PERFORMANCE WORK STATEMENT

2 GENERAL INFORMATION

SECTION 1

DESCRIPTION OF SERVICES

1. OBJECTIVE:

The Contractor shall provide management and labor to of the Kadena Air Base Airman and

Family Readiness Center (A&FRC) as set forth in this PWS. The purpose is to conduct and maintain data entry, administrative assistance and client services for the Transition Assistance

Program (TAP). These duties are to be performed in the A&FRC, or any location the A&FRC conducts transition services.

SERVICES DESCRIPTION:

1.1 Administers the timely and accurate input of data received from TAP Core Compliance

Expert.

1.2 Responsible for the daily collection of accurate data and input into the Air Force Family

Integrated Reporting and Statistical Tracking (AFFIRST) system.

1.3 Maintain an inventory log of TAP material.

1.4 Maintains and tracks the coordination of forms within TAP.

1.5 Provides technical assistance to users of the TAP Resource Center, assisting in the operation of computer-based programs and other programs available to transitioning personnel.

1.5.1 Maintains system documentation, memory and application configuration; maintains network schema, checklists, and systems operations logbook.

1.5.2 Perform routine system maintenance and weekly system backups.

1.5.3 Ensures all clients completely fill out sign-in log and generate a monthly client usage report in the format established by A&FRC management not later than the fifth working day of the next month.

1.5.4 Maintains computer security at all times to include the following:

a) TAP Resource Center clients only access authorized programs using the main menu.

b) Access on-line programs under the direct supervision of the TAP consultants.

c) Document /report all computer software/hardware malfunctions or abuse to the WGM.

3. OTHER SIGNIFICANT FACTORS/QUALIFICATIONS: Contractor must have a background in, and working knowledge of:

3.1 Must be proficient in applying and troubleshooting Microsoft Office Suite applications to include MS Windows, MS Word, MS excel, MS PowerPoint and must quickly become proficient on other available resource software in order to maintain integrated reports using Microsoft

Word with Excel spreadsheet and data analysis.

3.2 Contractor shall dress in appropriate business casual attire while in performance of contract

4. GOVERNMENT FURNISHED PROPERTY AND SERVICES:

4.1 The government will provide workspace, desk, computer, associated equipment and office supplies.

GENERAL INFORMATION

5. HOURS OF WORK:

5.1 Services will be performed Monday through Friday, except U.S. holidays from 0800-1700 hours.

5.2 FEDERAL HOLIDAYS: When the holiday falls on a Saturday, the preceding Friday will be the federal holiday. When the holiday falls on a Sunday, the following Monday will be the federal holiday. Asterisk (*) indicates the day of the week change each year. The Wing

Commander sometimes declares “family days” or other days for base personnel to take a holiday. These holidays are not chargeable to the government if the schedule is not worked.

6. SECURITY REQUIREMENTS:

6.1. Security Training. All contractor employees shall receive initial and recurring security education training from the sponsoring agency’s security manager. Training must be conducted in accordance with DoD 5200.1-R, Information Security Program, and AFI 31-401, Information

Security Program Management.

6.2 E-Mail Access. In accordance with AFI 33-119, Electronic Mail (E-mail) Management and

Use, paragraph 9.2.5., DoD military, civilians, or contractor personnel using unclassified automated information systems that have access to sensitive information must possess, at a minimum, a National Agency Check or Entrance National Agency Check in accordance with

DoD 5200.2-R, Personnel Security Program, or screening in accordance with AFI 31-501, Personnel Security Program Management. Those personnel requiring access to classified systems are subject to the appropriate investigation.

6.3 Physical Security. The contractor shall be responsible for safeguarding all government property provided for contractor use. At the close of each work period, government facilities, equipment, and materials shall be secured.

6.4 Pass and Identification Items. The contractor shall ensure the pass and identification items required for the contract performance are obtained.

6.5 Retrieving Identification Media. The contractor shall return all identification media prior to contract expiration or termination of service.

6.6 Traffic Laws. The contractor shall comply with base traffic regulations.

6.7 Weapons Firearms and Ammunitions. Contract employees are prohibited from possessing weapons, firearms, or ammunition on themselves or within their contractor-owned vehicle or privately owned vehicle while on Kadena Air Base (AB).

6.8 Key Control. The contractor shall establish and implement methods of making sure all keys issued to the contractor by the Government are not lost or misplaced and are not used by unauthorized persons. The contractor shall not duplicate any keys issued by the government.

6.8.1 The COR or Program Manager shall be notified of any occurrences of lost or duplicated keys.

6.8.2 In the event keys, other than master keys, are lost or duplicated, the contractor may be required, upon written direction of the contracting officer, to re-key or replace the affected lock or locks without cost to the government. The government may, however, as its option, replace the affected lock or locks or perform re-keying and deduct the cost of such from the monthly payment due the contractor. In the event a master key is lost or duplicated, all locks and keys for that system shall be replaced by the government and the total cost deducted from the monthly payment due the contractor.

7. JAPANESE ROAD LAWS

7.1. The contractor shall comply with Japanese Road Laws while on US Military installations to specifically include: Article 43 The Road Law (Japan) (prohibited acts relative to roads). No person shall commit the following described acts:

7.1.1. Damaging the road, causing roads to be littered with debris, polluting the roads with debris or fall out from motor vehicles or otherwise defacing roads unreasonably or without due cause.

7.1.2. Article 43-2 (Measures to be taken to prevent cargo or things loaded on motor vehicles from falling out)

7.2. The agency managing a road may when there is reasonable cause for him to feel that items or cargo being carried in or on a motor vehicle may fall out and damage, pollute, or otherwise deface the road in such manner as to hinder or obstruct the traffic, order the operator of the vehicle in question to take steps or measures necessary to prevent occurrence of such hindrance by stopping operation of the vehicle, or correcting the method of loading or traveling or operation of the vehicle.

8. REQUIRED INSURANCE (In addition to that required by Japanese Law)

8.1 The Contractor shall procure and maintain during the entire period of his performance under this contract the following minimum insurance on each vehicle used by the Contractor at the work site.

8.1.1 Type: Automobile Property Damage Insurance Amount: ¥3,000,000 or Dollar equivalent

Bodily Injury Insurance Amount: ¥30,000,000 or Dollar equivalent

8.1.2 All vehicles must be properly inspected/insured in accordance with Japanese Compulsory

Insurance (JCI) requirements.

8.2 Prior to the commencement of work hereunder, the Contractor shall furnish to the

Contracting Officer a certificate or written statement of the above required insurance. The policies evidencing required insurance shall contain an endorsement to the effect that cancellation or any material change in the policies adversely affecting the interests of the U.S.

Government in such insurance shall not be effective for such period as may be prescribed by the laws of the state in which this contract is to be performed and in no event less than thirty (30) days after written notice thereof to the Contracting Officer.
