The purpose of this Amendment is to make the follow ing changes:

1. Remove section 1.3.2.1 Configuration Support from the Performance Work Statement (PWS).

2. Remove subsections (a), (b), and (c) from section 1.4.2.1 Preventative Maintenance of the PWS.

3. Corrected 1.4.2 (b) typographical error from GIGGIG to GIG of the PWS.

4. Changed Facility/Link Data Reports to Facility Reports in Appendix 1,3, and 5 of the PWS.

5. Changed deadline for Past Performance Questionnaire's (PPQs) submission from 1600 on 08 April 2013 to 1600 on 12 April 2013.

6. IAW Section 4, Question 2 of the PPQ, adequate justif ication and specif ic examples must be provided for any rating of Exceptional, Marginal, or Unsatisfactory by an offeror's past performance contract reference.

7. IAW Section 1 of PPQ, all question rating check box's must be completed w ith either a rating of Exceptional, Very Good, Satisfactory, Mariginal, Unsatisfactory, or Not Available to suff iciently analyze past performance.

C. Information concerning this solicitation or requests for clarification will not be provided in response to telephone calls initiated by an Offeror. L-2 DISCREPANCIES

If an offeror believes that the requirements in these instructions contain an error or omission, or are otherwise unsound, the offeror shall immediately notify the CO in writing with supporting rationale.

L-3 INCURRING COSTS

The Government assumes no liability for and will not be obligated to the Contractor for payment for costs incurred prior to award including proposal preparation, submission, pre-proposal and site visit costs.

L-4 SUBMISSION OF COMPLETE PROPOSALS

Offerors are cautioned to strictly comply with Sections B, C, G, H, K, L, and M to ensure submission of complete proposals. Failure to provide a complete proposal at the time of proposal submission may result in the proposal not being acceptable to the Government and elimination from consideration for award.

L-5 SOURCE SELECTION PROCEDURES

This source selection is being conducted in accordance with Performance Price Trade-off (PPT): FAR 15.101-1.

L-6 SITE VISIT

Offerors are extended the opportunity to make a site visit to the Technical Control Facility for the purpose of seeing facility layout, equipment, and contractor office space as well as discuss program needs with the Government personnel. This site visit will be at no expense to the Government. A representative from 747 CS and the 647 CONS will accompany the offeror’s representatives. The site visit will commence on 21 March 2013 at 10:00 A.M. Hawaii Standard Time. To attend, Contractors must submit the name, institution/company name, individual security clearance, and position title of each offeror’s representative to the following address no later than 18 March 2013, 4:00 P.M, Hawaii Standard Time.

Details on security and transportation requirements will be provided after requests to participate in the site visit is received. Airfare to and from the site visit is the responsibility of the participants.

Participation is limited to two (2) individuals per company.

Offerors who later decide not to attend the site visit for any reason must notify Joshua Berger and A1C Robert Ronayne in writing no later than 20 March 2013, 4:00 P.M, Hawaii Standard Time for http://www.fedbizopps.gov/ mailto:Robert.Ronayne@us.af.mil cancellation. An offeror’s absence or non-participation during the site visit does not preclude submission of proposal for this solicitation.

L-7 GENERAL INFORMATION AND INSTRUCTIONS

A. The Offeror’s proposal must include all data and information requested by the Instructions to

Offerors (ITO) and must be submitted in accordance with these instructions. The proposal must be compliant with the requirements of the PWS. Non-conformance with the ITO may result in an unfavorable proposal evaluation. The proposal must be clear and concise, and must include sufficient detail for effective evaluation.

B. The Government reserves the right to revise or amend the PWS or the solicitation prior to the proposal closing time. Such revisions or amendments will be communicated by formal amendment (Standard Form 30) to this RFP. If such amendments require material changes in quantities or prices, the proposal closing date may be postponed by enough days to enable offerors to revise their proposals. In such cases, the amendment will include an announcement of the new proposal closing date and time.

L-8 PROPOSAL PREPARATION INSTRUCTION

A. To assure timely and equitable evaluation of proposals, offerors must follow the instructions contained herein. Offerors are required to meet all solicitation requirements, including terms and conditions, representations and certifications, and technical requirements, in addition to those identified as evaluation factors or subfactors. Offerors must clearly identify any exception to the solicitation terms and conditions and provide complete accompanying rationale. The response shall consist of three (3) separate parts; Part I - Price Proposal, Part II - Technical Proposal, and Part III - Past Performance Information.

B. The CO has determined there is a high probability of adequate price competition in this acquisition.

Upon examination of the initial offers, the CO will review this determination and if, in the contracting officer's opinion, adequate price competition exists no additional data will be requested and certification under FAR 15.406-2 will not be required. However, if at any time during this competition the contracting officer determines that adequate price competition no longer exists; offerors may be required to submit cost data to the extent necessary for the contracting officer to determine the reasonableness and affordability of the price.

Specific Instructions:

1. PART I – PRICE PROPOSAL - Submit original and one (1) copy

(a) Complete blocks 13, 15, 16, 17 and 18 of the RFP Section A, SF33 (In doing so, the offeror accedes to the contract terms and conditions as written in the RFP Sections A through K. These sections constitute the model contract.

(b) Insert proposed unit and extended prices in Section B for each Contract Line Item Number (CLIN) or Sub CLIN (SCLIN), including all option periods. The extended amount must equal the whole dollar unit price multiplied by the number of units.

(c) Complete the necessary fill-ins and certifications in Sections I through K. Section K shall be returned in its entirety. For Sections C through I, the offeror shall submit only those pages that require a fill-in.

(d) If applicable, provide a copy of the letter from the Small Business Administration (SBA) showing proof of Section 8(a) status.

2. PART II – TECHNICAL PROPOSAL - Limited to no more than 9 pages. Submit original plus three (3) copies.

(a) SUBFACTOR 1: Experience (limited to 3 pages):

1) The offeror shall demonstrate a minimum of 3 years (Government or Commercial) within 5 years from the RFP date, experience in technical control/long haul communication services.

The offeror must exhibit sufficient experience on all of the following aspects: operating and maintaining circuit-based networks, maintain copper and fiber-based connectivity and associated equipment, providing end-to-end support service, and adhering to outlined reporting procedures in the prescribed timeframes required.

2) The offeror shall demonstrate a minimum of 3 years (Government or Commercial), within 5 years from the RFP date, experience in managing a Technical Control Facility (TCF), encompassing wired telecommunications, voice, and data transmissions.

(b) SUBFACTOR 2: Management Approach – Staffing Plan (limited to 6 pages):

1) The proposal shall include a manning approach, which includes hiring, retaining, and backfilling vacancies with fully trained and qualified technicians with the appropriate level of security clearance and experience. Members assigned to performed duties within the TCF must have experience with operating and maintaining circuit-based networks, maintain copper and fiber-based connectivity and associated equipment, provide end-to-end support service, and adhere to outlined reporting procedures in the prescribed timeframes required.

3. PART III – PAST PERFORMANCE INFORMATION - Limited to no more than 6 pages per contract listed. Only references for relevant past efforts/contracts are desired. Submit original plus five (5) copies.

(a) Quality and Satisfaction Rating for Contracts Completed in the Past Three Years:

Provide any information currently available (letters, metrics, customer surveys, independent surveys, etc.)

which demonstrates customer satisfaction with overall job performance and quality of completed product for relevant past efforts/contracts (similar in scope and magnitude of effort and complexities this solicitation requires). In addition, explain corrective actions taken in the past, if any, for substandard performance and any current performance problems such as cost overruns, extended performance periods, numerous warranty calls, etc.

(b) Performance Surveys: The government will evaluate the quality and extent of offeror's performance deemed relevant to the requirements of this RFP. The government will use information submitted by the offeror and any other sources of information available to the Government to assess past performance. Provide a list of no more than eight (8), of the most relevant contracts performed for Federal agencies and commercial customers within the last three (3) years, a maximum of five (5) contracts for the prime, teaming partner or joint venture and a maximum of three (3) contracts for subcontractors. Relevant contracts are those of similar scope, magnitude and complexity to the requirement of the current solicitation, see Section M, paragraph M-1C(1). The evaluation of past performance information will take into account past performance information regarding predecessor companies, key personnel who have relevant experience, or subcontractors that will perform major or critical aspects of the requirement when such information is relevant to the instant acquisition. Furnish the following information for each contract listed:

(i) Company/Division name

(ii) Product/Service

(iii) Contracting Agency/Customer

(iv) Contract Number

(v) Contract Dollar Value

(vi) Period of Performance

(vii) Verified, up-to-date name, address, FAX & telephone number of the contracting officer

(viii) Comments regarding compliance with contract terms and conditions

(ix) Comments regarding any known performance deemed unacceptable to the customer, or not in accordance with the contract terms and conditions.

The Past Performance Surveys (PPS) shall be used by the offeror in obtaining and providing past performance information. The Government requests the offeror send out PPS to each of the Points of Contact (POCs) identified in the Past Performance Volume.

The Offeror shall send out the PPS (Attachment J-6), using the Past Performance Evaluation Cover Letter (Attachment J-5), to each of their references. The offeror shall indicate in Part III to which references the surveys were sent.

THE RESPONSIBILITY TO SEND OUT THE PAST PERFORMANCE SURVEY RESTS

SOLELY WITH THE OFFEROR.

Once the surveys are completed by the POCs, the information contained therein shall be considered source selection sensitive and shall not be released outside the Government. The POCs shall forward their completed Surveys directly to the Government – NOT BACK TO THE OFFEROR!

The offeror shall provide the Past Performance Surveys to the contracting office at the address specified below no later than 12 April 2013 by 4:00 P.M. HST.

Offerors must ensure that the PPS’s are sent to ALL REFERENCES and completed within sufficient time, in accordance with directions stipulated in this solicitation. It is the sole responsibility of the Offeror to track and ensure that the Government receives the PPS’s no later than the proposal receipt date established in this solicitation.

If a teaming arrangement is contemplated, provide complete information as to the arrangement, including any relevant and recent past performance information on previous teaming arrangements with same partner. If this is a first time joint effort, each party to the arrangement must provide a list of past relevant contracts. Please provide information on team partners and subcontractors performing major or critical aspects of the requirement, describing the portions of the effort for which they are responsible.

(c) Subcontractor Consent: Past performance information pertaining to a subcontractor cannot be disclosed to the prime offeror without the subcontractor’s consent. Provide with the proposal a letter from all subcontractors that will perform major or critical aspects of the requirement, consenting to the release of their past performance information to the prime contractor.

A. Documents submitted in response to this RFP must be fully responsive to and consistent with the following:

1. Requirements of the RFP (CLINs & PWS) and government standards and regulations pertaining to the PWS.

2. Evaluation Factors for Award in Section M of this RFP.

3. Any limitation on the number of proposal pages. Pages exceeding the page limitations set forth in this Section L will not be read or evaluated, and will be removed from the proposal.

4. Format for proposal Parts II and III shall be as follows:

(a) The proposals will be 8 1/2” x 11” paper except for fold-outs used for charts, tables, or diagrams, which may not exceed 11” x 17”.

(b) A page is defined as one face of a sheet of paper containing information.

(c) Typing shall not be less than 12 pitch.

(d) Elaborate formats, bindings or color presentations are not desired or required.

All other Parts I & II must be received at the above address no later than 15 April 2013, by 4:00 p.m.
