FA5270.53.2.215-9Z01 PROPOSAL PREPARATION INSTRUCTIONS
A. To assure timely and equitable evaluation of proposals, offerors must follow the instructions contained herein. Offerors are required to meet all solicitation requirements, including terms and conditions, representations and certifications, and technical requirements, in addition to those identified as evaluation factors or subfactors. Offerors must clearly identify any exception to the solicitation terms and conditions and provide complete accompanying rationale. The response shall consist of three (3) separate parts; -Summary and Miscellaneous Data, - Price Proposal, and -Past Performance Proposal.Part I Part II Part III
B. PROPOSAL DELIVERY INSTRUCTIONS
(i) Offerors must follow the instructions contained herein. Offerors shall submit only one proposal and the Government will review only one proposal per offeror.
Proposals must be received by 18 CONS/PKC no later than the date and time specified in Block 13 of the solicitation. Proposals delivered after the exact date and time for receipt of proposals will not be considered.
(ii) Electronic bidding will be utilized for this acquisition. Prospective offerors SHALL be submitted to SrA Rubino, Emily at emily.rubino@us.af.mil and Rosita Crawford at Rosita.crawford@us.af.mil. Only files created using Microsoft WORD, Microsoft EXCEL, and/or Adobe .pdf files will be accepted. No zip files will be accepted. Files are not to exceed 3 MB. For larger files, use DoD SAFE site to send submittals. By submitting an electronic copy, offers assume all risk of quotation not getting through security measures and firewalls.

(iv) All files must be in Microsoft Office format and must be able to be opened on Government computers. Documents provided by the Government in PDF format are exempt from this requirement and may be submitted in PDF format. Offerors are highly encouraged to submit proposals in time to verify that the Government received the files and that the files can be opened and read. The Government is not responsible if files submitted are not readable or are in the wrong format.
Replacement files will not be accepted after the deadline for submittal of proposals. All spreadsheets must be unlocked, and all formulas must be included and readable by the Government.
All proposal documents must be uploaded, submitted, and received by the proposal deadline in order to be considered timely for purposes of responsiveness and evaluation.
Upon submission of all proposal documents, a courtesy email may be sent to the Points of Contact SrA Emily Rubino at and Rosita emily.rubino@us,af,mil Crawford at . The email shall not include any proposal documents; all documents must be submitted per the instructions contained in rosita.crawford@us.af.mil these instructions. Sending of a courtesy email does not constitute submission of a proposal. A courtesy email is only requested to inform the Points of Contact that a proposal has been submitted.
C. Specific Instructions:
Documents submitted in response to this RFP must be consistent with the following:
(i) Requirements of the solicitation (Contract Line Item Numbers (CLINs) and Statement of Work (SOW)) and Government standards and regulations pertaining to the SOW.
(ii) Evaluation Factors for Award in Section M of this solicitation.
(iii) Each Part shall contain a Table of Contents, Cross References, and the List of Tables and Drawings (if applicable).
(iv) Each Part shall be complete in itself and shall not reference information contained in other Parts. Page 1 of a Part is defined as the first page after the Table of Contents. All pages shall be numbered.
(v) Price data of any kind shall be presented only in the Price Proposal Part.
(vi) Elaborate formats, bindings, and color presentations are not desired or required.
(vii) Proposals shall be in the English language and all monies in YEN.
(viii) Parts (i.e., files) shall be separated and easily identified (ex. Parts I, II, & III).
(ix) Only the SF 1442, Past Performance Fact Sheet, Past Performance Questionnaires (PPQs) received from the applicable Government Points of Contact (POCs), and bonding information will be considered part of the offeror's proposal. Additional proposal documentation will not be considered.
(x) Each page containing proprietary information should be marked:

1. PART I - SUMMARY ANDMISCELLANEOUS DATA. Submit original copy.
The offeror shall submit the SF 1442 with all required fill-ins as directed in this section. An authorized official of the firm must sign Block 20b of the SF 1442.
Complete pricing information on all CLINs. Complete all appropriate and applicable Representations and Certifications. In doing so, the offeror accedes to the contract terms and conditions as written in the solicitation Sections A through K. These sections constitute the model contract.
(a) The offeror shall provide a concise summary and master table of contents of the entire proposal.
(b) Complete blocks 14, 15, 16, 17,18, 20a, 20b, 20c of the RFP Section A, SF1442.
(c) Complete block 19 of the RFP Section A, SF 33, if any solicitation amendment(s) to the original solicitation documents was/were issued.
(d) Complete the necessary fill-ins and certifications in Sections I through K. Section K shall be returned in its entirety. For Sections C through I, the offeror shall submit only those pages that require a fill-in.
(e) Financial Statement Requirement: Offeror must provide the most recent 3 years of Company Annual Closing Balance Sheet or Taishaku-taisho-hyo. These should be certified by an independent public accountant or by an appropriate officer of the firm. Failure to submit the necessary documents may result in the Contractor being deemed on-responsible in accordance with FAR 9.104-1, and subsequently removed from consideration for award.
(f) The offeror shall submit a bid bond with this proposal as referenced on SF1442, Block 13b (IAW FAR provision 52.228-1).
2. PART II - PRICE PROPOSAL - Submit original copy.
(a) This part shall contain a completed Section B (Supplies or Services and Prices/Costs).
(b) Insert proposed unit prices and extended amounts in Section B for each Contract Line Item Number (CLIN) or Sub CLIN (SCLIN). The extended amount must equal the unit price multiplied by the number of units. It is imperative that mathematical calculations are correct; particular care should be taken to ensure the amounts match when using a spreadsheet program, such as Microsoft EXCEL. The unit prices and extended amounts are limited to two decimal places.
(c) Extreme care should be exercised to ensure that no price data of any kind is included in any other Part of the proposal.
3. PART III - PAST PERFORMANCE PROPOSAL - Only references for past efforts/contracts of relevance are desired. Submit original copies.
a) Past Performance Fact Sheet (PPFS): The offeror shall submit a minimum of three (3) Past Performance Fact Sheets (Section J, Attachment 9). Information on each Past Performance Fact Sheet shall closely relate to the scope, size, and complexity of the proposed work under this solicitation. To demonstrate relevancy, the offeror shall compare the past performance work completed with the work description in the Statement of Work. To be considered relevant, past performance records must demonstrate successful execution of projects as cited in Section M, paragraph A.
In addition, explain corrective actions taken in the past, if any, for substandard performance and any current performance problems, such as cost overruns, extended performance periods, numerous warranty calls, etc.
Furnish the following information for each contract listed:
(1) Company/Division name
(2) Product/Service
(3) Contracting Agency/Customer
(4) Contract Number
(5) Contract Dollar Value
(6) Period of Performance
(7) Verified, up-to-date name, mailing and e-mail addresses, and telephone number of the contracting officer (Point-of-Contact)
(8) Comments regarding compliance with contract terms and conditions
(9) Comments regarding any known performance deemed unacceptable to the customer, or not in accordance with the contract terms and conditions.
(10) Include rationale supporting your assertion of relevance and identify aspects (scope, magnitude of effort, and complexity) of the contracts deemed relevant and how they relate to the proposed effort.
Offerors shall provide the most current information for the POCs identified on the Past Performance Fact Sheets. At least two of the following (in descending order of availability) should be identified as current POCs:
(1) Procuring Contracting Officer/Contract Negotiator or equivalent
(2) Program/Project Manager, or equivalent
(3) Administrative Contracting Officer/Contract Administrator or equivalent
(4) Other (As specified in your Past Performance Fact Sheet)
The Past Performance Fact Sheet information is intended to assist in determining relevancy of a given project and will not, by itself, be evaluated for performance.
Any past performance submitted by a subcontractor on behalf of the prime contractor must include a copy of a formal subcontractor agreement specific to the past performance project.
In lieu of a Past/Performance Questionnaire, if a project is currently available in the Contractor Performance Assessment Reporting System (CPARs), the offeror must attach the evaluation to the questionnaire with the solicitation number and submit it directly to the attention of SrA Emily Rubino via email to emily.rubino@us.
af.mil, as well as to the 18 CONS/PKC Solicitation Management Organization Mailbox at18cons.pkc.solicitationmgmt@us.af.mil
If an offeror has no past performance history of relevance, the offeror must state affirmatively it possesses no past performance history of relevance.
(b) Performance Questionnaires: Past Performance Questionnaires (PPQs) shall be used by the offeror in obtaining and providing past performance information to validate their Past Performance Fact Sheet. Each Past Performance Fact Sheet should have a corresponding PPQ. Offerors shall exert their best effort to ensure a
. The Government requires the offeror send out a PPQ (Attachment 10) to each of the POCs PPQ is returned for each Past Performance Fact Sheet submitted identified in the Past Performance Proposal.
Once the questionnaires are completed by the POCs, the information contained therein will be considered source selection sensitive and will not be released outside the Government. The POCs shall forward their completed questionnaires directly to the Government- NOT BACK TO THE OFFEROR. A completed PPQ must be submitted directly to the Government not later than the date and time established in the solicitation for receipt of proposals. COMPLETED PPQs

THE RESPONSIBILITY TO SEND OUT THE PAST PERFORMANCE QUESTIONNAIRE RESTS SOLELY WITH THE OFFEROR.
Past Performance evaluation is not limited to review of the information provided in Part III or PPQs received. The Government reserves the right to review all available past performance information in the integrated assessment system.
(c) If a teaming arrangement of any is contemplated, provide complete information as to the arrangement, including any relevant and recent past performance information on previous teaming arrangements with same partner. If this is a first-time joint effort, each party to the arrangement must provide a list of past contracts of relevance. The Government will evaluate not more than three (3) past performances for each party of a first time joint effort.
