PERFORMANCE-BASED WORK STATEMENT

FOR

GREASE TRAP CLEANING,

WASTE COOKING OIL COLLECTION AND RECYCLING SERVICE

YOKOTA AB AND TAMA ANNEX

PERFORMANCE-BASED WORK STATEMENT

GREASE TRAP CLEANING, WASTE COOKING OIL COLLECTION

AND RECYCLING SERVICE

FOR YOKOTA AB AND TAMA ANNEX

1. DESCRIPTION OF SERVICE. The contractor shall provide all necessary personnel, supervision, equipment, tools, material, vehicles, and other items and services necessary to perform cleaning grease traps, collecting waste cooking oil, and recycling service for all waste collected through this service contract, for Yokota Air Base and

Tama Annex in accordance with applicable Japanese Environmental Governing Standards (JEGS), Japanese laws, pertinent regulations, and commercial standards and practices. The contractor shall accomplish all services below to meet the requirements of this Performance-based Work Statement (PWS). Estimated workload data is shown in

Appendix A, Estimated Workload Data.

1.1. Grease Trap Cleaning. The contractor shall remove solids from basket(s) connecting inlet pipes. The contractor shall pump out all contents, including grease/oil and sludge, and clean inside of grease traps. The contractor shall flush down with water and pump out remaining residue, and clear obstructions in inlet/outlet pipes to ensure unrestricted flow, required by the unique configuration of the grease trap. Before completion, the contractor shall add sufficient recharge water to restore the grease trap to proper operation and inspect all parts for defects. The recharged water must be higher than the lowest dividing plate. Problems shall be reported to the

Contracting Officer Representative (COR) in writing and orally within 24 hours of discovery.

1.2. Waste Cooking Oil Collection. The contractor shall collect used cooking oil stored in the contractor-furnished containers. The contractor shall furnish leak-proof, waterproof, and vermin-proof containers (minimum size 208.19 liters) for each building identified in Appendix A, Estimated Workload Data, to store used cooking oil. Each container shall have a latching lid or equivalent for easy customer access. In addition, the container shall be labeled or stenciled with “USED COOKING OIL”, serial number and Point of Contact (374 CES/CEOSS, DSN; 225-8124) in English and Japanese. The contractor shall maintain cleanliness of each container. All contractor-furnished containers shall remain as contractor’s property at the conclusion of this contract.

1.3 Transportation, Recycling and Disposal. The contractor shall properly transport and dispose of collected waste in accordance with “Waste Management and Public Cleansing Law”. The waste cooking oil shall be recycled to an off-base market.

1.4. Business Permits. The contractor shall possess any and all applicable business permits, certifications, licenses

,and other related documents to accomplish grease trap cleaning, waste cooking oil collection and recycling services in accordance JEGS and Japanese laws. The contractor will ensure all documents are current. The contractor shall submit a copy of each of these documents to the Contracting Officer as required.

1.5. Submittal and Reporting

1.5.1 Work Schedule. The contractor shall establish the monthly collection schedule in accordance with appendix

A, Estimated Workload Data. The schedule for the next month shall be submitted to the COR and Contracting

Officer by the 20 th of each month for approval.

1.5.2. Waste Treatment and Recycling Process. The contractor shall submit the waste treatment and recycling process chart to the Contracting Officer (CO) ten (10) days before contract start date.

1.5.3. Manifest. The contractor shall prepare and control manifests in the specific form specified in Regulation of

Waste Management and Public Cleaning Law. The manifests shall be submitted to the COR and CO with the contractor's monthly invoice.

1.5.4. Reports. The monthly report will show quantities of sludge and oil collection, date, collection point, recycled amounts and disposal amounts. This report shall be submitted to the COR, CO and CES/CEIE Environmental Office each month along with the contractor's invoice.

1.6. Spillage. The contractor shall be responsible for spillage cleanup and recovery at locations before collection vehicle transport from both Yokota AB and Tama Annex. In the event of a spill, the contractor shall be responsible for filling out and turning in a 374 CES/CEIE Spill Report (Appendix B) to COR prior to leaving base. All spills must be reported to COR.

1.7. GENERAL INFORMATION

1.7.1. Quality Control (QC). The contractor shall acquire samples from grease traps listed in Appendix B in order to develop a quality control plan and checklist to ensure all services are performed in accordance with JEGS, Japanese Laws, pertinent regulations, and commercial standards and practices. The QC plan will identify proper service, list ways the contractor will prevent defective service, ensure that all contractor activities are performed safely, and will at minimum address areas identified in paragraph 2 (Services Summary). The QC checklist will be a condensed list that is used to ensure application of the QC plan during all activities. The QC plan and checklist shall be submitted to the Contracting Officer (CO) for coordination within ten (10) calendar days after contract award.

1.7.1.1. Quality Assurance. The Government will evaluate the contractor’s performance by appointing a COR to monitor performance to ensure services are received. The COR will evaluate the Contractor’s performance through intermittent on-site inspections of the Contractor’s QC Plan. The Government may inspect each task during/after performance.

1.7.1.2. Government Remedies. The Contracting Officer shall follow FAR 52.212-4, Contract Terms and Conditions-

Commercial Items, for the contractor’s failure to perform satisfactory services or failure to correct non-conforming services.

1.7.2. Hours of Operation.

1.7.2.1. Normal Working Hours. The Contractor shall perform the contract services between the hours of 0730 to

1630, Monday through Friday including Japanese holidays, but excluding US legal holidays.

1.7.2.2. Recognized US Holidays. The recognized US legal holidays are listed below:

* Holidays falling on a Saturday will be observed on the preceding Friday. Holidays falling on a

Sunday will be observed on the following Monday.

1.7.3. Base Passes. The contractor’s employees will be issued base passes (identification cards), which shall be carried by the individual bearers at all times while on Yokota Air Base and Tama Annex. The contractor shall submit written requests for issuance of base passes to CO and shall be responsible for all base passes issued to his/her employees. The contractor shall return all base passes to the Japanese Pass Section at Yokota Air Base upon termination of employment or completion of the contract.

1.7.4. Special Qualifications.

1.7.4.1. Contract Manager (CM): The contractor shall provide a CM who shall be responsible for the performance of the work. The name of this person and alternate(s) who shall act for the contractor when the manager is absent, shall be designated in writing to the CO within ten (10) days after contract start date.

1.7.4.2. The CM and alternate, upon notification, shall be available during normal duty hours within two (2) hours to meet on the installation with the COR to discuss problem areas.

1.7.4.3. The CM and alternate(s) must be able to read, write, speak, and understand English, or provide a translator.

1.8. Contractor Employees. The contractor shall not employ persons for work on this contract if; he/she is identified to the contractor by the CO as a potential threat to the health, safety, security, general well-being or operational mission of the installation and its population.

1.8.1. The contractor personnel shall present a neat appearance and be easily recognized. This may be accomplished by wearing distinctive clothing bearing the name of the company or by wearing appropriate badges, which contain the company name and employee's names. The contractor shall adhere to all base safety and traffic regulations and policies.

1.8.2. The Contractor shall not employ any person who is an employee of the US Government and employing that person would create a conflict of interest. The Contractor shall not employ any person who is an employee of the

Department of the Air Force if such employment would be contrary to the policies of the Department of the Air

Force.

1.8.3. The Contractor is cautioned that off duty active military personnel hired under this contract may be subject to permanent change of station, change in duty hours or deployment. Military Reservists and National Guard members may be subject to recall to active duty. The abrupt absence of these personnel could adversely affect the

Contractor’s ability to perform under this contract; however, their absence at any time shall not constitute an excuse for nonperformance under this contract.

1.9. Damage. The contractor shall exercise care not to damage any facilities and grounds at Yokota Air Base and

Tama Annex. Any damages determined to have been caused by the contractor shall be repaired by the contractor at no expense to the Government. The contractor shall be responsible for verification of the job site and the existing condition of all serviced equipment before beginning work.

3. GOVERNMENT FURNISHED SERVICES.

3.1. The Government will furnish water in existing facilities at no cost to the contractor for the sole purpose of accomplishment of this contract.

3.2. The Government will provide, for emergency purposes only, the following services:

a. Security Police Telephone 911 or 225-7227

b. Fire Protection Telephone 911

c. Hospital Emergency (Reimbursable expense) Telephone 911

Any time the contractor uses one of the above services, the Contract Manager (CM) shall immediately report to the COR with all of the details. The CM will be required to submit, in writing, within two (2) days, the details surrounding the use of this service to the CO.

4. CONTRACTOR FULL-TIME REPORTING

4.1. The Contractor shall report ALL contractor labor hours (including subcontractor labor hours) required for performance of services provided under this contract via a secure data collection site. The Contractor is required to completely fill in all required data fields at http://www.ecmra.mil.

4.2. Reporting inputs will be for the labor executed during the period of performance for each Government fiscal year (FY), which runs 1 October through 30 September. While inputs may be reported any time during the FY, all data shall be reported no later than 31 October each calendar year. Contractors may direct questions to the CMRA help desk.

4.2.1. Reporting Period: Contractors are required to input data by 31 October of each year.

4.2.2. Uses and Safeguarding of Information: Information from the secure web site is considered to be proprietary in nature when the contract number and contractor identity are associated with the direct labor hours and direct labor dollars. At no time will any data be released to the public with the contractor name and contract number associated with the data.

4.2.3. User Manuals: Data for Air Force service requirements must be input at the Air Force CMRA link.

However, user manuals for Government personnel and contractors are available at the Army CMRA link at http://www.ecmra.mil.
