1. DESCRIPTION OF TRAINING / INSTALLATION.

The contractor must provide all personnel, equipment, tools, materials, supervision, transportation, safety equipment, any other items and services necessary to perform installation of the complete Integrated Force Plate System as defined in this Statement of Work (SOW) in a Tactical Human Rapid Rehabilitation and Reconditioning (THOR3) Facility on Torii Station, Okinawa, Japan.

1.1. CONTRACTOR RESPONSIBILITIES

1.1.1. Basic Services: The contractor shall provide all labor, material and equipment to accomplish the scope of work.

1.1.2. Liability for damages – The contractor will be held responsible for any damages that occur by the contractor and/or any personnel that work under this contract to Government facilities, Government owned property and Government purchase furniture.

1.1.3. Contractor is responsible for delivery to Tracy, California where the US Government will forward the INTEGRATED FORCE PLATE SYSTEM to Okinawa, Japan. After the INTEGRATED FORCE PLATE SYSTEM’s arrival to Okinawa via US Government means, the contractor shall be responsible for the unpacking of equipment from its shipping crating and installation of INTEGRATED FORCE PLATE SYSTEM in the designated area, and removal/disposal of rubbish at a coordinated date IAW para 1.1.4.

1.1.4. Contractor will coordinated schedule with 1-1 SFG (A) installation and beneficial training date. Government will provide storage for Integrated Force Plate System should beneficial training date change.

1.2. CLEANING AND PROTECTION

1.2.1. Maintain Walk Through – The contractor must maintain common walk through areas to allow proper evacuation of the building in the case of emergencies.

1.2.2. Remove Rubbish – The contractor must remove rubbish, wrapping paper, salvages and scrap. The contractor must not leave waste material inside or outside of the facility overnight, or for a prolonged period of time, when work is not done. Remove and dispose of waste materials only at authorized off base locations, daily.

1.3. TRAINING

1.3.1. The contractor is responsible for providing the following on-site training for up to six 1-1 SFG (A) personnel over the course of two days at the THOR3 Facility on Torii Station, Okinawa, Japan once the system is available for use:

1.3.2. Setup and Storage Procedures: Demonstrate proper setup, calibration, tear down, sanitation and long-term storage procedures. Fully test all equipment and hardware for normal operation.

1.3.2.1. End-user Training – Fully demonstrate how to correctly operate the product and correct utilization of included software and hardware to perform accurate force plate measurements. Project leader should provide interactive sessions in order to train staff on software and testing protocols with actual live subjects. Protocols include jumping and landing movements, as well as loaded and unload squats, and isometric mid-thigh pull exercises. By the end of training THOR3 staff should be able to properly use the integrated video system to look for asymmetries and “high risk” movement patterns, so that clinicians can adapt training or rehabilitation programs to ensure Soldiers can return to duty.

1.3.2.2. Maintenance Procedures - Fully demonstrate all daily, monthly, and annual maintenance requirements to ensure usability of the product.

1.4. SALIENT CHARACTERISTICS

1.4.1. System software for the force plate measurement system that integrates 3D Force Plate measurement system with synchronized high-speed HD color video and displays user-selected variables and statistical comparisons in real-time.

1.4.2. A complete solution for integrating with Olympic lifting stations or jumping stations— complete with a surrounding floor panels to provide a large, safe area for testing and training.

1.4.3. A portable solution for 3D force measurement that provides several features for educational, tactical, and sport assessment purposes: A large landing surface for a portable plate; 2000 vertical load capacity to handle large impacts; Hall-Effect sensors to handle large loads and minimize drift; and very easy to setup.

1.4.4. Methods in analysis that are extremely reliable and valid and have demonstrated data integrity through numerous independent, peer reviewed articles allowing us to know we are capturing accurate reliable data.

1.4.5. System software must provide diversity in test collection and analysis options to include.

Standard Jump, Landing and Olympic Weightlifting tests with full analysis, including:

Repeated Jump Tests, counter-movement and squat jumps, many single leg hops and jumps, drop jump and landings, isometric lift tests, and dynamic weight lifting exercises

1.4.6. Force plate system and software must include fully integrated on board video capture and replay capable of providing data streams fully integrated and synchronized with up to 5 HD Color video at frames rates to 150+ fps. Must provide a real force vector overlay, and user customized information to be displayed on instant replay. Must also be available to the user for post processing, analysis and sharing.

1.4.7. Must have a video export and reporting feature which allows users to export customized mp4 videos for sharing or PDF reports complete with screenshots and user-generated comments.

1.4.8. Data files must be able to be stored in Excel-friendly ascii format, allowing users to export data to third-party software for statistical analysis. The system must allow access to the raw data.

1.4.9. The system must allow for real time display capability for use with teaching, coaching, and live feedback.

2. GENERAL INFORMATION.

2.1. SECURITY REQUIREMENTS.

The contractor employees will be required to obtain and display identification badges accordingly. Anticipate delays in getting commercial vehicles on base and allow time for commercial vehicles to reach their destination by driving designated routes at posted speed limits throughout the base. Procedures for commercial vehicle access to the base are subject to change without prior notice.

2.1.1. Access. Daily, in order to access Torii Station, contractor personnel must have government issued identification, those not native to Japan must have their passport. 1-1 SFG (A) will provide escorts to contractor for access to Torii Station, Okinawa, Japan.

2.2. HOURS OF OPERATION.

2.2.1. Normal Hours of Operation. The contractor shall perform the services required under this contract during between the hours of 0800 to 1700, Monday to Friday. The contractor may work, with prior approval of the Contracting Officer, extended hours to ensure timely completion of work at no additional cost to the government.

2.2.1.1.Recognized Holidays. The contractor is not required to provide service on the following days: New Year’s Day, Martin Luther King Day, Presidents Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Veterans Days, Thanksgiving Day and Christmas Day. If the holiday falls on Saturday, it is observed on Friday. If the holiday falls on a Sunday, it is observed on Monday.

2.2.2. Emergency Services. On occasion, services may be required to support an activation or exercise of contingency plans outside the normal duty hours described above.

2.3. CONSERVATION OF UTILITIES.

The contractor shall instruct employees in utilities conservation practices. The contractor shall be responsible for operating under conditions which prevent the waste of utilities which include the following:

2.3.1. Lights shall be used only in areas where and when work is actually being performed.

2.3.2. Mechanical equipment controls for heating, ventilation, and air conditioning systems shall not be adjusted by the contractor or by contractor employees unless authorized.

2.3.3. Water faucets or valves shall be turned off after the required use has been accomplished.

2.3.4. Government telephones shall be used only for official government business.

2.4. SAFETY REQUIREMENTS

In performing work under this contract, the contractor shall:

2.4.1. Conform to the safety requirements contained in the contract for all activities related to the accomplishment of the work.

2.4.2. Perform work in a safe manner as required by OSHA 2206, General Industry, Occupational Safety and Health Standards (29 CFR 1910).

2.4.3. Develop and provide at the start of the orientation period or the start of the first operational performance period (if there is no orientation period) a safety plan for the protection of government facilities and property and to provide a safe work environment for contractor personnel.

2.4.4. Provide protection to government property to prevent damage during the period of time the property is under the control or in possession of the contractor.

2.4.5. Include a clause in all subcontracts to require subcontractors to comply with the safety provisions of this contract as applicable.

2.4.6. Record and report promptly (within one hour) to the contracting officer or designated government representative (GR), all available facts relating to each instance of damage to government property or injury to either contractor or government personnel.

2.4.7. In the event of an accident/mishap, take reasonable and prudent action to establish control of the accident/mishap scene, prevent further damage to persons or property, and preserve evidence until released by the accident/mishap investigative authority through the contracting officer.

2.4.8. If the government elects to conduct an investigation of the accident/mishap, the contractor shall cooperate fully and assist government personnel in the conduct of investigation until the investigation is completed.

2.4.9. Include a clause in each applicable subcontract requiring the subcontractor's cooperation and assistance in accident reporting and investigation.

2.5. CONTINUATION OF ESSENTIAL DOD CONTRACTOR SERVICES DURING

CRISIS IAW DoDI 3020.37. (MANDATORY)

IAW AFI 63-124, para 2.7.1, and DODI 3020.37 this requirement is not considered mission essential.

2.6. CONTRACTOR INCURRED EXPENSES.

2.6.1. All expenses incurred by Contractor and employees while executing this Switchgear contract to include, but not limited to: travel expenses (taxi, plane, rental vehicle, fuel, and public transportation), lodging, and dining will be the sole responsibility of the Contractor to procure. Any and all compensation for these expenses will be the responsibility of the Contractor, and thus considered as include in provides pricing quote.

2.6.2. Contractor shall arrange, book, and pay for all travel reservations and related expenditures for the Contractor and its employees while traveling to and from the vicinity of the Silencer install site and while working and residing in the vicinity of Torii Station.

2.6.3. Contractor shall procure and pay for all lodging and dining for contracted employees.

2.6.4. Contractor shall be responsible for all other miscellaneous expenses necessary to support the subsistence of contracted employees.
