GENERAL
DESCRIPTION OF SERVICES
A1 Overview. The Service Provider (SP) shall provide all personnel, supervision, equipment, tools, materials, supplies, test equipment, and other items and services necessary to accomplish Supply, Vehicle Operations and Maintenance, Traffic
Management, Transient Maintenance, Real Property Maintenance, Fuels Management, Airfield Management, and Weather at Homestead ARB, FL as defined in this Performance Work Statement (PWS), except as specified in Section A-2, Government-Furnished Property and Services. All services, labor, supplies, materials, and parts are included in the Firm
Fixed Price unless otherwise specified in the contract. All days are in calendar days unless otherwise specified.
A1.1. Inherently Governmental Activity. Specific categories of PWS tasks, processes, and responsibilities, have been identified and partially restricted to performance by government employees. In general the SP is prohibited from committing or obligating the government and may not establish policy. Only government employees may perform and provide services or activities as related to the public interest that require either exercise of discretion in applying government authority or the making of value judgment in making decisions for the government. More specifically, government employees are responsible for performing overall functional management, administrative and managerial oversight for all PWS areas. While the SP shall provide supervision of SP employees, PWS performance and results;
participate in specific activities; prepare reports and analyses; provide input, technical advice, education and training;
and work with government representatives in all the functional areas listed above, it is only the totality of the tasks that are SP limited. SP performance and responsibilities must cease short of any point that could be reasonably construed as: Management oversight of the PWS functions; committing or obligating the government; validating results, and approval or direction of government activities.
A1.2 Compliance with Domestic Violence Amendment (hereinafter, “Lautenberg Amendment”) to the Gun
Control Act of 1968 (18 U.S.C. 922). The Lautenberg Amendment prohibits an individual who has a misdemeanor conviction for a crime of domestic violence from shipping, transporting, possessing or receiving firearms or ammunition, and prohibits sales or any other transfers of firearms and ammunition to such individuals, as described further in 18 U.S.C. 922. DoD policy extends this prohibition to individuals who have felony convictions for crimes of domestic violence.
The SP shall not permit individuals who have a misdemeanor or felony conviction for a crime of domestic violence to perform any work under the instant contract which would require them to perform activities in contravention of 18
U.S.C. 922 or DoD policy (e.g., which would permit or require such individuals to ship, transport, possess, or receive firearms or ammunition in the course of performing their duties under the contract).
Upon becoming aware that an employee has a conviction for a felony or misdemeanor crime of domestic violence
(hereinafter “qualifying conviction”), or upon having reasonable cause to believe that an employee has a qualifying conviction, the SP shall immediately deny such employee access to firearms and ammunition in the course of the employee’s performance of duties under the contract. The SP shall also immediately retrieve any Government-owned or Government-controlled firearms or ammunition in the employee’s possession and return same to the Government.
The SP shall immediately contact the local Administrative Contracting Officer (ACO) for further guidance as to required actions. If any SP employee is prohibited pursuant to 18 U.S.C. 922 or DoD policy from performing the duties of his or her position, the SP is responsible for substituting an acceptable employee in the vacated position. The
SP is solely responsible for any delays, contract performance disruption or increased costs attributable to any substitutions required by this provision. The Government also reserves the right to disallow a SP employee from performing his or her SP-assigned duties if the Government becomes aware of information from any source that any
SP employee is prohibited from performing his or her duties due to the prohibitions of 18 U.S.C. 922 or DoD policy.
In such a case, the SP and ACO shall meet to determine an acceptable course of action, to include the substitution of an acceptable employee as appropriate. The SP is solely responsible for any delays, contract performance disruption or increased costs attributable to the substitution. The SP shall require any employee proposed by the SP to be placed in and perform the duties of any covered position to certify in writing that he/she does not have a qualifying conviction, prior to that SP employee assuming the duties of the Covered Positions as defined in Technical Exhibit A-TE-5. Re-certification shall be accomplished by employees occupying covered positions annually or whenever events change the accuracy of a previous certification. These certifications shall be maintained by the SP for review by the
Government upon request.

A2 Personnel. This PWS sets forth minimum qualifications for certain positions. The SP shall employ only personnel who meet or exceed any such minimum qualifications. The resumes and any additional documentation (required certifications, diplomas, etc.) for the personnel proposed to fill the contract manager, functional area manager and all other positions requiring minimum qualifications shall be provided in accordance with A-TE-3, A01. Documentation shall also be provided during contract performance when changes are made in the personnel occupying these positions. HQ AFRC/A7K shall provide written confirmation of all personnel proposed to fill a position where minimum qualifications are specified. Proposed personnel shall not report to work until confirmation is received from HQ AFRC/A7K.
A2.1 Certifications and Licenses. Prior to beginning work, the SP shall ensure employees performing work under this
PWS have and maintain certifications and licenses acceptable in the commercial market and required by federal, state, and local laws. The SP shall maintain certifications on site and make available for government review. Certifications and licenses shall be at the expense of the SP.
A2.1.1 Project Manager (PM). The SP shall provide a full-time PM who shall be physically on-site during normal work hours, and be responsible for performance of all work. The name of this person and an alternate or alternates who shall act for the SP when the manager is absent, shall be designated in writing to the Contracting Officer (CO). The PM and alternate shall have full authority to contractually bind the SP for prompt action on matters pertaining to execution of the Contract.
The PM shall possess a minimum of three years experience within the last six years simultaneously managing three or more functions of base operating support services, or the commercial equivalent, of similar magnitude and scope as the total requirement of this PWS.
A2.1.2 Language Requirements. Any SP employee who will interface with government representatives shall be able to read, write, speak, and understand the English language to the extent necessary in the performance of this PWS.
A2.1.3. SP Employees. The SP shall not employ individuals identified to the SP by the CO as potential threats to the health, safety, security, or general well being, of the population, installation, or operational mission. The SP’s employees shall not pose an actual or potential threat or risk to the security or mission of the AFRC or its supported activities, or the health and safety of the AFRC workforce. The government has an inherent right under law, practice, and regulation to restrict and control access to its facilities, including those that are the subject of this PWS. The government’s exercise of its right to grant and revoke access to particular individuals to its facilities shall not constitute a breach or change to this PWS, regardless of whether said individuals are employed by the SP. The SP shall also comply with the Department of Defense (DoD) smoking policy, AFI 40-102, Tobacco Use in the Air Force, and Air Force Reserve Command (AFRC) smoking policies within DoD facilities.
A2.1.3.1 The SP personnel shall present a neat appearance, and be easily recognizable as SP employees. The SP shall provide all SP employees with an identifying badge, which shall include as a minimum, the person’s name, picture, and the
SP’s name. While on the installation, the SP employees shall wear this picture identification at all times in a clearly visible manner. The SP shall obtain restricted or controlled area badges from the Homestead Security Forces (SF) in order to gain access to certain restricted areas. The SP shall ensure badges do not present a foreign object damage/debris (FOD) potential on the flightline.
A2.1.3.2. Employee Rosters. The SP shall provide an employee roster in accordance with A-TE-3, A02
A2.1.3.3. The SP shall maintain procedures for the immediate removal of employees for misconduct or other causes prejudicial to the maintenance of health, welfare, morale or security of the facilities, populace, and computer system information. The SP shall exercise these provisions when notified by the CO that action is necessary.
A2.1.3.4 Contractor Full-Time Equivalent Reporting. The contractor shall report ALL contractor labor hours (including subcontractor labor hours) required for performance of services provided under this contract via a secure data collection site. The contractor is required to completely fill in all required data fields at http://www.ecmra.mil.
Reporting inputs will be for the labor executed during the period of performance for each Government fiscal year (FY), which runs 1 October through 30 September. While inputs may be reported any time during the FY, all data shall be reported no later than 31 October * of each calendar year. Contractors may direct questions to the CMRA help desk.

*Report Period: Contractors are required to input data by 31 October of each year. However, due to the delayed implementation, input of FY12 data is required by 31 Mar 13.
Uses and Safeguarding of Information: Information from the secure web site is considered to be proprietary in nature when the contract number and contractor identity are associated with the direct labor hours and direct labor dollars. At no time will any data be released to the public with the contractor name and contractor number associated with the data.
User Manuals: Data for Air Force service requirements must be input at the Air Force CMRA link. However, user manuals for government personnel and contractors are available at the Army CMRA link at http://www.ecmra.mil.
A2.2 Employment:
A2.2.1 Current Government Employees. All current government employees hiring shall be IAW DoD 5500.7-R, Joint
Ethics Regulation, and AFI 64-106, Air Force Industrial Labor Relations Activities.
A2.2.2 Off-Duty Active and Reserve Military Employees. The SP is cautioned that off-duty active military personnel hired may be subject to permanent change of station, change in duty hours, or deployment. Military reservists and National
Guard members may be subject to recall to active duty. The abrupt absence of these personnel could adversely affect the
SP’s ability to perform; however, their absence at any time shall not constitute an excuse for non-performance under this contract.
A2.2.3 Employee Training. The SP is responsible for providing fully-trained and qualified journeyman or craftsman level workforce IAW the clauses included in the contract. Therefore, except as otherwise indicated, the SP shall provide at its expense any required employee training, including the cost of instruction, per diem, travel, salaries, and benefits. If the SP-identified training is available only from government sources, requests for such training shall be submitted to the ACO IAW AFCAT 36-2223, USAF Formal Schools, and the SP shall reimburse the government for all costs associated with the training.
A2.2.3.1 Employee Training Records. Training and qualification must be documented IAW AFSC requirements.
A3 Quality Assurance/Control.
A3.1 Quality Assurance (QA). In accordance with the Inspection of Services clause, (“Inspection of Services - Fixed Price”) incorporated by reference in Contract Section E, the Government will evaluate the SP’s performance under this contract. For those tasks listed on the Service Delivery Summary (SDS) (Technical Exhibit 1, Tabs B thru I), Quality Assurance Evaluator (QAE), Quality Assurance Specialist (QAS) and Contracting Officer (Technical) Representative (COTR/COR), will follow the methods of surveillance specified for this contract. Government personnel will record all surveillance observations. The SP is responsible, and therefore subject to government surveillance, for all of the performance requirements in the PWS. When an observation indicates defective performance, the COR will require the SP at the site to initial the observation to acknowledge that the SP has been made aware of the defective performance.
Government surveillance of tasks not listed in the SS may occur during the performance period of this contract. Such surveillance will be done according to standard inspection procedures or other contract provisions. Any action taken by the ACO as a result of surveillance will be according to the terms of this contract.
A3.2 Quality Control Plan (QCP). The SP shall submit a final QCP IAW A-TE-3, A03. The records of inspections shall be kept and made available to the ACO and COR throughout the contract performance period and for the period after contract completion until final settlement of any claims under this contract.
A3.2.1 The QCP shall contain a Customer Satisfaction Measurement Plan. The plan shall demonstrate the SP’s ability to measure customer satisfaction using Point of Service Surveys and Statistically Valid Random Sampling for each PWS performance objective and as specified in Technical Exhibit-1, Services Summary, Tabs B-H.
A3.2.2 In the plan, the SP shall clearly articulate how the SP proposes to address services that will be surveyed in a general population customer survey; services that will be administered under a point of service survey; survey tools and process (e-mail, paper, telephone or other); random sampling methodology; sample size determination; survey distribution methods; definition of survey scale and “acceptable” performance thresholds; and methods of preparation, coordination, reviewing, distribution, and retention of reports.

A3.3. Performance Management.
A3.3.1 The SP shall meet as often as required with the Multi-Functional Team (MFT) to review government and SP’s performance data. The MFT will evaluate and discuss with the SP the results of performance measurement data for possible action, as required. The SP shall be responsible for making the required changes in processes and practices to ensure performance is managed effectively. The SP shall formulate tactical as well as long-term courses of action jointly with the MFT. A member of the MFT will record meeting minutes, and the ACO will sign the minutes.
A3.3.2 In compliance with the contract clause entitled “Inspection of Services-Fixed Price” incorporated by reference in Section E of the solicitation/contract, the SP shall submit an initial draft of a written QCP, with severable sections addressing each PWS Tab, to the ACO NLT 30 days prior to contract performance. The ACO will notify the SP of acceptance or required revisions within 10 days of receipt. The SP shall submit the finalized plan to the ACO 10 days after receipt of notification of required revisions and provide updates to the ACO as changes occur.
A3.4 Customer Complaints: The SP shall ensure that a customer complaint process is established and advertised. The SP shall track, report, and inform the MFT of the resolution of customer complaints.
A4 Security.
A4.1 Security Requirements. The SP shall comply with all security regulations and directives as identified herein and other security requirements as shown elsewhere in this contract. The SP shall comply with the DD Form 254, DoD Contract
Security Classification, attached to this contract.
A4.1.1 Clearance Requirements. The contractor must possess or obtain an appropriate facility security clearance of SECRET prior to performing work on a classified government contract. If the contractor does not possess a facility clearance the government will request one. The government assumes costs and conducts security investigations for security clearances. The contractor shall request security clearances for personnel requiring access to classified information within 15 days after receiving a facility clearance or, if the contractor is already cleared, submissions shall be made within 15 days after contract award. Due to costs involved with security investigations, requests shall be kept to an absolute minimum necessary to perform contract requirements.
A4.1.2 SP employees who already possess a security clearance on a previous contract, granted under DoD 5220.22-M, National Industrial Security Program Operating Manual, qualify for unescorted entry into USAF restricted areas, use of unclassified automated information systems, and access to the base network. Validation of the favorable investigation shall be required from the SP’s home office facility security officer before a restricted area badge is issued, use of unclassified automated information systems is approved, or access to the base network is allowed.
A4.1.3 All SP employees requiring access to classified information or materials shall (1) possess a valid and appropriate security clearance, (2) have executed an SF 312, Classified Information Non-Disclosure Agreement, and (3) have a valid need for access to the information to perform a lawful and authorized government function IAW AFI 31-501, Personnel
Security Program Management.
A4.2 Visitor Group Security Agreement (VGSA). The SP shall enter into a VGSA with the installation Commander.
The SP shall comply with the provisions of all applicable Air Force and Homestead ARB instructions. The Homestead
ARB Information Security Program (ISPM) will sign on behalf of the installation commander.
A4.3 Inventory of Classified Documents and Material. No later than 5 days prior to the start of the first operational performance period or notification of issuance of security clearances to required SP personnel, whichever occurs last, the
SP and the government representative, as designated by the CO, shall conduct a joint inventory of all classified documents and material to be inventoried. The SP shall receipt for all items on the inventory.
A4.4 Unescorted Entry Access and Processing.
A4.4.1. SP employees who require unescorted entry into restricted areas, access to sensitive information areas, or equipment shall comply with the investigative requirement IAW AFI 31-501, Personnel Security Program Management.

A4.4.2. To obtain unescorted entry access to restricted areas it is necessary, at a minimum to obtain favorable results from a National Agency Check and Inquiries Investigations (NACI) investigation. AF 2586, Unescorted Entry Authorization
Certificate, will be required for issuance of the restricted area badges (AF Form 1199, USAF Restricted Area Badge) for those personnel whose duties require frequent access to the flight line or other restricted areas. The Security Forces Pass and Registration section may be contacted for instructions on completing the form.
A4.4.3 Organizational commanders of the host organization identify the need and justify to the installation commander, through the local Servicing Security Activity, requests for NACI’s on SP employees requiring unescorted entry to restricted areas. The commander grants the unescorted entry upon recommendation by the servicing security activity. Installation commanders approve all denials or withdrawals of unescorted entry for SP employees.
A4.5 Automated Information Systems/Base Network
A4.5.1 Contractor personnel shall successfully complete, as a minimum, a National Agency Check and Inquiries
(NACI) investigation, before operating government furnished workstations that have access to Air Force e-mail systems. These investigations shall be submitted by the government at no additional cost to the contractor. The contractor shall comply with the DoD 5200.2-R, Personnel Security Program, and AFI 33-119, Electronic Mail (E-
Mail) Management and Use, requirements.
A4.5.2 SP employees may be granted interim access to automated information systems and the base network based on favorable completion of local personnel, law enforcement, or other security records and the initiation of the NACI.
Installation commanders approve all denials or withdrawals of use of the automated information systems or the base network for SP employees. All SP employees who are granted interim access shall receive mandatory information assurance training by the government; training shall be documented by the SP. Government System Administrators will implement measures to limit access to the information required to conduct assigned duties.
A4.5.3 Processing of National Agency Check and Inquiries Investigations. The Personnel Security section, 482
FW/IPP, will process and forward requests for SP NACIs to the Office of Personnel Management (OPM) via the Electronic
Questionnaires for Investigation Processing (e-QIP) system. The personnel security office will fingerprint the SP employee on a DD Form 258, Fingerprint Card.
A4.5.4 The 482 FW/IPP will notify the program manager and/or contracting officer with the results of the NACI when investigations are returned from the Air Force Central Adjudicative Facility with an eligibility of “No
Determination Made”. When an eligibility of No Determination is made the contractor group shall not allow its employees access to sensitive information, base network, or restricted areas until final determination is made by the installation commander or designee.
A4.5.5 Pending the results of a favorable NACI, contractor employees shall work in restricted areas only with government escorts. When the Government is in the process of conducting a NACI investigation, and that individual's employment is terminated before the investigation is completed, the contractor shall immediately forward a written notice of the individual's termination to the Personnel Security Section.
A4.6 COMSEC. The SP shall ensure proper utilization and management of COMSEC material to include receipt, accountability, safeguard, destruction, storage, and investigation, IAW AFI 33-201, Volume 2, Communications
Security (COMSEC) User Requirements, AFI 33-201, Volume 4, Cryptographic Access Program, AFI 33-201, Volume 5, Controlled Cryptographic Item (CCI), AFSSI 4212, Reporting COMSEC Deviations, AFI 31-401, Information Security Program Management, and DoD 5300.1-R. The SP shall ensure all COMSEC materials are accounted for and documented on an AFCOMSEC Form 16, COMSEC Account Daily-Shift Inventory, at each shift change. The SP shall initiate investigation procedures for unaccounted COMSEC material IAW AFI 33-201, Volume 2, Section J.
A4.6.1 The SP shall appoint a primary and alternate Communications Security Responsible Officer CRO IAW AFI
33-201, Volume 2, Paragraph 4 using the sample appointment letter for COMSEC Responsible Officers and

Alternates in Attachment 2. The SP shall also provide a communication security access list to the base COMSEC manager IAW AFI 33-201, Volume 2, Paragraph 18.3 and Attachment 4.
A4.6.2 The SP is responsible for developing a COMSEC Operating Instruction (OI) IAW AFI 33-201, Volume 2, Paragraph 6. (See A-TE-3, A10).
A4.6.3 The OI shall include provisions for conducting COMSEC operations and safeguarding COMSEC material;
specific for the users activity; procedures for cryptographic operations; local accountability for COMSEC material;
COMSEC maintenance support, access restriction; storage, routine and emergency destruction; incident reporting, and procedures for relieving people who have received COMSEC material from accountability when they are released from employment; and end of shift inventory using AF Form 16.
A4.6.4 Training is mandatory for all personnel listed on the access list. Ensure all personnel with authorized access know how to handle, control, inventory, and use COMSEC material, to include how to report COMSEC deviations.
Ensure that all personnel are familiar with correct procedures in operating associated cryptographic equipment utilizing applicable AFKAOs, KAOs, AFSSIs, or similar instruction provided by the base COMSEC Account
Manager. Refresher training will not exceed 365 days. Document annual training requirements by completing a new
AF Form 4168.
A4.7 Physical Security. The SP shall safeguard all government property IAW AFI 31-101, The AF Installation Security
Program, Chapters 18 thru 26, and controlled forms provided for SP use. At all times, facilities, equipment, and supplies shall be secured. The SP shall immediately report all thefts, vandalism, or destruction of property and equipment
(Government or SP-owned) to the CO.
A4.7.1 Key Control. The SP shall establish and implement methods of making sure all keys issued by the government to the SP for SP employee use are not lost or misplaced and are not used by unauthorized persons. The SP shall develop procedures covering key control that shall be included in the QCP. Such procedures shall include turn-in of any issued keys by personnel who no longer require access to locked areas. The SP shall immediately report to the COR any occurrences of loss, unauthorized use, or unauthorized duplication of keys. The SP shall control access to all government provided lock combinations to preclude unauthorized entry.
A4.7.2 Corrective Actions. In the event keys, other than master keys, are lost or duplicated, the SP may be required, upon written direction of the CO, to re-key or replace the affected lock or locks without cost to the government. In the event a master key is lost or duplicated; the SP at no cost to the government will replace all locks and keys for that system.
A4.7.3 Lock Combinations. The SP shall establish and implement methods of ensuring that lock combinations are not revealed to unauthorized persons. These procedures shall be included in the SP’s QCP.
A4.7.4 Tool and Equipment Control Accountability Program. The SP shall submit a Tool Control and
Accountability Plan IAW A-TE-3, A13. All functions within the responsibility of the contractor shall comply with the contractor’s approved tool and equipment control and accountability program. (See A-TE-3, A13)
A4.8 Operations Security (OPSEC). SP shall appoint an OPSEC coordinator and an alternate. SP will participate in the OPSEC program locally administered by the AF activity. The OPSEC coordinator will ensure levels of standardized OPSEC training and education are established and provided to all visitor group personnel who have access to mission critical information. SP OPSEC coordinators shall maintain a SECRET clearance and follow guidance contained in AFI 10-701, Operations Security, and local sups.
A4.9 Tool and Equipment Control Accountability Program. The SP shall submit a Tool Control and Accountability
Plan IAW A-TE-3, A12. All functions within the responsibility of the SP shall comply with the SP's approved tool and equipment control and accountability program.
A5 Hours Of Operation. The SP core hours of operations shall be 0730L to 1630L hours, Monday through Friday, excluding recognized days for federal holidays, unless otherwise specified in Tabs B thru I. All times in this PWS are specified in Local (L) time on a 24 hour clock. The SP shall ensure that service to customers is not interrupted during break and lunch periods. Variances in operating hours, other than that specified herein, must be approved by the CO. The
A-7 government retains the right to change normal duty hours at any time. Unless otherwise specified in Tabs B thru I, the SP will not be required to perform services on the following recognized holidays or when the base is officially closed by direction of the Base Commander:

A5.1 Call-out and After-Hours Services Under Routine Operations. The SP shall provide fully qualified personnel, accessible by phone, on a 24-hour a day, 7 days a week basis, to perform unscheduled mission or emergency requirements.
For unscheduled mission requirements, SP personnel shall respond within 15 minutes of the initial notification and be on-site within one hour after the initial response in order to perform services and satisfy unscheduled mission requirements.
A5.2 Reservist Support and Access. The SP shall provide support and access to reservists on UTAs as specified in Tabs
B thru I. Homestead ARB conducts one (1) primary and (1) alternate UTAs each month. Except that work described in
Tab F, the government will not pay any overtime or over and above for SP support specified in the PWS as being required during normal hours on UTAs. SP support shall be in the manner of granting access to reservists, providing training, and certification of training. The SP shall grant access to reservists for training use of any and all Government Furnished Property
(GFP) listed in Tabs A-I, Technical Exhibits TE-4.
A6 Emergency Or Special Situation Services. Services may be required to support an activation or exercise of contingency plans or emergency services. Emergency and disaster situations (example: accident and rescue operations, civil disturbances, natural disasters and military peacetime/wartime contingency operations, air shows, and exercises) may necessitate an increase in SP effort. This effort will be ordered by the CO under the Over and Above CLIN and corresponding option CLINs set forth in Section B of this contract, with the exception of effort performed under Tab-F, which will be ordered under the Labor for Service Call CLIN of the contract.
A6.1 Upon notification by the CO, the SP shall provide services to support base, local or national commitments, up to and including a declared war, that would require emergency or contingency support. These services may be required 24 hours a day, 7 days a week for the duration of the emergency. Requirements beyond the capability of the SP may be augmented by the Government, at its option, when it perceives that mission accomplishment is endangered. Any such augmentation will be subject to the provisions of AFRCFARS 5352.237-9101, Rights of Government to Perform Functions With Its Own
Personnel. The CO may advise the SP of the required level of effort verbally and follow up immediately with written direction.
A6.2 When multiple contingency plans generate conflicting support requirements, the installation Crisis Action Team
(CAT) will establish the priority for the events as they occur. The SP project manager will be notified by either the CAT or the CO of those priorities.
A6.3 Performance of Services During Crisis Declared by the Secretary of Defense or Overseas Combatant
Commander (OCC). All services identified in this PWS are considered critical services during a crisis. The SP shall continue providing services 24 hours a day until the crisis is over. The SP shall ensure enough skilled personnel are available during a crisis for any operational emergency. A crisis management plan shall be submitted IAW A-TE-3, A04. The SP will be notified by the CO should a crisis be declared affecting needed SP support.
A6.4 Full Spectrum Threat Response (FSTR). The SP shall support and participate in the Homestead ARB FSTR program and appoint FSTR representatives as needed for program coverage IAW AFMAN 32-4004, Emergency Response
Operations.
A6.4.1 Unit Control Center (UCC). The SP shall establish UCCs to monitor unit resources, mission capability and coordinate unit activities during disaster or contingency operations such as accidents, natural disasters, mobility and recovery requirements and during all planned exercises.

A6.4.1.1 Specialized Team. The SP shall appoint specialized FSTR team members (Disaster Control Group (DCG), Contamination Control Teams (CCT), Shelter Management teams (SMT) and support teams) to support local response and contingency plans. The SP shall perform functions necessary to minimize the loss of personnel, resources, and operational capability caused by accidents, natural disasters and contingencies.
A6.4.1.2. Response Procedures. The SP shall develop and implement response procedures and checklists to support
Homestead ARB FSTR OPLAN 32-1, Homestead ARB Disaster Preparedness Operations Plan, other local response plans, as well as war and contingency planning documents. Checklists shall be drafted and forwarded for approval prior to use.
The SP shall review checklists annually to ensure accuracy and coverage. (See A-TE-3, A09).
A6.4.2 FSTR Equipment. The SP shall obtain, maintain and store equipment and supplies needed for response and recovery operations for members of their team. This includes all elements of the Disaster Response Force (DRF) including
UCCs, DCG members, CCTs, shelters, SMTs and support teams.
A6.4.3 Base Shelter Program. Shelters as identified in Homestead ARB FSTR OPLAN 32-1, Homestead ARB Disaster
Preparedness Operations Plan, shall be maintained as such for use during contingency situations, natural disasters, and exercises by the owners and users of the shelter.
A6.5 HAZMAT Spill Notification And Response Procedure. This procedure applies when the SP causes or discovers a spill, release or leakage of any HAZMAT substance, which includes but is not limited to: Petroleum, Oil, Lubricant (POL) fuel, waste oil, hazardous material, hazardous waste, Aqueous Film Forming Foam (AFFF) liquid, sewage, toxic gases or vapors, which could potentially damage or endanger the environment, or threaten public health or personnel safety. Specific non-POL chemicals are listed on the Hazardous Substance list in 40 CFR 302.4, Designation of Hazardous Substances and on the Extremely Hazardous Substance list in 40 CFR 355.40, Emergency Release Notification. Reporting and response for all hazardous material releases shall be IAW the Hazardous Material Emergency Planning and Response Plan for Homestead
ARB and notification to the Base Fire Department at ext 7117. Additional requirements are as follows:
A6.5.1 If the SP causes a spill, then financial liability for all spill related costs shall rest solely with the SP. In the case of pre-existing environmental contamination within a spill area, the SP is only liable for costs related to a spill which they caused. Such liability shall include, but is not limited to: Spill response costs, spill area cleanup and remediation costs, spill waste disposal costs, and any penalties that may be assessed by regulatory agencies as a result of the spill.
A6.5.1.1 If the SP discovers a spill, which is not caused by the SP, then financial liability for all spill related costs shall not rest with the SP, unless determined otherwise by a fair and reasonable investigation of the spill.
A6.5.1.2 Regardless of whether the SP causes or discovers a spill, the SP shall comply with the procedure, that was developed from the base HAZMAT Emergency Planning & Response Plan for Homestead ARB.
A6.5.1.3 When cleaning up a spill, use proper absorbent media for HAZMAT. Appropriate use of granular clean up material such as kitty litter or speedy dry is acceptable in conformance with the methods and procedures as described in the HAZMAT Plan.
A6.5.2 If the spill can be contained and controlled by the SP without undue risk of injury to personnel and without assistance from the base Fire Dept or other HAZMAT response team, then the SP shall perform the following:
A6.5.2.1 Immediately upon recognizing a spill, use materials from any available spill kit or other equipment as required, to contain and control the spill.
A6.5.2.2 Upon observation of a spill, the SP shall notify the base Fire Dept at extension 7117 and provide (1) the name of person reporting spill; (2) the exact location of spill; (3) any personnel injuries; (4) the substance spilled; (5) an estimate of amount spilled; (6) the date and time of spill; (7) the status of spill containment and control; (8) and the status of spill cleanup. The SP shall provide a courtesy notification concerning the spill to CEV.
A6.5.2.3 If the spill can be cleaned up by the SP, and there is no environmental (soil or water) contamination, and no excavation is required, then perform the following:

A6.5.2.3.1 Use absorbent materials from any available spill kit or other equipment as required, to collect and clean up spilled substances as much as possible.
A6.5.2.3.2 Do not hose down spilled substances into floor drains or storm drains.
A6.5.2.3.3 Properly package and dispose of spilled substances and waste materials generated during the spill response or clean up, IAW the Hazardous Material Emergency Planning and Response Plan.
A6.5.2.4 If the spill cannot be cleaned up by the SP, or there is environmental contamination, or excavation is required, the
SP shall notify the base CEV office at ext 7163 for technical assistance. CEV will determine which organization, other than the SP, is going to perform the spill cleanup, and will ensure that the cleanup methods used are in compliance with applicable environmental regulations.
A6.5.3 If the spill cannot be contained and controlled by the SP, or involves undue risk of injury to personnel, or requires assistance from the base Fire Dept or other HAZMAT response team, then the SP shall perform the following:
A6.5.3.1 Immediately upon recognizing a spill, notify the base Fire Dept at ext 7117 and provide the same information as listed in paragraph A.6.5.2.2 above.
A6.5.3.2 Stop the source of a spill in progress, if possible without undue risk of injury to personnel, using materials from any available spill kit or other equipment as required.
A6.5.3.3 Ensure that all employees in spill area, shut down their work operations and secure their equipment as required.
Restrict all sources of ignition.
A6.5.3.4 Make the spill area “Off Limits” to unauthorized personnel. Evacuate to an area upwind of the spill, if appropriate for the type of substance spilled.
A6.5.3.5 Report all known spill information to the On Scene Commander, Incident Commander, or Senior Fire Official on duty, as appropriate when they arrive at the area.
A6.5.3.6 If the spill resulted in environmental (soil or water) contamination, or excavation is required for clean up, then notify the base CEV office at ext 7163 for technical assistance. CEV will determine which organization, other than the SP, is going to perform the spill cleanup, and will ensure that the cleanup methods used are in compliance with applicable environmental regulations.
A7 Conservation Of Utilities. The SP shall ensure employees practice utility conservation. The SP shall operate under conditions that prevent the waste of utilities.
A8 Government Observations. As approved by the CO, government personnel other than COs and COR may from time to time observe SP operations. However, these personnel will not interfere with SP performance.
A8.1 Regulatory Observation. Outside regulatory agencies such as Department of Environmental Protection (DEP), Environmental Protection Agency (EPA), local conservation commissions may inspect the installation unannounced. SP shall cooperate fully with these agencies. In the event the SP receives a deficiency report or other derogatory finding, upon
CO direction, the SP shall take immediate action to correct the deficiency. Any cost associated with the finding due to SP’s negligence shall be at the SP’s expense.
A8.2 Records Management. All data created for Government use and delivered to, or falling under the legal control of the Government are Federal records and shall be managed IAW objectives and policies of the DoD, AF directives and the records management legislation as codified at 44 U.S.C., Public Printing and Documents, Chapters 21, 29, 31, and 33, the
Freedom of Information Act (5 U.S.C. 552), and the Privacy Act (5 U.S.C. 552a), and shall be scheduled for disposition
IAW 36 CFR part 1228. When data deliverable includes electronic records, the SP must provide and deliver sufficient technical documentation to permit Government agencies to use the data. If requested by the government, the SP shall
