SECTION SF 30 BLOCK 14 CONTINUATION PAGE
SUMMARY OF CHANGES
SECTION SF 1449 - CONTINUATION SHEET
The following have been modified:
52.212-1 INSTRUCTIONS TO OFFERORS--COMMERCIAL ITEMS (JAN 2017)
(a) North American Industry Classification System (NAICS) code and small business size standard. The NAICS code and small business size standard for this acquisition appear in Block 10 of the solicitation cover sheet (SF 1449). However, the small business size standard for a concern which submits an offer in its own name, but which proposes to furnish an item which it did not itself manufacture, is 500 employees.
(b) Submission of offers. Submit signed and dated offers to the office specified in this solicitation at or before the exact time specified in this solicitation. Offers may be submitted on the SF 1449, letterhead stationery, or as otherwise specified in the solicitation. As a minimum, offers must show--
(1) The solicitation number;
(2) The time specified in the solicitation for receipt of offers;
(3) The name, address, and telephone number of the offeror;
(4) A technical description of the items being offered in sufficient detail to evaluate compliance with the requirements in the solicitation. This may include product literature, or other documents, if necessary;
(5) Terms of any express warranty;
(6) Price and any discount terms;
(7) "Remit to" address, if different than mailing address;
(8) A completed copy of the representations and certifications at FAR 52.212-3 (see FAR 52.212-3(b) for those representations and certifications that the offeror shall complete electronically);
(9) Acknowledgment of Solicitation Amendments;
(10) Past performance information, when included as an evaluation factor, to include recent and relevant contracts for the same or similar items and other references (including contract numbers, points of contact with telephone numbers and other relevant information); and
(11) If the offer is not submitted on the SF 1449, include a statement specifying the extent of agreement with all terms, conditions, and provisions included in the solicitation. Offers that fail to furnish required representations or information, or reject the terms and conditions of the solicitation may be excluded from consideration.
(c) Period for acceptance of offers. The offeror agrees to hold the prices in its offer firm for 30 calendar days from the date specified for receipt of offers, unless another time period is specified in an addendum to the solicitation.
(d) Product samples. When required by the solicitation, product samples shall be submitted at or prior to the time specified for receipt of offers. Unless otherwise specified in this solicitation, these samples shall be submitted at no expense to the Government, and returned at the sender's request and expense, unless they are destroyed during preaward testing.
(e) Multiple offers. Offerors are encouraged to submit multiple offers presenting alternative terms and conditions, including alternative line items (provided that the alternative line items are consistent with subpart 4.10 of the Federal Acquisition Regulation), or alternative commercial items for satisfying the requirements of this solicitation.
Each offer submitted will be evaluated separately.
(f) Late submissions, modifications, revisions, and withdrawals of offers:
(1) Offerors are responsible for submitting offers, and any modifications, revisions, or withdrawals, so as to reach the Government office designated in the solicitation by the time specified in the solicitation. If no time is specified in the solicitation, the time for receipt is 4:30 p.m., local time, for the designated Government office on the date that offers or revisions are due.
(2)(i) Any offer, modification, revision, or withdrawal of an offer received at the Government office designated in the solicitation after the exact time specified for receipt of offers is “late” and will not be considered unless it is received before award is made, the Contracting Officer determines that accepting the late offer would not unduly delay the acquisition; and--
(A) If it was transmitted through an electronic commerce method authorized by the solicitation, it was received at the initial point of entry to the Government infrastructure not later than 5:00 p.m. one working day prior to the date specified for receipt of offers; or
(B) There is acceptable evidence to establish that it was received at the Government installation designated for receipt of offers and was under the Government's control prior to the time set for receipt of offers; or
(C) If this solicitation is a request for proposals, it was the only proposal received.
(ii) However, a late modification of an otherwise successful offer, that makes its terms more favorable to the Government, will be considered at any time it is received and may be accepted.
(3) Acceptable evidence to establish the time of receipt at the Government installation includes the time/date stamp of that installation on the offer wrapper, other documentary evidence of receipt maintained by the installation, or oral testimony or statements of Government personnel.
(4) If an emergency or unanticipated event interrupts normal Government processes so that offers cannot be received at the Government office designated for receipt of offers by the exact time specified in the solicitation, and urgent Government requirements preclude amendment of the solicitation or other notice of an extension of the closing date, the time specified for receipt of offers will be deemed to be extended to the same time of day specified in the solicitation on the first work day on which normal Government processes resume.
(5) Offers may be withdrawn by written notice received at any time before the exact time set for receipt of offers.
Oral offers in response to oral solicitations may be withdrawn orally. If the solicitation authorizes facsimile offers, offers may be withdrawn via facsimile received at any time before the exact time set for receipt of offers, subject to the conditions specified in the solicitation concerning facsimile offers. An offer may be withdrawn in person by an offeror or its authorized representative if, before the exact time set for receipt of offers, the identity of the person requesting withdrawal is established and the person signs a receipt for the offer.
(g) Contract award (not applicable to Invitation for Bids). The Government intends to evaluate offers and award a contract without discussions with offerors. Therefore, the offeror's initial offer should contain the offeror's best terms from a price and technical standpoint. However, the Government reserves the right to conduct discussions if later determined by the Contracting Officer to be necessary. The Government may reject any or all offers if such action is in the public interest; accept other than the lowest offer; and waive informalities and minor irregularities in offers received.
(h) Multiple awards. The Government may accept any item or group of items of an offer, unless the offeror qualifies the offer by specific limitations. Unless otherwise provided in the Schedule, offers may not be submitted for quantities less than those specified. The Government reserves the right to make an award on any item for a quantity less than the quantity offered, at the unit prices offered, unless the offeror specifies otherwise in the offer.
(i) Availability of requirements documents cited in the solicitation. (1)(i) The GSA Index of Federal Specifications, Standards and Commercial Item Descriptions, FPMR Part 101-29, and copies of specifications, standards, and commercial item descriptions cited in this solicitation may be obtained for a fee by submitting a request to--GSA Federal Supply Service Specifications Section, Suite 8100, 470 East L'Enfant Plaza, SW, Washington, DC 20407, Telephone (202) 619-8925, Facsimile (202) 619-8978.

(j) Unique entity identifier. (Applies to all offers exceeding $3,500, and offers of $3,500 or less if the solicitation requires the Contractor to be registered in the System for Award Management (SAM) database.) The Offeror shall enter, in the block with its name and address on the cover page of its offer, the annotation ``Unique Entity Identifier'' followed by the unique entity identifier that identifies the Offeror's name and address. The Offeror also shall enter its Electronic Funds Transfer (EFT) indicator, if applicable. The EFT indicator is a four-character suffix to the unique entity identifier. The suffix is assigned at the discretion of the Offeror to establish additional SAM records for identifying alternative EFT accounts (see subpart 32.11) for the same entity. If the Offeror does not have a unique entity identifier, it should contact the entity designated at www.sam.gov for unique entity identifier establishment directly to obtain one. The Offeror should indicate that it is an offeror for a Government contract when contacting the entity designated at www.sam.gov for establishing the unique entity identifier.
(k) System for Award Management. Unless exempted by an addendum to this solicitation, by submission of an offer, the offeror acknowledges the requirement that a prospective awardee shall be registered in the SAM database prior to award, during performance and through final payment of any contract resulting from this https://assist.dla.mil/online/start/ http://quicksearch.dla.mil/ http://assistdocs.com/ https://assist.dla.mil/wizard/index.cfm http://www.sam.gov/ solicitation. The technical offer shall be clear, concise, and include all stated elements to ensure effective evaluation and for substantiating the validity of stated claims in the offeror’s offer. The offer should not simply rephrase or restate the government’s requirements, but rather shall provide convincing rationale to address how the offeror intends to meet these requirements. Statements that the offeror understands, can, or will comply with the PWS (including reference publications, technical data, etc.); statements paraphrasing the PWS or parts thereof (including applicable publications, technical data, etc.); and phrases such as “standard procedures will be employed”; or “well known techniques will be used”, etc. will be considered unacceptable. Offerors shall assume that the Government has no prior knowledge of their facilities and experience, and will base its evaluation on the information presented in the offeror’s offer. Elaborate brochures or documentation, binding, detailed art work, or other embellishments are unnecessary and are not desired.
The Technical Volume shall, at a minimum, be prepared in a form consistent with the PWS and the evaluation criteria for award set forth in the “Evaluation Factors for Award” section of this solicitation. Also identify if the warehouse(s) space is owned or rented, if it is available for the duration of the contract and how it is secured. Offerors must also provide a physical address of the warehouse(s) intended to be utilized for contract performance. NOTE: Warehouses will be inspected by the government for purpose of awarding the contract. The Governments’ inspection of warehouse storage facilities will be conducted in accordance with the attachments, Appendix D – Pre-award Survey Guidelines, Appendix E – Warehouse Inspection Guide, and Attachment E1 – Fire Prevention and Warehouse Standards.

3. Prime Contractor's Prior Technical Experience Subfactor
The offeror shall provide evidence of prime contractor’s minimum two (2) contracts of experience in packing and moving household goods within the last five (5) years from the issuance date of this solicitation. The experience of a subcontractor cannot substitute for the prime contractor’s experience. The offeror shall provide evidence of prior experience in the following format:

b. Description of work completed
c. Length of contract/project (year/month/date – year/month/date)

Evaluation Factors for Award in provision 52.212-2, Evaluation—Commercial Items
52.212-2 EVALUATION--COMMERCIAL ITEMS (OCT 2014)
(a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers:
I. Price II. Technical Capability
1. Contractor’s Qualifications
2. Contractor’s Quality Control Plan
3. Prime Contractor’s Technical Experience
(b) Options. The Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. The Government may determine that an offer is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the Government to exercise the option(s).
(c) A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer's specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award.
(End of provision)
Addendum to 52.212-2 – Evaluation – Commercial Items
BASIS FOR CONTRACT AWARD: Government evaluation will be conducted on offerors’ offers considering the evaluation factors listed herein. Evaluation of the factors will utilize a Lowest Price Technically Acceptable (LPTA) source selection process in accordance with FAR Part 13.106-2; whereby the Government seeks to award on the basis of the lowest evaluated price of offers meeting or exceeding the acceptability standards of non-price factors (i.e. Technical Capability). Under the LPTA source selection process, trade-offs are not permitted, offers are evaluated for acceptability but not ranked using the non-price factors.
The Government intends to award a single Requirements Type Contract as a result of this solicitation. Failure to include a price for all items in the solicitation will result in the offer not being considered for award.
The government intends to award a contract without formal discussions. Upon completion of the Government’s initial evaluation of offers, interchanges changes with one, two or more offerors may occur.
The evaluation process shall proceed as follows:
EVALUATION FACTORS FOR AWARD: Two (2) evaluation factors will be used to evaluate offers: a.) Price, and b.) Technical Capability. To be considered technically acceptable, a offer must be rated acceptable in all factors/subfactors.
All subfactors are determined to be critical. An unacceptable rating in any subfactor will automatically carry forward to the factor rating.
I. Price Evaluation. The Government will evaluate prices that are submitted in the Price Schedule of the completed SF 1449 Section B. Attachment A – Price Worksheet should be completed and submitted as a substantiation of the costs proposed in Section B. Failure to propose on all CLINs will render the offer non-responsive. The offers will be ranked by Total Evaluated Price TEP. An offeror’s TEP will be determined by multiplying the quantities identified in Attachment A by the proposed unit price for each ITEM (X001-X033).
The TEP will be calculated as the total sum of all FFP CLINs for the base period, all option periods, and six-month extension.
The contract will include FAR Clause 52.217-8, Option to Extend Services, with the potential to extend the contract an additional 6 months beyond the awarded contract performance period. In the event the Government requires continued performance, the Government's evaluation of prices proposed for the base and all option periods will suffice to be the Government's evaluation of prices under the extension of services (ref. 52.217-8).
The price for the six-month extension will be calculated using the unit prices of the last option period, as extended to cover a six months period and will be added to the total arrived at above. Evaluation of the option period will not obligate the Government to exercise the option.
The total of the annual extended prices for each CLIN and the price for the six-month extension authorized by 52.217-8, Option to Extend Services will be added together to arrive at the TEP.
Explained: (Total Sum of CLINS for base year and all options) + (6 additional months at final option price) = TEP
Arithmetic Discrepancies. For the purpose of initial evaluation of offers, the following will be utilized in resolving arithmetic discrepancies found on the face of the pricing schedule as submitted by the offeror.
(A) Obviously misplaced decimal points will be corrected;
(B) Discrepancy between unit price and extended price, the unit price will govern;
(C) Apparent errors in extension of unit prices will be corrected;
(D) Apparent errors in extended prices per CLIN will be corrected.
II. Technical Capability Evaluation. The government will evaluate technical offers and the below listed sub-factors on a pass/fail-basis, assigning ratings of acceptable, or unacceptable. An unacceptable rating for any of the below sub-factors will result in an unacceptable rating for this Technical Capability primary factor.
Accordingly, under the LPTA source selection process, offerors who receive an unacceptable rating for this factor may not be considered for award. Technical Acceptability shall be evaluated against the following sub-factors in the following manner:
1. Contractor’s Qualifications:
The offeror shall submit all the documents requested in paragraph 1. Contractor’s Qualifications of the Addendum to 52.212-1, paragraphs 1.a. through c.
Offeror’s qualifications have to demonstrate the offeror’s knowledge, expertise, and in depth experience of the tasks identified in PWS and how the offeror intends to leverage this knowledge, expertise and experience to provide the best possible service to the Government. In addition, offeror shall demonstrate how facilities meet Defense Transportation Regulation DoD4500.9R, PWS and evaluation criteria appendix D and E and PWS, Facilities as well as how offeror meets Cargo Insurance as described in PWS.
The offeror‘s warehouses will be evaluated to assure all facilities utilized for storage are acceptable based upon the government‘s inspection described in Addendum to FAR 52.212-1 and in accordance with inspection guides and checklists as contained in Appendix D – Pre-award Survey Guidelines, Appendix E – Warehouse Inspection Guide, and Attachment E1 – Fire Prevention and Warehouse Standards.
2. Contractor’s Quality Control Plan:
The Quality Control Plan will be evaluated for its completeness in thoroughly addressing the offeror’s proposed surveillance methods which ensures delivery of the required level of quality service as identified in paragraph 2. Contractor’s Quality Control Plan of the addendum to FAR 52.212-1.
3. Prime Contractor’s Technical Experience:
In evaluation of this subfactor, the Government will confirm that the offeror did submit an Individual Reference for 2 contracts within the last 5 years through the issue date of subject solicitation, demonstrating that the offeror provided services relevant to those required in the PWS. To be found acceptable the offeror must demonstrate that the offeror provided services relevant to those required in the PWS. Relevant performance includes effort involving similar scope or greater in scope, magnitude and complexity than the effort described in this solicitation.
The Technical Capability factor will be assigned one of the ratings defined within Table A-1 below. The subfactor ratings will be used to determine an overall rating for the factor.
Table A-1. Technical Acceptable/Unacceptable Ratings Rating Description Acceptable Offer meets the requirements of the solicitation.
Unacceptable Offer does not meet the requirements of the solicitation.
AWARD DETERMINATION: All offers will be checked to ensure compliance with the solicitation requirements and determine responsiveness of the offer. If a offer is deemed non-responsive it may be excluded from further consideration for award at this time. If a offer is deemed responsive it will then be included in ranking of offers by price.
For the purpose of conducting evaluations, the Government will first rank all proposals by Total Evaluated Price (TEP). TEP is the sum total of all FFP Contract Line Item Numbers (CLINs), as well as the 6 month option to extend services which will be calculated as half of the total cost of the last option period. Price evaluation IAW FAR 13.106-3(a) and Technical evaluation will then be conducted on the two lowest priced responsible proposals. If the lower priced responsible proposal of the two is evaluated as technically acceptable, that proposal represents the best value for the government and the evaluation process stops at this point.
If the lowest priced offer is rated unacceptable, unreasonable or unbalanced the Government will then evaluate the proposals of the next two lower responsible proposals using the same process as outlined in the paragraph. This process will repeat until a proposal is evaluated as technically acceptable and has both reasonable and balanced pricing and then proceed with the evaluation the Technical Proposal. The two lowest priced proposals will be technically evaluated. In the case of technical acceptability the evaluation process will stop and selection for award will be made to the lowest priced of the two acceptable proposals.
