Combined Synopsis- Solicitation for Commercial Items (IAW FAR 12.603) 15 May 2018 Contracting Office Address Department of the Air Force, Pacific Air Forces, 766 SCONS – Joint Base Elmendorf Richardson (JBER), 10480 Sijan Ave, Suite 253, JBER, AK 99506, United States.

Description

This is a combined synopsis/solicitation prepared in accordance with Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This is not to be construed as a commitment by the Government. All information is to be submitted at no cost to the Government.

The solicitation will be issued as a Request for Quotes (RFQ) using FAR part 13 procedures under the solicitation number FA5215-18-Q-9008.

Due Date and Time:

Quotes are due no later than 11:00 a.m., 21 June 2018 – Alaska Standard Time.

This solicitation document and incorporated provisions and clauses are those in effect through the Federal Acquisition Circular (FAC) 2005-95 as of 20170119, Defense Federal Acquisition Regulation Supplement Publication Notice (DPN) 20161222 and Air Force Acquisition Circular (AFAC) 2017-0127.

This acquisition is Full and Open.

The following attachments are applicable to this solicitation and will be provided with the RFQ: Attachment 1 – Statement of Work (SOW) dated, 28 Dec 2017 Attachment 2 – Scheduled Government Furnished Property Attachment 3 – Technical Order 35E8-2-5-3 applicable sections Attachment 4 – MIL-DTL-5541F

Upon written request for wage determinations, the solicitation will be amended to include those WD’s requested by an offeror when submitted by: 11:00 a.m., Alaska time, 12 June 2018. Requests received late will be handled in accordance with FAR 22.1009-4(c).

The following provisions are applicable to this solicitation. Please note, this is an RFQ using simplified acquisition procedures in accordance with FAR 13. For the clauses and provisions listed in this solicitation, the terms “offeror” and “bidder” are considered to mean “vendor”; and the terms “offer” and “proposal” are considered to mean “quote”

The FAR provision 52.212-1, Instruction to Offerors-Commercial Items (Jan 2017), applies to this acquisition. Addenda to the following paragraphs of 52.212-1 are:

ADDENDUM TO 52.212-1

52.212-1 INSTRUCTION TO OFFERORS-COMMERCIAL ITEMS

Addendum to 52.212-l(c), Period for Acceptance of Offers. The paragraph is tailored as follows: "The Offeror agrees to hold the prices in its offer firm for 120 calendar days from the date specified for receipt of offers."

NOTE

All quote preparation cost will be the sole responsibility of the Offeror. The Government will not reimburse any firm for preparation of their quote.

A. To assure timely and equitable evaluation of proposals, offerors must follow the instructions contained herein. Offerors are required to meet all solicitation requirements, including terms and conditions, representations and certifications, and technical requirements, in addition to those identified as evaluation factors or subfactors. Offerors must clearly identify any exception to the solicitation terms and conditions and provide complete accompanying rationale. The response shall consist of two (2) separately bound parts, Part I - Price Quote and Part II – Technical Requirements.

B. Specific Instructions:

1. PART I - PRICE QUOTE

a. Complete all CLINS in the Combined Synopsis-Solicitation above. The extended amount must equal the whole dollar unit price multiplied by the number of units. It is imperative that mathematical calculations are correct; particular care should be taken to ensure the amounts match when using a spreadsheet program, such as Microsoft 3.

EXCEL. The unit prices and extended amounts are limited to two decimal places.

b. Complete the necessary fill-ins and certifications in provisions. The provisions FAR 52.212-3 shall be returned along with the quote if they have not been completed in System for Award Management (SAM).

c. Acknowledge any and all amendments to the solicitation.

2. Part II, Technical Requirements

The vendors must state their qualifications and approvals to perform the services in accordance with the SOW and applicable portions of T.O. 35E8-2-5-3 - 17 June 2011, Change 3 - 21 January 2015, and MIL-DTL-5541F.

In addition to approved vendors identified in T.O. 35E8-2-5-3 for Shuttle Valve refurbishment, only approved vendors are permitted to perform brake carrier re-sleeving. Approved vendors are those vendors that manufacture and sell brake carriers to the United States Air Force or have successfully completed First Article Testing (FAT) IAW procedures established by WRLCMC/WNZEC, Robins AFB Georgia.

General Information

Quotes shall be submitted in electronic format and emailed to jeannette.morphew@us.af.mil no later than the solicitation due date and time. Email is the required method; however it is the contractor’s responsibility to follow up with the listed POC to ensure their quote was received on time.

Since award will be based on initial responses, offerors are highly encouraged to quote their most advantageous pricing in their initial response.

NO CORRECTIONS AND/OR CHANGES ARE ALLOWED AFTER DUE DATE AND TIME OF SUBMISSION OF OFFORS.

52.212-2 EVALUATION--COMMERCIAL ITEMS (OCT 2014)

(a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers:

(b) A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer's specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award.

52.252-1 Solicitation Provisions Incorporated by Reference (FEB 1998)

This solicitation incorporates one or more solicitation provisions by reference, with the same force and effect as if they were given in full text. Upon request, the Contracting Officer will make their full text available. The offeror is cautioned that the listed provisions may include blocks that must be completed by the offeror and submitted with its quotation or offer. In lieu of submitting the full text of those provisions, the offeror may identify the provision by paragraph identifier and provide the appropriate information with its quotation or offer. All responsible sources may submit a quote, which will be considered by the Government. Award will be made to the offeror whose quote conforms to the solicitation requirements and is able to satisfy the Government’s requirements. Offeror must ensure the provision FAR 52.212-3, Offeror Representation and Certifications- Commercial Items (APR 2016) with its Alternate I (this must be completed on http://www.sam.gov). If the Offeror has not completed the annual representations and certifications electronically, the Offeror shall complete only paragraphs (c) through (r) of this provision All referenced Federal Acquisition Regulation (FAR) clauses and provisions in this solicitation may be accessed via the internet at https://farsite.hill.af.mil.

All offerors must complete the following paragraph from FAR 52.212-3(b). If no changes were made to the offerors annual representations and certifications in response to this solicitation, enter N/A below.

(1) Annual Representations and Certifications. Any changes provided by the offeror in paragraph (b)(2) of this provision do not automatically change the representations and certifications posted on the SAM website.

(2) The offeror has completed the annual representations and certifications electronically via the SAM website accessed through https://www.acquisition.gov. (d) Comptroller General Examination of Record. The Contractor shall comply with the provisions of this paragraph (d) if this contract was awarded using other than sealed bid, is in excess of the simplified acquisition threshold, and does not contain the clause at 52.215-2, Audit and Records--Negotiation.

(1) The Comptroller General of the United States, or an authorized representative of the Comptroller General, shall have access to and right to examine any of the Contractor's directly pertinent records involving transactions related to this contract.

(2) The Contractor shall make available at its offices at all reasonable times the records, materials, and other evidence for examination, audit, or reproduction, until 3 years after final payment under this contract or for any shorter period specified in FAR Subpart 4.7, Contractor Records Retention, of the other clauses of this contract. If this contract is completely or partially terminated, the records relating to the work terminated shall be made available for 3 years after any resulting final termination settlement. Records relating to appeals under the disputes clause or to litigation or the settlement of claims arising under or relating to this contract shall be made available until such appeals, litigation, or claims are finally resolved.

(3) As used in this clause, records include books, documents, accounting procedures and practices, and other data, regardless of type and regardless of form. This does not require the Contractor to create or maintain any record that the Contractor does not maintain in the ordinary course of business or pursuant to a provision of law.

This clause prohibits discrimination against qualified individuals on the basis of disability, and requires affirmative action by the Contractor to employ and advance in employment qualified individuals with disabilities.

252.209-7010 Critical Safety Items. (AUG 2011)

Definitions.

“Aviation critical safety item” means a part, an assembly, installation equipment, launch equipment, recovery equipment, or support equipment for an aircraft or aviation weapon system if the part, assembly, or equipment contains a characteristic any failure, malfunction, or absence of which could cause— A catastrophic or critical failure resulting in the loss of, or serious damage to, the aircraft or weapon system; An unacceptable risk of personal injury or loss of life; or An uncommanded engine shutdown that jeopardizes safety“Design control activity” means— With respect to an aviation critical safety item, the systems command of a military department that is specifically responsible for ensuring the airworthiness of an aviation system or equipment, in which an aviation critical safety item is to be used; and With respect to a ship critical safety item, the systems command of a military department that is specifically responsible for ensuring the seaworthiness of a ship or ship equipment, in which a ship critical safety item is to be used.

“Ship critical safety item” means any ship part, assembly, or support equipment containing a characteristic, the failure, malfunction, or absence of which could cause— A catastrophic or critical failure resulting in loss of, or serious damage to, the ship; or An unacceptable risk of personal injury or loss of life.

Identification of critical safety items. One or more of the items being procured under this contract is an aviation or ship critical safety item. The following items have been designated aviation critical safety items or ship critical safety items by the designated design control activity:

Heightened quality assurance surveillance. Items designated in paragraph (b) of this clause are subject to heightened, risk-based surveillance by the designated quality assurance representative.

252.222-7000 RESTRICTIONS ON EMPLOYMENT OF PERSONNEL (MAR 2000)

(a) The Contractor shall employ, for the purpose of performing that portion of the contract work in Alaska, individuals who are residents thereof and who, in the case of any craft or trade, possess or would be able to acquire promptly the necessary skills to perform the contract.

(b) The Contractor shall insert the substance of this clause, including this paragraph (b), in each subcontract awarded under this contract.

The following AFFARS Mandatory Procedure applies: 5352.201-9101 OMBUDSMAN (JUN 2016)

(a) An ombudsman has been appointed to hear and facilitate the resolution of concerns from offerors, potential offerors, and others for this acquisition. When requested, the ombudsman will maintain strict confidentiality as to the source of the concern. The existence of the ombudsman does not affect the authority of the program manager, contracting officer, or source selection official. Further, the ombudsman does not participate in the evaluation of proposals, the source selection process, or the adjudication of protests or formal contract disputes. The ombudsman may refer the interested party to another official who can resolve the concern.

(b) Before consulting with an ombudsman, interested parties must first address their concerns, issues, disagreements, and/or recommendations to the contracting officer for resolution. Consulting an ombudsman does not alter or postpone the timelines for any other processes (e.g., agency level bid protests, GAO bid protests, requests for debriefings, employee-employer actions, contests of OMB Circular A-76 competition performance decisions).

(c) If resolution cannot be made by the contracting officer, the interested party may contact the ombudsman,CMSGT Carr, AFICA/KH (Chief Enlisted Manager), 25 E. Street, Suite B-100, JBPH-H, HI 96853-5427, Phone: (808) 449-8569 or FAX: (808) 449-8571. (d) The ombudsman has no authority to render a decision that binds the agency.

(e) Do not contact the ombudsman to request copies of the solicitation, verify offer due date, or clarify technical requirements. Such inquiries shall be directed to the Contracting Officer.

5352.223-9001 HEALTH AND SAFETY ON GOVERNMENT INSTALLATIONS (NOV 2012)

(a) In performing work under this contract on a Government installation, the contractor shall:

(1) Take all reasonable steps and precautions to prevent accidents and preserve the health and safety of contractor and Government personnel performing or in any way coming in contact with the performance of this contract; and

(2) Take such additional immediate precautions as the contracting officer may reasonably require for health and safety purposes.

(b) The contracting officer may, by written order, direct Air Force Occupational Safety and Health (AFOSH) Standards and/or health/safety standards as may be required in the performance of this contract and any adjustments resulting from such direction will be in accordance with the Changes clause of this contract.

(c) Any violation of these health and safety rules and requirements, unless promptly corrected as directed by the contracting officer, shall be grounds for termination of this contract in accordance with the Default clause of this contract.

5352.242-9000 CONTRACTOR ACCESS TO AIR FORCE INSTALLATIONS (Nov 2012)

a) The contractor shall obtain base identification and vehicle passes, if required, for all contractor personnel who make frequent visits to or perform work on the Air Force installation(s) cited in the contract. Contractor personnel are required to wear or prominently display installation identification badges or contractor-furnished, contractor identification badges while visiting or performing work on the installation.

(b) The contractor shall submit a written request on company letterhead to the contracting officer listing the following: contract number, location of work site, start and stop dates, and names of employees and subcontractor employees needing access to the base. The letter will also specify the individual(s) authorized to sign for a request for base identification credentials or vehicle passes. The contracting officer will endorse the request and forward it to the issuing base pass and registration office or Security Forces for processing. When reporting to the registration office, the authorized contractor individual(s) should provide a valid driver’s license, current vehicle registration, and valid vehicle insurance certificate, to obtain a vehicle pass.

(c) During performance of the contract, the contractor shall be responsible for obtaining required identification for newly assigned personnel and for prompt return of credentials and vehicle passes for any employee who no longer requires access to the work site.

(d) When work under this contract requires unescorted entry to controlled or restricted areas, the contractor shall comply with AFI 31-501 citing the appropriate paragraphs as applicable.

(e) Upon completion or termination of the contract or expiration of the identification passes, the prime contractor shall ensure that all base identification passes issued to employees and subcontractor employees are returned to the issuing office.

(f) Failure to comply with these requirements may result in withholding of final payment. (End of clause)
