SECTION SF 1449 - CONTINUATION SHEET

The following have been modified:

ADDENDUM TO 52.212-1

Failure to meet a requirement may result in a proposal rated non-responsive or unacceptable and therefore ineligible for award.

Complete blocks 12, 17a, and 30a, b, and c of the Standard Format SF1449. In doing so, the Offeror accedes to the contract terms and conditions as written in the solicitation, with attachments. The solicitation constitutes the model contract.

Acknowledged Solicitation Amendments. Provide all executed Standard Forms 30, or other appropriate amendment acknowledgements (if applicable).

Proposals must demonstrate a comprehensive understanding of the nature and scope of work required. The Government will not search for data to cure problems or address inconsistencies in proposals. The Government may consider any failure to comply with these instructions to be indicative of what could be expected from an Offeror during performance and may consider it a deficiency of the proposal.

Statements such as "will comply," "noted and understood," "IAW industry best practices/standards," etc. without supporting narrative are not acceptable. Cursory responses or responses that merely reiterate or paraphrase the Performance Work Statement (PWS) language will not be considered to satisfy the requirements of the Request for Proposal (RFP). Each Offeror shall submit a complete proposal in accordance with (IAW) these instructions.

The Government’s terms, conditions, and respective clauses contained within this solicitation are prescribed in accordance with United States Federal Acquisition Regulations (FAR), and are not subject to conditionally proposed revisions or changes requested by offerors. Offerors’ specific terms and conditions that may be contingently proposed for subsequent inclusion into the contract if awarded, will not be considered by the Government in its evaluation of proposals. Offerors that are not able to comply with the aforementioned conditions, will be determined as non-responsive to the solicitation requirements, and therefore will be removed from consideration for award.

Submit signed and dated offers to this office prior to the date and time specified in this RFP. Offers shall be submitted on the Standard Form SF1449.

1. PROPOSAL PREPARATION INSTRUCTIONS

To assure timely and equitable evaluation of proposals, offerors must follow the instructions contained herein. This section specifies the format and content that offerors shall use in this Request for Proposals (RFP). The intent is not to restrict the offerors in the manner in which they will perform their work but rather to ensure a certain degree of uniformity in the format of the responses for evaluation purposes. Offerors must submit a proposal that is legible and comprehensive enough to provide the basis for a sound evaluation by the Government. Information provided should be precise, factual and complete. Legibility, clarity, completeness, and responsiveness are of the utmost importance.

Proposals shall contain a response to each of the areas identified below. Any proposal which does not provide as a minimum, that which is required in this solicitation may be determined to be substantially incomplete/non-compliant and not warrant any further consideration at the Government’s discretion. Proposals must be completed and self-sufficient.

In addition to the requirements outlined above required to determine responsiveness of prospective offerors, the proposal shall consist of two separate parts.

Factor I – Price Proposal Factor II - Technical Capability

The Contracting Officer has determined there is a high probability of adequate price competition in this acquisition. Upon examination of the initial offers, the Contracting Officer will review this determination and if, in the contracting officer’s opinion, adequate price competition exists no additional price information will be requested. However, if at any time during this competition the Contracting Officer determines that adequate price competition no longer exists; offerors may be required to submit information to the extent necessary for the Contracting Officer to determine the reasonableness and affordability of the price.

Complete the necessary fill-ins and certifications in provisions and in the on-line Representations and Certifications at www.sam.gov. Return the provision FAR 52.212-3, Offeror Representations and Certifications – Commercial Items along with the proposal. For other provisions and clauses in the solicitation, the offeror is required to submit the pages that require an offeror fill-in.

Specific Instructions: Complete the necessary fill-ins and certifications in all provisions. The provisions FAR 52.212-3 (alt I) shall be returned along with the proposal.

Italy Sealed envelope or package used to submit your proposal must show the time and date specified for receipt, Solicitation Number, and the name and address of the offeror.

Electronic copies must be submitted on CD or DVD. Flash drives will not be accepted.

Only one electronic copy is required.

Note: Offerors are cautioned that Aviano AFB has visitor control procedures requiring individuals not affiliated with the installation to obtain a visitor pass prior to entrance. SOME DELAY SHOULD BE ANTICIPATED IF HAND CARRYING PROPOSALS. Offerors should allow sufficient time to obtain a visitor pass and arrive at the contracting office PRIOR to the time specified for receipt.

The submittal due date for proposals is 20 March 2017 no later than 16.00 hours Central European Time.

Late proposals will be processed in accordance with FAR 52.212-1(f) ―Late submission, modifications, revisions, and withdrawals of offers.

Further changes to the original clause 52.212-1.

0. Paragraph 52.212-1(c), Period for Acceptance of Offers is tailored as follows: The offeror agrees to hold the prices in its offer firm for 60 calendar days from the date specified for receipt of offers.

0. Paragraphs 52.212-1(d) and 52.212-1(h) and 52.212-1(i) do not apply to this acquisition.

FACTOR I - PRICE PROPOSAL

Provide a firm fixed price (FFP) proposal in accordance with the Contract Line Item Numbers (CLINs), by completing Schedule of Supplies/Services in Section B of the Standard Form (SF) 1449 Continuation Sheet.

All proposed prices shall be submitted in Euro (€). Insert proposed unit and extended prices in the Schedule of Supplies/Services for a Base Year and four (4) option years (SF 1449).

FACTOR II – TECHNICAL CAPABILITY

General: Information the offeror deems appropriate to address the technical criteria should be included in the proposal in an orderly format and be consistent with the evaluation criteria for award set forth in FAR Clause 52.212-2 including Addendum to FAR Clause 52.212-2 of this solicitation. Proposal paragraphs shall correspond to the below evaluation factors and pertinent Performance Work Statement (PWS) paragraph (where referenced). To facilitate evaluation, the information contained in Factor II shall be specific, detailed and clearly demonstrate that the offeror has a thorough understanding the requirement for the accomplishment of this effort.

Technical proposals must demonstrate the offeror’s ability to successfully meet the needs of the Government. All technical information shall be in English language. Documents in other than this language shall have an English translation attached. The offeror shall, at a minimum, address each item listed below individually.

Technical capability evaluator factors are following:

1. Six (6) yrs experience as Project Manager (PM) in Healthcare environment;

1. Fluent command of English and Italian - minimum English level equivalent to a C1 " EFFECTIVE OPERATIONAL PROFICIENCY OR ADVANCED" level of the Common European framework of reference for languages; and

1. Education level as minimum of "Laurea triennale".

ADDENDUM TO 52.212-2

BASIS FOR CONTRACT AWARD: This is a competitive Lowest Price Technically Acceptable (LPTA) source selection conducted in accordance with Federal Acquisition Regulation (FAR) Parts 12 and 15, Department of Defense (DoD) FAR Supplement Procedures, Guidance and Information Subpart 215.3, and Air Force FAR Supplement (AFFARS) Mandatory Procedure (MP) 5315.3. By submission of its offer in accordance with the instructions provided in clause FAR 52.212-1, Instructions to Offerors – Commercial Items, the offeror accedes to the terms of this model contract. Failure to meet a requirement may result in an offer being determined unacceptable. Award will be made to the lowest Total Evaluated Price (TEP) offer that meets the acceptability standards for non-price factors. The evaluation process will include the following:

Technical Acceptability Factor. The government will evaluate technical proposals on a pass/fail basis and assign an Acceptable or Unacceptable rating as described in Table 1 below.

The proposals will be evaluated against the following factor(s):

Factor 1 – Price

Price Evaluation Factor. The government will rank all offers by Total Evaluated Price TEP. The price evaluation will document the fairness and reasonableness of the TEP.

Total Evaluated Price: A TEP will be computed for each offer in order to meet the requirements of FAR Parts 6 and 17. The price of a six-month extension authorized by 52.217-8, Option to Extend Services, will be added to the proposed price for each potential contract performance period.

First, the offeror’s total proposed price will be determined by multiplying the quantities identified in the Pricing Schedule (SF 1449, continuation of blocks 19 – 24) by the unit price, limited to two decimal places, for each Contract Line Item Number (CLIN) to confirm the extended total amount for each CLIN; the sum of all extended amounts will be the offeror’s total proposed price. Second, the total proposed price (total amount for CLINs 0001 – 4001) plus the sum of Option to Extend Services clause 52.217-8 (six months at last option period) will constitute the TEP.

(When a CLIN is an annual price, use the 0.5 multiplicative factor to derive the six months performance value.)

Evaluating the Option CLINs does not obligate the government to exercise the Options.

Offerors are cautioned to submit sufficient information and in the format specified in 52.212-1, Instructions to Offerors – Commercial Items, of this solicitation. Offerors may be asked to clarify certain aspects of their proposals. Exchanges conducted to resolve minor or clerical errors will not constitute discussions and the contracting officer reserves the right to award a contract without the opportunity for proposal revision. The government intends to award a contract without discussions with respective offerors. The government, however, reserves the right to conduct discussions if deemed in its best interest.

Award will be made to the lowest priced offer, based on TEP, that meets the acceptability standards for non-price factors.”

Factor 2 – Technical capability

The government will evaluate information on all offerors based on the references provided by the offeror, and any data independently obtained by the government.

1. Six (6) yrs experience as Project Manager (PM) in Healthcare environment

Experience regarding predecessor companies of the offeror and/or subcontractors, key personnel, and subcontractors that will perform major aspects of the requirement.

Furnish the following information:

a. Company name

b. Point of contact (POC), name and phone number

2. Fluent command of English and Italian - minimum English level equivalent to a C1 " EFFECTIVE OPERATIONAL PROFICIENCY OR ADVANCED" level of the Common European framework of reference for languages.

a. The Offeror shall provide English language certificate

3. Education level as minimum of "Laurea triennale"

a. The Offeror shall provide College diploma.
