shall designate the Contractor's on-site team leader and alternate(s) as the Site POC for individual projects in their Site Visit Request Letter. The Site POC or alternate(s) shall be on site during duty hours until project completion.

The Site POC shall be the interface for all work site communications with the Government, including quality, safety, and discrepancy matters.

2.1.7.3 Personnel Requirements. The Project Manager, Site POC, and respective alternate(s) shall be able to read, write, speak, and understand English.

2.1.8 Warranty. The Contractor shall provide a one year warranty or manufacturer’s standard commercial warranty, whichever is longer. This warranty shall include a one year workmanship warranty. The warranty period shall start from the date of system and/or project acceptance.

The Contractor shall provide written procedures and required information for warranty services at or prior to site acceptance.

2.1.9 Manuals and Practices. The Contractor shall provide the latest version of operation, installation, and maintenance manuals and practices/users guide for each system installed as provided by the original manufacturer with all new equipment.

2.1.10 In-brief. The Contractor shall conduct an in-brief with the site POC or designated representative and other site personnel before the start of implementation. The in-brief shall familiarize the customer with the purpose of the Contractor's visit and coordinate the project schedule (e.g. arrange for required escorts; Base POC schedule; etc.).

2.1.11 Out-brief. The Contractor shall conduct an out-brief with the site POC or designated representative and other site personnel at the conclusion of the implementation. The implementation out-brief shall inform the customer of implementation results and necessary maintenance issues (including warranties).

2.2 SPECIFIC REQUIREMENTS.

2.2.1 Description of Specific Requirement. Contractor shall conduct site survey and consult with the customer upon the followings. POC for B191 Secure Vault is SMSgt NORTON, THOMAS G 482 FW/CVN, DSN 535-6739.

Unclassified MM fiber – 56 drops in Green color + 10% grow Secret MM fiber – 66 drops in Red color + 10% grow Secret/CASNET MM fiber – 70 drops in Blue (not aqua) color + 10% grow.

Note: 10% grow means customer has added/planned an additional drops since this project was surveyed.

Leave existing 6 strands SM uplink fiber cable “AS-IS” on the cable tray as this SM fiber may be used for future (identify and label this SM fiber). Install new 6 strands MM uplink fiber cable from Main Comm Room 226 to ACMI Room 207 and make termination.

Replace SM SFP modules with (CISCO) MM SFP modules LC connectors to 4507R and 3750 switches.

Replace SM NIC Cards with MM NIC Cards with LC connections in all the workstations/desktops in the Secure Area.

Reuse the fiber patch panel coupler (bulkhead) but terminate new MM fiber cable with SC connectors at the patch panel and replace with MM SC/LC patch cords of the corresponding color for the switches.

Replace all Comm outlets in the Secure Area with MM LC connector outlets. According to the customer, there are ten desktops in the ACMI area that need to be terminated with MM LC fiber outlets.

Follow existing cable tray in the false ceiling to replace any SM cabling with MM cabling.

Tags and re-labeling MM fiber hardware accordingly as previous conditions.

Re-pack all the existing SM SFP modules, SM NIC Cards and other SM associated hardware accordingly. These parts are subjected to be reused at different AF location, contact 482CS for proper disposition.

Upon completion, contractor shall provide as-builts for all comm jack locations and labeling scheme to include their comm room termination annotated on a printable floor plan. This should be in 2 forms, hard copy and soft copy.

2.2.2 Site Restoration/Debris Removal. Contractor is responsible for removal of all debris

2.2.3 Service Outages. The Contractor shall be responsible for preventing any unscheduled (i.e. cutting or disabling any in-service cables or equipment.), Contractor-caused, interruptions of communications capabilities that are properly identified. The Contractor shall coordinate planned outages with the site POC at least 1 day in advance of the outage if the implementation necessitates disruption of service, (e.g., communications, electrical, or other utilities).

2.2.4 Configuration Management. The Contractor shall perform configuration management of all equipment associated with the completed system. The Contractor shall provide configuration management information to the base SCX project manager. The configuration management information shall include, but not be limited to, a list of all components installed by system serial number (hardware), all passwords and if applicable software/IOS. The Contractor shall also provide schematics and wiring diagrams incorporating the new equipment and cabling into the existing configuration. The Contractor shall provide catastrophic failure recovery instructions including equipment initial and operational configuration instructions.

2.2.5 Identification/Marking. The Contractor shall clearly mark all Contractor-Furnished Property and Equipment (CFP/CFE) with their company's name. The Contractor shall place an easily read, very visible, sign (minimum 8.5" x 11") on large containers, construction equipment, or un-manned rental vehicles while on the Government installation indicating the company name and both the Contractor and Site POC's names and local telephone numbers.

2.2.6 Installation Schedules. The Contractor shall provide a complete milestone schedule that denotes project activities to include time-phased start and completion dates for the project and sub-projects associated with the installation of the components and system. (CDRL A002).

2.2.7 Weekly Status Reports. The Contractor shall prepare a Weekly Status Report and distribute IAW (CDRL A003). The purpose of the report is to inform IPT members of project progress, problems being encountered, and other topics necessary/beneficial to ensure success and timely completion of the contract requirements.

2.2.8 Red-Line and As-Built Drawings. The Contractor shall submit drawings showing the as-built configuration in format specified by base SCX project manager. The Contractor shall provide As-Built Rack Elevation, Inside Cable Plant and Outside Cable Plant drawings and distribute in accordance with (CDRL A001).

2.2.9 Test and Acceptance/Installation Test Plan. The Contractor shall provide a test plan as to how the system shall be tested to demonstrate to the Government that the system is fully operational and meets or exceeds the specified requirements and that the system is fully ready to be placed into service (CDRL A006). The Contractor shall test the system to demonstrate to the Government quality assurance evaluation of the system. These tests shall be accomplished prior to the system being placed into service.

2.2.10 Acceptance/Installation Test Report. The Contractor shall provide an installation test report of the results of the testing accomplished under the installation test plan (CDRL A005).

2.2.11 Final Acceptance. The Contractor shall schedule a final project walk-through with the Base POC. This should be scheduled 5 calendar days prior to acceptance.

2.2.12 Training Plan. Upon completion of the installation process, the Contractor shall provide User Training to the designated Site Staff in accordance with the customer requirements. User training shall include step-by-step instructions on the utilization of all features of the new system as well as basic system troubleshooting. The Contractor shall provide operational training to the users and system training to the Operators/Maintainers as needed. Instructional sessions shall be permitted to be videotaped for future training purposes. The Government shall retain the right to duplicate and distribute the training material for future training purposes. This training shall include setup, configuration, operations, and user diagnostic, repair and replacement (CDRL A007).

3.0 GENERAL INFORMATION.

3.1 Period of Performance. The period of performance is a base period of proposed project duration including installation, training, and testing & acceptance.

3.2 Place of Performance. The place of performance is HOMESTEAD ARB, FL.

3.3 Hours of Operation. The Contractor shall routinely work during normal duty hours of the site. However, mission requirements may necessitate work outside normal hours (nights and/or weekends), especially if existing service must be interrupted. Any site work requested by the Contractor to be performed outside of normal duty hours shall be coordinated with the Base POC at least 1 day in advance.

3.4 Holidays/Down Days. The Contractor shall not perform under this contract on federal holidays or site-unique down-days unless expressly authorized by the CO and coordinated with the base POC.

All deliverables are subject to Government acceptance and approval. They shall meet professional standards and the requirements set forth in this SOW. All deliverables shall be produced using recommended software tools/versions as accepted by the Government. The Contractor shall submit the following deliverables:

2. Test Report (for all lines)

3. Test Plan
