PERFORMANCE WORK STATEMENT (PWS)

ANDERSEN AFB, GUAM

MATERIAL TESTING EQUIPMENT CALIBRATIONS

1. GENERAL INFORMATION:

1.1 General: This is a non-personnel services contract to provide calibration services for material testing equipment, on Andersen AFB Guam (to include the Northwest Field area). The Government shall not exercise any supervision or control over the contract service providers performing the services herein. Such contract service providers shall be accountable solely to the Contractor who, in turn is responsible to the Government.

1.2 Description of Services/Introduction: The contractor shall provide all personnel, equipment, supplies, facilities, transportation, tools, materials, supervision, and other items and non-personal services necessary to perform material testing equipment calibration, preventative maintenance, repairs, and parts replacement as defined in this Performance Work Statement except for those items specified as government furnished property and services. Please see page 7 of this PWS for a list of equipment to be serviced. The contractor shall perform to the standards in this contract.

1.3 Background: The 554 RED HORSE Squadron Engineering Flight has equipment utilized for performing quality assurance of the design properties of soil, asphalt and concrete. These pieces of equipment shall be calibrated annually in order to ensure equipment is able to perform measured calculations within manufacturer’s tolerances.

1.3 Objectives: Perform all necessary calibrations, preventative maintenance, repairs, and parts replacement IAW the Contract terms and conditions.

1.4 Scope: Calibration of material testing equipment. The contractor shall accomplish complete inspection and documentation of devices prior to calibration. Devices will be disassembled and/or cleaned, inspected, lubricated (if required), parts are to be replaced (if necessary per contractor recommendation), and then calibrated. All calibrations are performed on certified NIST-traceable, calibrated devices and certificate documenting the calibration is provided to the user upon completion. Concrete air meters are cleaned and repaired as necessary before calibration. Calibration of air meters follows the ASTM procedure outlined in ASTM C231. The attachment lists equipment description, quantity, make and model of all equipment that needs service. The contractor, including all of it’s employees or technicians servicing the equipment, shall be ISO 17025 accredited. The contractor shall submit the proper documents as proof of their accreditation with their quotation, and must maintain the accreditation for the life of the contract.

1.5 Period of Performance: The period of performance shall be for one (1) Base Year of 12 months and four (4) 12-month option years. The Government also reserves the right to exercise additional performance periods from one (1) month, up to a total of six (6) month option period, IAW FAR clause 52.217-8, as included in the Contract.

The Period of Performance reads as follows:

1.6 Recognized Holidays: Contractor is not required to perform services on holidays.

1.7 Hours of Operation: The contractor is responsible for conducting business, between the hours of 0800-1600 Monday thru Friday except Federal holidays or when the Government facility is closed due to local or national emergencies, administrative closings, or similar Government directed facility closings. For other than firm fixed price contracts, the contractor will not be reimbursed when the government facility is closed for the above reasons. The Contractor must at all times maintain an adequate workforce for the uninterrupted performance of all tasks defined within this PWS when the Government facility is not closed for the above reasons. The contractor shall schedule work to be performed with 554 RED HORSE Squadron personnel during their annual visit to Guam and will notify 554 RED HORSE personnel no later than two weeks prior to arrival.

1.8 Place of Performance: The work to be performed under this contract will be performed at the 554 RED HORSE Squadron Engineering Building 61313, Northwest Field, Andersen Air Force Base, Guam.

1.9 Type of Contract: The government will award a service contract.

1.10 Contractor Personnel: The contractor shall provide a point of contact (POC) for all contractual matters during all hours of operation.

1.11 Security

1.11.1 Pass and Identification Items: The contractor shall ensure the pass and identification items required for contract performance are obtained for employees and will coordinate with 554 RED HORSE personnel as needed to obtain base access/escort.

1.11.2 Retrieving Identification Media: The contractor shall return all identification media and/or visitor passes from employees who depart for any reason before the contract expires; i.e. terminated for cause, reassignment, retirement, etc.

1.11.3 Traffic Laws: The contractor and its employees shall comply with base traffic regulations. Traffic regulations include, but are not limited to:

a. Employees shall wear seatbelts when operating or riding in any vehicle (military, company or private owned) while on installation.

b. Speed limits on Andersen AFB are 25 MPH unless otherwise posted. Be aware of reduced speed limits and exercise caution when driving in base housing and school zones.

c. All vehicle operators on the installation shall not use cell phones unless the vehicle is safely parked or they are using a 100% hands free device. The wearing of any other portable headphones, earphones or other listening devices while operating a motor vehicle is prohibited.

1.11.4 Weapons, Firearms, and Ammunition: Employees are prohibited from possessing weapons, firearms, or ammunition on themselves or within vehicles (military, contractor-owned, or privately owned) while on Andersen AFB.

1.11.5 For Official Use Only (FOUO): The contractor shall comply with DoD 5400.7-R, Chapter 4, DoD Freedom of Information Act (FOIA) Program, requirements. This regulation sets policy and procedures for the disclosure of records to the public and for marking, handling, transmitting, and safeguarding FOUO material.

1.11.6 Reporting Requirements: Contractor personnel shall report to an appropriate authority any information or circumstances of which they are aware may pose a threat to the security of DoD personnel, contractor personnel, resources, and classified/unclassified defense information.

1.11.7 Physical Security: The contractor shall be responsible for safeguarding all Government property and controlled forms provided for contractor use.

1.11.8 Controlled/Restricted Areas: The contractor shall implement local base procedures for entry to AF Controlled/Restricted areas where contractor personnel will work. An AF Form 2586, Unescorted Entry Authorization Certificate, must be completed and signed by 554 RHS security manager before restricted area badges (RAB) will be issued.

Employees must have a completed and favorable NACI investigation before receiving a RAB. Interim access can be granted in accordance with AFI 31-501, Personnel Security Program Management.

1.11.9 Military Exercises: No compensation will be claimed for any delays resulting from re-entry procedures due to exercises.

1.12 Post Award Conference/Periodic Progress Meetings: The Contractor agrees to attend any post award conference convened by the contracting activity or contract administration office in accordance with Federal Acquisition Regulation Subpart 42.5.

The contracting officer, Contracting Officers Representative (COR), and other Government personnel, as appropriate, may meet periodically with the contractor to review the contractor's performance. At these meetings the contracting officer will apprise the contractor of how the government views the contractor's performance and the contractor will apprise the Government of problems, if any, being experienced.

Appropriate action shall be taken to resolve outstanding issues. These meetings shall be at no additional cost to the government.

1.13 Organizational Conflict of Interest: Contractor and subcontractor personnel performing work under this contract may receive, have access to or participate in the development of proprietary or source selection information (e.g., cost or pricing information, budget information or analyses, specifications or work statements, etc.) or perform evaluation services which may create a current or subsequent Organizational Conflict of Interests (OCI) as defined in FAR Subpart 9.5. The Contractor shall notify the Contracting Officer immediately whenever it becomes aware that such access or participation may result in any actual or potential OCI and shall promptly submit a plan to the Contracting Officer to avoid or mitigate any such OCI. The Contractor’s mitigation plan will be determined to be acceptable solely at the discretion of the Contracting Officer and in the event the Contracting Officer unilaterally determines that any such OCI cannot be satisfactorily avoided or mitigated, the Contracting Officer may effect other remedies as he or she deems necessary, including prohibiting the Contractor from participation in subsequent contracted requirements which may be affected by the

OCI.

2. CONTRACTOR FURNISHED ITEMS AND SERVICES:

2.1 Reports: The contractor shall submit an inspection, preventive maintenance and load test report in English. The report shall be submitted to the customer and Contracting Officer within 15 business days after the date of actual performance and at a minimum shall contain the following information:

(1) Date Work Started

(2) Name of technician or individual who performed the service

(3) Description and method of service performed

(4) Inspection, maintenance and load test data

(5) Recommendation for effective maintenance and operation

(6) Work Completion Date

(7) Signature of Contractor Representative

(8) Any other pertinent information

2.2 Materials: The Contractor shall provide all materials needed to perform cleaning, inspection, calibration and replacement parts for equipment to be calibrated.

2.3 Equipment: The Contractor shall provide all equipment needed to perform cleaning, inspection, calibration and replacement parts for equipment to be calibrated.

3. DEFINITIONS AND ACRONYMS:

3.1. CONTRACTOR. A supplier or vendor awarded a contract to provide specific supplies or service to the government. The term used in this contract refers to the prime.

3.1.2. CONTRACTING OFFICER. A person with authority to enter into, administer, and or terminate contracts, and make related determinations and findings on behalf of the government. Note: The only individual who can legally bind the government.

3.1.3. PHYSICAL SECURITY. Actions that prevent the loss or damage of Government property.

3.1.4. SUBCONTRACTOR. One that enters into a contract with a prime contractor. The Government does not have privity of contract with the subcontractor.

3.1.5. WORK DAY. A work day is defined as a standard business day (Monday through Friday with the exception of Federal Holidays or PACAF Family days), which occurs from 8:00 AM to 4:00 PM. The Contractor may request extended or different work hours in writing to the Contracting Officer. Any request for extended or different work hours must be approved by the Contracting Officer, and the appropriate 36 Security Forces Squadron Office. The Contractor should allow for at least two weeks approval time for any request.

3.1.6. WORK WEEK. Monday through Friday, unless specified otherwise.
