SUMMARY OF PERFORMANCE WORK STATEMENT

ASH RECYCLING SERVICE FOR YOKOTA AIR BASE, JAPAN

28 Mar 19

1. DESCRIPTION OF SERVICES/GENERAL INFORMATION. The Contractor shall provide all personnel, equipment, tools, materials, vehicles, supervision, and other items and services necessary to perform ash-recycling service for Yokota AB, Japan in accordance with applicable Japanese laws and pertinent regulations, and commercial practices. The Contractor shall accomplish all services below to meet the requirements of this Performance Work Statement (PWS). The Contractor shall possess all applicable business permits.

1.1. Recycling Ash. The Contractor will recycle the ash generated by the base refuse incinerator plant, Bldg 1526, at Yokota Air Base in accordance with Air Force Policy on Achieving Efficiencies through Pollution Prevention and Waste Elimination (See Para 3.1.2 below). The transportation of ash shall be provided by the Government’s designated contractor to the Contractor’s Recycling Facility.

1.2. Submittals and Reports

1.2.1. Business Permits. The Contractor, as a minimum, shall possess and submit a copy of the valid business permits listed below to perform the services specified in this PWS in conformance with Japanese laws and pertinent regulations:

Required Business Permit: Industrial Waste Treatment Business Permit (産業廃棄物処分業許可)

Item: Bottom Ash (燃え殻)and Fly Ash (ばいじん)

Method: Final and/or Intermediate Treatment (Recycling): (最終処分または中間処分)

1.2.2. Flow Chart. The Contractor shall submit the ash recycling flow chart to the Contracting Officers Representative (COR) who will perform the complete audit trail of the ash recycling procedure, to include the final disposition, within ten (10) calendar days after contract award.

1.2.3. Monthly Reports. The Contractor shall submit four (4) copies of the all reports in English & Japanese to the COR within thirty (30) calendar days after measurements are taken. The contractor shall submit receipts translated in English from every ash recycling transaction to include the quantity of recycled product. If Ash is not recycled the contractor must submit receipts of any disposal transactions to include quantity and final disposition location (i.e. name and location of landfill). The contractor will also submit any Ash compound analysis reports that are performed on the ash generated at Yokota AB Japan. This report must be provide to the COR before submitting the invoice.

1.2.4. Manifest. The Contractor shall control manifests in accordance with the Japanese Waste Management and Public Cleansing Law Para 12-3, Manifest. The manifests for the generator’s pages shall be submitted to the COR with the Contractor's monthly invoice.

2. GENERAL INFORMATION

2.1. Quality Control.

2.1.1. The Contractor shall ensure all services are performed in accordance with Japan Environmental Governing Standards (JEGS) Chapter 7, Japanese laws (Industrial Waste Treatment Low, 産業廃棄物処 理法), pertinent regulations and commercial standards and practices.

2.1.2. The Contractor shall meet the requirement in Air Force Policy on Achieving Efficiencies through Pollution Prevention and Waste Elimination. The goal applicable to this project is:

“Divert 65 percent, by weight, of non-hazardous solid waste from landfill disposal.

3.5. SPECIAL QUALIFICATIONS.

3.5.1. Business Permits. The contractor shall possess any and all applicable business permits, certifications, and licenses to complete service in accordance with this PWS as required by Japanese Environmental Government Standards and Japanese laws. The contractor will ensure all documents are current. The contractor shall submit a copy of each of these documents to the Contracting Officer (CO) within one week after receiving updated or renewed documents.

3.5.1. Project Manager (PM): The contractor shall provide a PM who shall be responsible for the performance of the work. The name of this person and his/her alternate(s) shall be designated in writing to the CO within ten (10) calendar days after contract award.

3.5.1.1. The PM or his/her alternate(s), upon notification, shall be available during normal working hours identified in Paragraph 3.4.1 within two (2) hours to meet on the installation with the COR to discuss problems.

3.5.1.2. The PM and his/her alternate(s) must be able to read, write, speak, and understand English, or provide a translator.
