In order to be determined responsive, an offeror must submit all documentation to demonstrate the following:
a. Acknowledgement of compliance with all solicitation terms and conditions by completing and signing all pages of this form.
b. Acknowledgement of all solicitation’s amendments by signing all pages of this form and/or each published amendment.
c. Price and any discount terms by completing the designated pricing and discount section of this form.

Warranty: The quoter shall provide the applicable express warranties as a separate document that shall be included in the final contract by attachment.
Terms and content of your quote: Your quote must be based on the terms of this RFQ. We may reject any quote that does not meet these terms in every respect.

Issuance of Purchase Order: The Government intends to evaluate quotations and issue a purchase order based on the initial quotations received. Therefore, the offeror’s initial quotation should contain the offeror’s best terms. The Contracting Officer may reject any or all quotes. After the evaluation of initial quotes received, the Contracting Officer may negotiate final terms with one or more offerors of the Government's choice before issuing a purchase order. The Contracting Officer will not negotiate with any offerors other than those of the Government's choice and will not use the formal source selection procedures described in RFO Part 15. The Contracting Officer will issue a purchase order to the vendor in accordance with the evaluation criteria cited in 52.212-2 below. RFO Provision 52.212-2, Evaluation — Commercial Products and Commercial Services (Dec 2025), applies to this acquisition.
According to RFO 12.205(a)(2), the following addendum is provided for this solicitation and hereby amends any language therein:
Evaluation factors. The Government will award a contract resulting from this solicitation to the responsible Offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers:
Price: The reasonableness of the quotation will be determined on the total evaluated price (TEP). Quoters should give their most competitive price. Additionally, as this requirement is commercial and will result in a "firm-fixed-price" a comparison of quoters' total price will satisfy the requirement for price analysis, in determining the price fair and reasonable. The Government may determine that an offer is unacceptable if the prices are significantly unbalanced.
Technical Exhibit: this factor evaluates how well the product or technical literature provided will meet the needs of the government. The submitted technical literature shall explicitly demonstrate the company capabilities to provide the product or service and therefore meet the requirements as set forth in the enclosed Technical Specification. A quote that exceeds the government’s minimum requirement may be considered for award and the best value determination will be made.
Proposed Delivery Schedule: this factor evaluates how favorable is the delivery schedule of the offered items.
