Notice to Offeror: The Government reserves the right to make award to the responsible vendor whose offer responsive and conforming to our requirements in the item description below represents the best overall value to the Government. Funds are not presently available for this effort. No award will be made under this solicitation until funds are available. The Government reserves the right to cancel this solicitation, either before or after the closing date. In the event the Government cancels this solicitation, the Government has no obligation to reimburse an offeror for any costs.

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued.

Contractor must be actively registered with the System Award Management (SAM). Lack of registration in the SAM database will make an offeror ineligible for award. The Contractor is required to confirm on an annual basis that its information in the SAM database is accurate and complete at https://www.sam.gov/.

Contractor must be registered in Wide Area Work Flow (WAWF). The WAWF system is the method to electronically process vendor payment requests and receiving reports, at http://wawf.eb.mil/. (Authorized by DFARS 252.232-7003, Electronic Submission of Payment Requests and Receiving Reports)

The provision 52.212-1, Instructions to Offerors-Commercial Products and Commercial Services, applies to this acquisition with the following addenda:

All prospective offerors shall be required to submit the following to be considered responsive:

A completed version of this combo with page one filled out completely.

A quote with all of the items required in exhibit A with part numbers, quantities, and prices for each item.

A training volume that includes a rough outline/agenda addressing operational, patient care, and servicing and maintenance training, the experience of the trainers demonstrating practical experience with the hyperbaric chambers. The training volume shall address all of the items listed in the statement of work regarding training, along with the duration of each training. This volume shall not exceed ten pages.

A completed CDRL, which is attached to this solicitation.

Offerors may indicate the delivery terms on either the combo or the quote with all items in exhibit A.

The provision 52.212-2, Evaluation-Commercial Products and Commercial Services, applies to this acquisition with the following addenda:

(a) The Government will award a contract resulting from this solicitation to the responsible responsive offeror who provides the lowest price technically acceptable. For technical acceptability, prospective offerors shall include the following or their offer shall be determined to be technically unacceptable:

The instructors listed in the technical volume possess practical experience with the brand name hyperbaric chamber provided in this solicitation and the technical volume demonstrates their experience.

The training volume includes a rough outline/agenda of the training topics per training and the agenda covers the topics required in the statement of work.

The training volume includes the duration of the operational, patient care, and servicing and maintenance trainings and those trainings match their respective duration in the statement of work.

A completed version of the CDRL attached to this solicitation.

Delivery terms consistent with this solicitation.

A five (5) year preventative maintenance plan.

(b) Offerors shall be evaluated in the following order:

All prospective offers that are submitted timely with delivery to Misawa AB, Japan shall be ranked by price alone. The lowest priced offer shall be evaluated for technical acceptability; if the offer is found to be technically acceptable, the evaluation stops and that offer shall be offered award; all other offers shall not be considered for award. If the offer is found to be unacceptable on any of the criteria listed above, the evaluation of that offer shall end and the next lowest priced offer shall be evaluated in the same manner.

Once the lowest priced offer meets all of the criteria for an acceptable offer is identified, all evaluations shall cease and that offer shall be offered contract award.

Offerors shall include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications-Commercial Products and Commercial Services, with their offer.

The provision 52.212-4, Contract Terms and Conditions-Commercial Products and Commercial Services, applies to this acquisition.

The provision 52.212-5, Contract Terms and Conditions Required To Implement Statutes or Executive Orders-Commercial Products and Commercial Services, applies to this acquisition and the applicable clauses can be found in the provisions/clauses attachment.

Offerors shall be required to obtain any commercial warranties that are customarily offered to the commercial market or provide evidence that no such warranties are customarily offered to the commercial market.

There is not a DPAS rating assigned to this product.
